Abstract

A medical device and associated method for discriminating cardiac events includes determining whether a first match score is within a first match zone having a first correlation with a non-treatable cardiac event, a second match zone having a second correlation with the non-treatable cardiac event less than the first correlation with the non-treatable event, a third match zone having a first correlation with a treatable cardiac event, and a fourth match zone having a second correlation with the treatable cardiac event greater than the first correlation with the treatable cardiac event. A determination is made as to whether a second match score and a third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone to generate a first adjusting factor, and cardiac event evidence is accumulated in response to the first adjusting factor for discriminating cardiac events.

Claims

1. A method of discriminating cardiac events, comprising:

sensing cardiac signals along a first sensing vector and a second sensing vector different from the first sensing vector;

determining a first match score corresponding to a first cardiac signal sensed along the first sensing vector;

determining whether the first match score is within a first match zone having a first correlation with a non-treatable cardiac event, a second match zone having a second correlation with the non-treatable cardiac event less than the first correlation with the non-treatable event, a third match zone having a first correlation with a treatable cardiac event, and a fourth match zone having a second correlation with the treatable cardiac event greater than the first correlation with the treatable cardiac event;

determining, in response to the first match score being within one of the second match zone and the third match zone, a second match score corresponding to a second cardiac signal sensed along the first sensing vector and a third match score corresponding to a third cardiac signal sensed along the second sensing vector;

determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone to generate a first adjusting factor;

accumulating cardiac event evidence in response to the first adjusting factor; and

discriminating cardiac events in response to the accumulated cardiac evidence.

2. The method of claim 1, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector.

3. The method of claim 1, further comprising determining whether the first cardiac signal is an event onset beat, wherein both the second cardiac signal and the third cardiac signal precede the first cardiac signal in response to the first cardiac signal being an event onset beat.

The method of claim 1, wherein accumulating cardiac event evidence comprises performing one of increasing and decreasing an event counter in response to determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone.

5. The method of claim 4, wherein accumulating cardiac event evidence comprises:

increasing the event counter by a first value in response to both the second match score and the third match score being within the fourth match zone; and

increasing the event counter by a second value less than the first value in response to at least one of the second match score and the third match score not being within the fourth match zone.

The method of claim 5, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector, and wherein the treatable cardiac event corresponds to a ventricular tachycardia and the non-treatable cardiac event corresponds to a supraventricular tachycardia.

7. The method of claim 1, wherein the first cardiac signal, the second cardiac signal and the third cardiac signal are spatially located across approximately a full duration of a predetermined sensing window, the method further comprising:

determining a beat feature corresponding to less than the full duration of the sensing window to generate a second adjusting factor; and

accumulating cardiac event evidence in response to both the first adjusting factor and the second adjusting factor;

8. The method of claim 7, wherein the beat feature is determined for the first cardiac signal sensed along the first sensing vector.

The method of claim 7, wherein accumulating cardiac event evidence comprises performing one of increasing and decreasing an event counter in response to both the determined beat feature and the determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone.

10. The method of claim 9, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector, the method further comprising:

determining whether the first cardiac signal is an event onset beat, wherein both the second cardiac signal and the third cardiac signal precede the first cardiac signal in response to the first cardiac signal being an event onset beat;

increasing the event counter by a first value in response to the beat feature and both the second match score and the third match score being within the fourth match zone; and

increasing the event counter by a second value less than the first value in response one or both the second match score and the third match score not being within the fourth match zone.

11. The method of claim 10, wherein the treatable cardiac event corresponds to a ventricular tachycardia and the non-treatable cardiac event corresponds to a supraventricular tachycardia.

