Abstract

Systems, methods and devices for managing patient medication data which include, in one implementation, receiving medication data for a patient from multiple sources; generating a list of medications for the patient by consolidating identical medication listings in the medication data; presenting the medication list to the patient in a user-interface; identifying a problem in the medication list; receiving patient input for a resolution of the problem; transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution; receiving a resolution of the problem from the health care professional; and displaying to the patient an updated medication list, updated according to the received resolution.

Claims

1. A computer implemented method for managing patient medication data comprising:

receiving medication data for a patient from multiple sources;

generating a list of medications for the patient by consolidating identical medication listings in the medication data;

presenting the medication list to the patient in a user-interface;

identifying a problem in the medication list;

receiving patient input for a resolution of the problem;

transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution;

receiving a resolution of the problem from the health care professional; and

displaying to the patient an updated medication list, updated according to the received resolution.

2. The method claim 1, wherein consolidating identical medication entries comprises:

identifying a first and second listings that are potentially identical;

receiving user input from the patient to confirm the entries are identical; and

removing the identical entries.

3. The method of claim 1,

wherein the transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution comprises:

presenting the problem in an application, and

sending a notification to the health care professional to access the application; and

wherein receiving a resolution comprises receiving the resolution via the application.

4. The method of claim 1,

wherein receiving a resolution comprises receiving a new prescription from the physician;

and

further comprising;

transmitting the prescription to a pharmacy,

displaying to the patient with the list a notification that the prescription is ready to be filled, and

receiving updated medication data indicating the prescription was filled; and

wherein displaying to the patient an updated medication list is in response to receiving the updated medication data indicating the prescription was filled; and

wherein the updated medication list comprises the updated prescription.

5. The method of claim 1,

wherein the resolution comprises an indication to remove a medication involved in the problem from the list;

and

further comprising;

removing the medication involved in the problem from the list in response to receiving the resolution, and

presenting an indication to the patient to stop taking the removed medication.

6. The method of claim 1, wherein identifying a problem in the medication list comprises identifying a first medication on the list that is a duplicate medication with a second medication on the list.

7. The method of claim 1, wherein identifying a problem in the medication list comprises identifying a first medication and a second medication on the list as similar medications.

8. The method of claim 1, wherein identifying a problem in the medication list comprises identifying a medication conflict on the list.

9. The method of claim 1,

wherein the medication data comprises prescription data and non-prescription data from the patient;

wherein identifying a problem in the medication list comprising identifying a conflict between a medication corresponding to the non-prescription data and a prescription medication corresponding to the prescription data.

10. The method of claim 1, wherein receiving prescription data from a patient from multiple sources comprises:

receiving prescription data from electronic medical records affiliated with a health care professional, from a pharmacy benefit management database, and from patient entered data.

11. The method claim 1, further comprising accumulating points for an incentive program for the patient when a patient input for a resolution of the problem is received, wherein the points can be redeemed by the patient.

12. A computer readable medium encoded with instructions that when executed by a data processing apparatus cause the data processing apparatus to perform operations comprising:

receiving medication data for a patient from multiple sources;

generating a list of medications for the patient by consolidating identical medication listings in the medication data;

presenting the medication list to the patient in a user-interface;

identifying a problem in the medication list;

receiving patient input for a resolution of the problem;

transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution;

receiving a resolution of the problem from the health care professional; and

displaying to the patient an updated medication list, updated according to the received resolution.

13. The computer readable medium of claim 12, wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

identifying a first listing and a second listing that are potentially identical;

receiving user input from the patient to confirm the entries are identical; and

removing the identical entries.

14. The computer readable medium of claim 12,

wherein the transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution comprises:

presenting the problem in an application, and

sending a notification to the health care professional to access the application; and

wherein receiving a resolution comprises receiving the resolution via the application.

15. The computer readable medium of claim 12,

wherein receiving a resolution comprises receiving a new prescription from the physician;

and

wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising;

transmitting the prescription to a pharmacy,

displaying to the patient with the list a notification that the prescription is ready to be filled, and

receiving updated medication data indicating the prescription was filled, wherein the medication data comprises claims data; and

wherein displaying to the patient an updated medication list is in response to receiving the updated medication data indicating the prescription was filled; and

wherein the updated medication list comprises the updated prescription.

16. The computer readable medium of claim 12,

wherein the resolution comprises an indication to remove a medication involved in the problem from the list;

and

wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising;

removing the medication involved in the problem from the list in response to receiving the resolution, and

presenting an indication to the patient to stop taking the removed medication.

17. The computer readable medium of claim 12, wherein identifying a problem in the medication list comprises identifying a first medication on the list that is a duplicate medication with a second medication on the list.

18. The computer readable medium of claim 12, wherein identifying a problem in the medication list comprises identifying a first medication and a second medication on the list as similar medications.

19. The computer readable medium of claim 12, wherein identifying a problem in the medication list comprises identifying a medication conflict on the list.

20. The computer readable medium of claim 12,

wherein the medication data comprises prescription data and non-prescription data from the patient;

wherein identifying a problem in the medication list comprising identifying a conflict between a medication corresponding to the non-prescription data and a prescription medication corresponding to the prescription data.

21. The computer readable medium of claim 12, wherein receiving prescription data from a patient from multiple sources comprises:

receiving prescription data from electronic medical records affiliated with a health care professional, from a pharmacy benefit management database, and from patient entered data.

22. The computer readable medium of claim 12, wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

accumulating points for an incentive program for the patient when a patient input for a resolution of the problem is received, wherein the points can be redeemed by the patient.

23. A system for managing patient medication data comprising,

a data storage device for storing medication data received from multiple sources over one or more networks;

a computing system including processor electronics configured to perform operations comprising;

receiving and storing medication data for a patient from multiple sources;

generating a list of medications for the patient by consolidating identical medication listings in the medication data;

presenting the medication list in a data processing apparatus to the patient over a network;

identifying a problem in the medication list;

receiving patient input for a resolution of the problem over the network;

transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution;

receiving a resolution of the problem from the health care professional; and

displaying to the patient an updated medication list, updated according to the received resolution.

24. The system of claim 23,

wherein the transmitting, in response to the user input, the identified problem on the medication list to a heath care professional for resolution comprises:

presenting the problem in an application, and

sending a notification to the health care professional to access the application; and

wherein receiving a resolution comprises receiving the resolution via the application.

25. The system of claim 23,

wherein receiving a resolution comprises receiving a new prescription from the physician;

and

wherein the processor electronics are further configured to perform the operations comprising;

transmitting the prescription to a pharmacy,

displaying to the patient with the list a notification that the prescription is ready to be filled, and

receiving updated medication data indicating the prescription was filled; and

wherein displaying to the patient an updated medication list is in response to receiving the updated medication data indicating the prescription was filled; and

wherein the updated medication list comprises the updated prescription.

26. The system of claim 23,

wherein the resolution comprises an indication to remove a medication involved in the problem from the list;

and

wherein the processor electronics are further configured to perform the operations comprising;

removing the medication involved in the problem from the list in response to receiving the resolution, and

presenting an indication to the patient to stop taking the removed medication.

27. The system of claim 23, wherein identifying a problem in the medication list comprises identifying a first medication on the list that is a duplicate medication with a second medication on the list.

28. The system of claim 23, wherein identifying a problem in the medication list comprises identifying a first medication and a second medication on the list as similar medications.
