Abstract

Adventitious lung sounds indicative of lung congestion are detected using an implantable sensor. The sensor is adapted to be positioned adjacent to a pulmonary system and to output signals indicative of lung sounds in response to pulmonary system activity. A controller receives the signals and processes the signals to detect the presence of adventitious lung sounds. A respiratory cycle sensor operating in conjunction with the lung-sound sensor enables classification of an adventitious lung sound according to its time occurrence within the respiratory cycle. Posture sensing in conjunction with lung-sound sensing provides valuable additional information as to the severity of the lung congestion.

Claims

1. A system, comprising:

an implantable housing;

a sensor adapted for ambulatory operation in a patient and adapted to be positioned adjacent to a pulmonary system of the patient, the sensor adapted to output signals indicative of lung sounds;

filtering circuitry provided at the housing and adapted to filter the sensor output signals by removing signal content unrelated to adventitious lung sounds;

detection circuitry provided at the housing and adapted to receive filtered sensor output signals from the filtering circuitry, the detection circuitry further adapted to detect presence of adventitious lung sounds based on a comparison between the filtered sensor output signals and criteria indicative of adventitious lung sounds;

a memory provided at the housing and configured to store at least data indicative of adventitious lung sounds detected by the detection circuitry; and

a processor provided at the housing and coupled to the detection circuitry and the memory, the processor configured to organize the adventitious lung sounds data as log entries in the memory.

2. The system of claim 1, wherein the processor is configured to record data as log entries in the memory only in response to detection of adventitious lung sounds by the detection circuitry.

3. The system of claim 1, wherein the processor is configured to record data as log entries in the memory on a continuous basis.

4. The system of claim 1, comprising communications circuitry configured to effect communications between the processor and an external device.

5. The system of claim 1, comprising communications circuitry configured to effect communications between the processor and an external device, the processor configured to transmit log entries stored in the memory to the external device in response to a playback command.

6. The system of claim 1, comprising communications circuitry configured to effect communications between the processor and an external device, the external device comprising a user interface and the processor configured to transmit log entries stored in the memory to the external device in response to a playback command for presentation on the user interface.

7. The system of claim 1, wherein the detection circuitry is further adapted to determine severity of detected adventitious lung sounds, and the processor is further configured to include data indicative of detected adventitious lung sounds severity in the log entries.

8. The system of claim 1, comprising an implantable posture sensor coupled to the processor, wherein the processor is further configured to include data indicative of patient posture in the log entries.

9. The system of claim 1, comprising an implantable respiratory cycle sensor coupled to the processor, wherein the processor is further configured to detect portions of the respiratory cycle based on a respiratory cycle signal produced by the respiratory cycle sensor and to record the data as log entries in the memory only during a specific portion of the respiratory cycle.

10. A system, comprising:

an implantable housing;

a sensor adapted for ambulatory operation in a patient and adapted to be positioned adjacent to a pulmonary system of the patient, the sensor adapted to output signals indicative of lung sounds;

detection circuitry provided at the housing and adapted to detect presence of adventitious lung sounds;

a memory provided at the housing and configured to store at least data indicative of adventitious lung sounds detected by the detection circuitry; and

a processor provided at the housing and coupled to the detection circuitry and the memory, the processor configured to organize the adventitious lung sounds data as log entries in the memory.

11. The system of claim 10, wherein the processor is configured to record data as log entries in the memory only in response to detection of adventitious lung sounds by the detection circuitry.

12. The system of claim 10, wherein the processor is configured to record data as log entries in the memory on a continuous basis.

13. The system of claim 10, comprising communications circuitry configured to effect communications between the processor and an external device.

14. The system of claim 10, comprising communications circuitry configured to effect communications between the processor and an external device, the processor configured to transmit log entries stored in the memory to the external device in response to a playback command.

15. The system of claim 10, comprising communications circuitry configured to effect communications between the processor and an external device, the external device comprising a user interface and the processor configured to transmit log entries stored in the memory to the external device in response to a playback command for presentation on the user interface.

16. The system of claim 10, wherein the detection circuitry is further adapted to determine severity of detected adventitious lung sounds, and the processor is further configured to include data indicative of detected adventitious lung sounds severity in the log entries.

17. The system of claim 10, comprising an implantable posture sensor coupled to the processor, wherein the processor is further configured to include data indicative of patient posture in the log entries.

18. The system of claim 10, comprising an implantable respiratory cycle sensor coupled to the processor, wherein the processor is further configured to detect portions of the respiratory cycle based on a respiratory cycle signal produced by the respiratory cycle sensor and to record the data as log entries in the memory only during a specific portion of the respiratory cycle.

19. A method, comprising:

implantably sensing lung sounds for an ambulatory patient and producing output signals indicative of sensed lung sounds;

implantably detecting adventitious lung sounds using the output signals;

storing at least data indicative of detected adventitious lung sounds in an implantable memory; and

implantably organizing the adventitious lung sounds data as log entries in the memory.

20. The method of claim 19, comprising transmitting log entries stored in the memory to an external location in response to a playback command.
