Abstract

A device and method for treating a subject is provided where electrical stimulation may be separately provided to each hemidiaphragm to cause diaphragm contraction. Fatigue may be avoided among other possible benefits. Each hemidiaphragm may also be stimulated with different stimulation parameters. Stimulation may be smoothly transitioned between hemidiaphragms. Such stimulation may provide an increase in functional residual capacity or other therapeutic effects.

Claims

1. A method of treating a subject who has been diagnosed with central sleep apnea, comprising:

providing a first burst of electrical stimulation pukes to a diaphragm or phrenic nerve tissue such that a diaphragm contraction is created and at least one airway is stabilized; and

subsequently providing a second burst of electrical stimulation pulses to the diaphragm or phrenic nerve tissue to elicit at least one breath.

2. The method of claim 1 wherein the first electrical stimulation is provided after detection of a decreased respiratory drive.

3. The method of claim 1 wherein the second electrical stimulation is provided after detection of central apnea.

4. The method of claim 1 wherein at least a portion of the first stimulation overlaps at least a portion of the second stimulation.

5. The method of claim 1 wherein prior to providing a first burst, detecting at least one sleep disorder detection criteria via polysomnography.

6. The method of claim 5 further comprising programming a controller with the sleep disorder detection criteria received from polysomnography to provide a disorder-specific electrical stimulation parameters.

7. A method for treating a subject having a respiratory disorder, comprising:

detecting a specific respiratory disorder; and

providing disorder-specific stimulation parameters to elicit a diaphragm contraction and thereby elicit a lung volume change to treat disorder breathing.

8. The method of claim 7 wherein the subject has a central sleep apnea disorder.

9. The method of claim 7 wherein the subject has a flow limitation breathing disorder.

10. The method of claim 7 wherein the subject has an obstructive breathing disorder.

11. The method of claim 7 wherein the subject has an unstable breathing disorder.

12. The method of claim 7 wherein the subject has a mixed apnea breathing disorder.

13. A method for treating a subject with breathing disorders comprising;

providing a first burst of electrical stimulation pulses to elicit a diaphragm contraction and thereby elicit a lung volume bias and stabilize the airways immediately prior to or after;

providing a second therapeutic electrical stimulation to elicit a diaphragm contraction wherein the second therapeutic electrical stimulation is selected from a group comprising;

breathing augmentation stimulation, deep inspiration therapy, increasing inspiration duration; and

manipulating duty cycle of inspiration with respect to a respiration cycle.

14. The method of claim 13 further comprising the step of providing the first electrical stimulation during the second electrical stimulation.

15. A device for treating a subject having central sleep apnea, comprising:

at least one electrode in communication with a diaphragm or phrenic nerve tissue;

an electrical stimulator Which is programmed to provide a first burst of electrical stimulation pulses through the at least one electrode to the diaphragm or phrenic nerve tissue to cause the diaphragm to contract to stabilize at least one airway; and

,

wherein the electrical stimulator is further programmed to subsequently provide a second burst of electrical stimulation pulses to the diaphragm or phrenic nerve tissue to elicit at least one breath.

17. The device of claim 15 further comprising a polysomnography monitor configured to detect at least one sleep disorder detection criteria.

18. The device of claim 17 wherein the electrical stimulator is further programmed with the sleep disorder detection criteria received from the polysomnography monitor to provide a disorder-specific electrical stimulation parameter.