13. The device of claim 11, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector.

12. A medical device for discriminating cardiac events, comprising:

a plurality of electrodes sensing cardiac signals along a first sensing vector and a second sensing vector different from the first sensing vector; and

a processor configured to determine a first match score corresponding to a first cardiac signal sensed along the first sensing vector, determine whether the first match score is within a first match zone having a first correlation with a non-treatable cardiac event, a second match zone having a second correlation with the non-treatable cardiac event less than the first correlation with the non-treatable event, a third match zone having a first correlation with a treatable cardiac event, and a fourth match zone having a second correlation with the treatable cardiac event greater than the first correlation with the treatable cardiac event, determine, in response to the first match score being within one of the second match zone and the third match zone, a second match score corresponding to a second cardiac signal sensed along the first sensing vector and a third match score corresponding to a third cardiac signal sensed along the second sensing vector, determine whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone to generate a first adjusting factor, accumulate cardiac event evidence in response to the first adjusting factor, and discriminate cardiac events in response to the accumulated cardiac evidence.

The device of claim 12, wherein the processor is further configured to determine whether the first cardiac signal is an event onset beat, wherein both the second cardiac signal and the third cardiac signal precede the first cardiac signal in response to the first cardiac signal being an event onset beat.

The device of claim 12, wherein the processor is further configured to perform one of increasing and decreasing an event counter in response to determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone.

16. The device of claim 15, wherein the processor is further configured to increase the event counter by a first value in response to both the second match score and the third match score being within the fourth match zone, and increase the event counter by a second value less than the first value in response to at least one of the second match score and the third match score not being within the fourth match zone.

17. The device of claim 16, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector, and wherein the treatable cardiac event corresponds to a ventricular tachycardia and the non-treatable cardiac event corresponds to a supraventricular tachycardia.

18. The device of claim 12, wherein the first cardiac signal, the second cardiac signal and the third cardiac signal are spatially located across approximately a full duration of a predetermined sensing window, and wherein the processor is further configured to determine a beat feature corresponding to less than the full duration of the sensing window to generate a second adjusting factor, and accumulate cardiac event evidence in response to both the first adjusting factor and the second adjusting factor;

19. The device of claim 18, wherein the beat feature is determined for the first cardiac signal sensed along the first sensing vector.

20. The device of claim 18, wherein the processor is further configured to perform one of increasing and decreasing an event counter in response to both the determined beat feature and the determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone.

21. The device of claim 20, wherein the first sensing vector corresponds to a far field sensing vector and the second sensing vector corresponds to a near field sensing vector, and wherein the processor is further configured to determine whether the first cardiac signal is an event onset beat, increase the event counter by a first value in response to the beat feature and both the second match score and the third match score being within the fourth match zone, and increase the event counter by a second value less than the first value in response one or both the second match score and the third match score not being within the fourth match zone, wherein both the second cardiac signal and the third cardiac signal precede the first cardiac signal in response to the first cardiac signal being an event onset beat.

22. The device of claim 21, wherein the treatable cardiac event corresponds to a ventricular tachycardia and the non-treatable cardiac event corresponds to a supraventricular tachycardia.

23. A computer readable medium having computer executable instructions for performing a method, the method comprising:

sensing cardiac signals along a first sensing vector and a second sensing vector different from the first sensing vector;

determining a first match score corresponding to a first cardiac signal sensed along the first sensing vector;

determining whether the first match score is within a first match zone having a first correlation with a non-treatable cardiac event, a second match zone having a second correlation with the non-treatable cardiac event less than the first correlation with the non-treatable event, a third match zone having a first correlation with a treatable cardiac event, and a fourth match zone having a second correlation with the treatable cardiac event greater than the first correlation with the treatable cardiac event;

determining, in response to the first match score being within one of the second match zone and the third match zone, a second match score corresponding to a second cardiac signal sensed along the first sensing vector and a third match score corresponding to a third cardiac signal sensed along the second sensing vector;

determining whether the second match score and the third match score are within the first match zone, the second match zone, the third match zone and the fourth match zone to generate a first adjusting factor;

accumulating cardiac event evidence in response to the first adjusting factor; and

discriminating cardiac events in response to the accumulated cardiac evidence.
