Abstract

A method for providing a consumer with the ability to access and collect health records associated with the consumer through use of a consumer account is provided. The method includes assigning a destination address individually associated with the consumer account for receiving communications from at least one healthcare provider, associating access information with the consumer account for the consumer to use to access a secure web site, the communications including a health record associated with the consumer for which the consumer has requested and given permission to the healthcare provider to send, storing a representation of the health record, and providing the consumer with secure access to the web site using the access information and providing on the web site an interface to the health records of the consumer for the consumer to use. The destination address can be a phone number or an email address.

Claims

1. A method for providing a health record service to a consumer, the service providing the consumer with the ability to access and collect health records associated with the consumer through use of a consumer account, the method comprising:

assigning a destination address individually associated with the consumer account for receiving communications from at least one healthcare providers, the step of assigning performed through a web site;

associating access information with the consumer account for the consumer to use to access the web site stored on a web server using a web-enabled device;

receiving a communication from one of the at least one health care providers at the web site, the communication directed to the destination address, the communication comprising a health record associated with the consumer for which the consumer has requested and given permission to the healthcare provider to send, the health record from the set consisting of a patient chart, a birth certificate, a vaccination record, a medical history, a lab report, a test result, a prescription, an x-ray result or image, a scan, and an EKG result or image;

storing a representation of the health record, the representation accessible by the web server and through the web site;

providing the consumer with secure access to the web site using the access information and providing on the web site an interface to the health records of the consumer for the consumer to use.

The method of claim 1 further comprising creating through the web site a document for the user to provide to the service providers requesting the service providers to send the records to the destination address and granting the service provider permission to send the records to the destination address.

3. The method of claim 1 wherein the destination address is a phone number.

4. The method of claim 3 wherein the phone number is a toll-free phone number.

5. The method of claim 1 wherein the destination address is an email address.

6. The method of claim 1 wherein the access information includes a first set of access information for providing a first level of security and at least one second set of access information for providing an additional level of security.

7. The method of claim 1 further comprising receiving an additional health record from the consumer uploaded to the web site.

8. The method of claim 1 wherein the destination address is a phone number adapted for accepting both fax communications and voice communications without entry of a personal identification number (PIN) associated with the consumer.

9. The method of claim 1 further comprising providing the consumer with functionality for uploading additional health records to the web site.

10. The method of claim 10 wherein the destination address is a phone number and wherein the method further comprises:

creating through the web site an email address individually associated with the consumer and associating the email address with the consumer;

receiving an additional health record addressed to the email address at the web site; and

providing the consumer access to the additional health record through the web site.

11. A system for storing health records, the system comprising:

a fax server for receiving a fax from a health care provider and converting at least a portion of the fax into a an image;

a web server in operative communication with the fax server and configured to receive the image and store the image as a health record associated with a user account;

wherein the web server being adapted to provide the user with access to view, store, and delete the health record;

wherein the web server being further adapted to receive uploaded health records uploaded by the user to the user account;

wherein the web server being further adapted to receive emailed health records emailed to an email address associated with the user account.

12. The system of claim 11 wherein the web server being further adapted to perform an analysis using the health record.

13. The system of claim 12 wherein the health record is a prescription.

14. The system of claim 11 wherein the web server being further configured to assign an individual fax number to the user account wherein faxes to the individual fax number are received at the fax server and wherein the individual fax number is used in determining the user account associated with the fax.

15. The system of claim 14 wherein the web server further provides for generating a document to provide to a health care provider exercising legal rights of a patient for access to health records, the document requesting the health care provider to send the health records to the individual fax number associated with the user account.

16. The system of claim 14 wherein the web server being further configured to assign an email address to the user account.

17. The system of claim 16 wherein the web server further provides for generating a document to provide to a health care provider exercising legal rights of a patient for access to health records, the document requesting the health care provider to send the health records to email the email address.

18. The system of claim 11 wherein the web server further provides a web site interface for organizing and annotating health records into separate folders with functionality for naming and adding file folders.

19. The system of claim 18 further comprising an emergency access card access information and adapted for use by a third-party to access the user account in event of a medical emergency associated with the patient.
