Abstract

A method for providing a user with a web-based service to access and collect records associated with the user in a secure and private manner, the method including assigning through a web site a phone number, assigning through the web site a user account to the user, associating access information with the user, creating through the web site a document for the user to provide to the service provider requesting the service provider to send the records to the phone number and granting the service provider permission to send the records to the phone number, receiving by the service provider the document from the user to provide the service provider permission to send the records to the phone number, and sending by the service provider a private fax communication to the phone number.

Claims

1. A method for providing a patient with the ability to access and collect personal health records associated with the patient in a secure and private manner, the method comprising:

associating access information with the patient for the patient to use to access a server storing files comprising personal health records of the patient in a computer readable storage medium;

providing the patient with a web-based user interface on a web-enabled device to remotely access the personal health records stored on the server, the web-based user interface further providing for;

(a) managing communications with health care providers, (b) managing prescriptions, and (c) providing emergency access;

receiving at the server the files comprising personal health records associated with the patient from a health care provider providing services to the patient;

receiving at the server a request from the patient made through the user interface of the web-enabled device for access to the files, wherein the access information is associated with the request;

sending the patient a file containing a personal health record of the patient from the server to the web-enabled device in response to the request;

wherein the files are maintained on the server separately from any files maintained by the health care provider and wherein the files are managed privately by the patient.

2. The method of claim 1 further comprising receiving at the server a request from the patient made through the user interface of the web-enabled device to send the file containing the personal health record of the patient from the server to a second health care provider.

3. The method of claim 2 further comprising sending the second health care provider the file containing the personal health record of the patient.

4. The method of claim 1 wherein the access information comprises a password.

5. The method of claim 1 wherein the web-based user interface further provides for organizing the files into separate file folders.

6. The method of claim 4 wherein the web-based user interface further provides for providing functionality for the patient to name the file folders.

7. The method of claim 1 wherein the files comprise image files associated with the patient.

8. The method of claim 1 wherein the files comprise laboratory test results.

9. The method of claim 1 further comprising electronically sending a reminder to the patient.

10. The method of claim 1 wherein the communications with the health care providers comprise at least one of email, faxes, and voice messages.

11. The method of claim 1 further comprising receiving at the server additional files comprising personal health records of the patient from a second health care provider of the patient.

12. The method of claim 1 further comprising receiving at the server a file upload of an additional file containing a personal health record of the patient.

13. The method of claim 1 wherein the access information includes a first set of access information for providing a first level of security and at least one second set of access information for providing an additional level of security.

14. The method of claim 1 wherein the health care provider is a hospital.

15. The method of claim 1 further comprising collecting insurance information associated with the patient through the web-based user interface.

30. The method of claim 1 further comprising collecting insurance information associated with the patient through the web-based user interface.

16. A method for providing a patient with a service to access and collect health records associated with the patient in a private manner, comprising:

providing a web-based system, the web-based system providing means for;

(a) maintaining a destination address associated with a patient for communications from a health care provider;

(b) associating access information with the patient for the patient to use to access a web site hosted on a web server using a web-enabled device;

(c) providing the patient with a web-based user interface on the web-enabled device to remotely access the health records stored on the server, the web-based user interface further providing for;

(1) managing communications with health care providers, (2) managing prescriptions, and (3) providing emergency access;

receiving authorization from the patient to send the patient protected health information to the web-based system; and

sending the patient protected health information by sending health records to the web-based system.

17. The method of claim 16 wherein the sending the health records to the web-based system comprises sending the health records via fax to the destination address wherein the destination address is a phone number.

18. The method of claim 16 wherein the web-based system further provides means for receiving direct upload of health records from the patient.

19. The method of claim 16 wherein the health care providers comprise a hospital.

20. The method of claim 16 wherein the patient protected health information comprises images.

21. The method of claim 16 wherein the patient protected health information comprises test results.

22. The method of claim 16 wherein the access information includes a first set of access information for providing a first level of security and at least one second set of access information for providing an additional level of security.

23. The method of claim 16 wherein the web-based system further provides for creating an authorization form to grant the authorization from the patient to send the patient protected health information to the web-based system.

24. The method of claim 16 wherein the health records maintained on the web-based system are maintained on the server separately from any files maintained by the health care provider.

25. A method for providing a patient with the ability to meaningfully access health records associated with the patient in a secure and private manner, the method comprising:

associating access information with the patient for the patient to use to access a server storing files comprising health records of the patient in a computer readable storage medium;

providing the patient with a web-based user interface on a web-enabled device to remotely access the health records stored on the server, the web-based user interface further providing for;

(a) managing communications with health care providers, (b) managing prescriptions, and (c) providing emergency access;

receiving at the server the files comprising health records associated with the patient from a health care provider providing services to the patient;

receiving at the server a request from the patient made through the user interface of the web-enabled device for access to the files, wherein the access information is associated with the request;

sending the patient a file containing a health record of the patient from the server to the web-enabled device in response to the request; and

wherein the files are maintained on the server separately from any files maintained by the health care provider and wherein the files are managed privately by the patient using the web-based user interface.

26. The method of claim 25 further comprising receiving at the server a request from the patient made through the user interface of the web-enabled device to send the file containing the health record of the patient from the server to a second health care provider.

27. The method of claim 26 further comprising sending the second health care provider the file containing the health record of the patient.

28. The method of claim 25 wherein the web-based user interface further provides for organizing the files into separate file folders.

29. The method of claim 25 further comprising electronically sending a reminder to the patient.
