1. An extender comprising:

  • a first extension including a first wall and extending between a proximal end and a distal end, the first wall defining an axial cavity including a distal portion having a first dimension, an intermediate portion having at least one ramp and a proximal portion having a second dimension, the first dimension being greater than the second dimension, the distal end including at least one fixation surface; and

    a second extension extending from adjacent the proximal end of the first extension to a distal end of the second extension, the distal end including a projection, wherein the first extension is configured for axial translation relative to the second extension such that the projection is disposable in the axial cavity in a first position such that the projection is disposed with the distal portion of the axial cavity and the distal end of the first extension is disposed in a non-expanded orientation, and the projection is slidably engageable with the at least one ramp for movement to a second position such that the projection is disposed with the proximal portion of the axial cavity and the distal end of the first extension is disposed in an expanded orientation.
