A system for decentralized collection and distribution of medical information through mobile devices. End users interact with the system through a software application installed on a mobile device. Data entered to the application is synced to a server that backs up the data and serves to populate a database of personal health data for analysis. Results of such analyses create value through medical management and informational intervention strategies tailored to meet evidence-based needs and preferences. Based on the analyses, end users and clinicians have the ability to visualize dynamics in health metrics relative to thresholds and to assess the efficacy of therapies and trends in vital signs/labs/vital functions. Functionalities of the system include the ability to populate medical records of end users, inform the full range of medical decision-making opportunities without the need for further examination, maintain adherence to existing evidence-based guidelines, and support the development of new clinical knowledge.
