Abstract

In a method for validating data, a text of a document is received. At least one fact is extracted from the text. At least one expert refinement is merged with the at least one fact to create at least one modified fact. The at least one modified fact is provided for a review. An expert refinement to the at least one modified fact is captured in response to the review. A superset document based on the at least one pre-existing refinement and the expert refinement is stored.

29. A method comprising:

extracting, using at least one processor, a first medical fact from a first portion of a free-form text narrative provided by a health care provider; and

providing to a user an indicator that indicates a linkage between the first medical fact and the first portion of the free-form text narrative.

30. The method of claim 29, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, visually identifying the first portion of the free-form text narrative.

31. The method of claim 30, wherein visually identifying the first portion of the free-form text narrative comprises highlighting the first portion of the free-form text narrative.

32. The method of claim 29, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, scrolling a display of the free-form text narrative to the first portion of the free-form text narrative.

33. The method of claim 29, further comprising visually distinguishing the first portion of the free-form text narrative from one or more surrounding portions of the free-form text narrative.

34. The method of claim 29, further comprising:

including the first medical fact in a patient record; and

indicating a linkage between the first medical fact in the patient record and the first portion of the free-form text narrative.

35. The method of claim 29, wherein the user is the health care provider.

36. The method of claim 29, wherein the user is a person other than the health care provider.

37. Apparatus comprising:

at least one processor; and

a memory storing processor-executable instructions that, when executed by the at least one processor, perform a method comprising;

extracting a first medical fact from a first portion of a free-form text narrative provided by a health care provider; and

providing to a user an indicator that indicates a linkage between the first medical fact and the first portion of the free-form text narrative.

38. The apparatus of claim 37, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, visually identifying the first portion of the free-form text narrative.

39. The apparatus of claim 38, wherein visually identifying the first portion of the free-form text narrative comprises highlighting the first portion of the free-form text narrative.

40. The apparatus of claim 37, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, scrolling a display of the free-form text narrative to the first portion of the free-form text narrative.

41. The apparatus of claim 37, wherein the method further comprises visually distinguishing the first portion of the free-form text narrative from one or more surrounding portions of the free-form text narrative.

42. The apparatus of claim 37, wherein the method further comprises:

including the first medical fact in a patient record; and

indicating a linkage between the first medical fact in the patient record and the first portion of the free-form text narrative.

43. At least one computer-readable storage medium encoded with a plurality of computer-executable instructions that, when executed, perform a method comprising:

extracting a first medical fact from a first portion of a free-form text narrative provided by a health care provider; and

providing to a user an indicator that indicates a linkage between the first medical fact and the first portion of the free-form text narrative.

44. The at least one computer-readable storage medium of claim 43, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, visually identifying the first portion of the free-form text narrative.

45. The at least one computer-readable storage medium of claim 44, wherein visually identifying the first portion of the free-form text narrative comprises highlighting the first portion of the free-form text narrative.

46. The at least one computer-readable storage medium of claim 43, wherein providing the indicator comprises:

in response to a selection by the user of the first medical fact, scrolling a display of the free-form text narrative to the first portion of the free-form text narrative.

47. The at least one computer-readable storage medium of claim 43, wherein the method further comprises visually distinguishing the first portion of the free-form text narrative from one or more surrounding portions of the free-form text narrative.

48. The at least one computer-readable storage medium of claim 43, wherein the method further comprises:

including the first medical fact in a patient record; and

indicating a linkage between the first medical fact in the patient record and the first portion of the free-form text narrative.
