Abstract

A computer-implemented method, including storing, in an electronic healthcare record of an individual or patient, a note, comment, or message, provided by the individual, patient, or a caregiver, transmitting the note, comment, or message, to a device used by a provider, receiving information regarding the individual or patient from the provider, processing the information from the provider, generating a report containing information regarding provider information, an examination or procedure finding, a diagnosis, treatment, or treatment plan, or wellness, exercise, fitness, nutritional, diet, rehabilitation, or therapy, information, wherein the report contains information regarding, or a link or hyperlink to, the electronic healthcare record or data or information contained therein, or the note, comment, or message, and transmitting the report to a communication device used by the individual, the patient, the caregiver, the provider or a second provider, or an insurer or payer of the individual, the patient, or the caregiver.

Claims

1. A computer-implemented method, comprising:

receiving, with a first receiver, and storing in an electronic healthcare record of an individual or a patient, a note, a comment, or a message, wherein the note, the comment, or the message, is transmitted from a first communication device, wherein the first communication device is associated with or is used by an individual, a patient, or a caregiver of or for an individual or patient;

transmitting, with or from a transmitter or a computer, the note, the comment, or the message, to a second communication device, wherein the second communication device is associated with or is used by a provider;

receiving, with the first receiver or the computer, information regarding the individual or the patient, an examination finding, a procedure finding, a diagnosis, a treatment, a treatment plan, or information provided by the provider, wherein the information regarding the individual or the patient, an examination finding, a procedure finding, a diagnosis, a treatment, a treatment plan, or information provided by the provider, is transmitted from the second communication device or from a third communication device, wherein the third communication device is associated with or is used by the provider;

processing, with the computer, the information regarding the individual or the patient, an examination finding, a procedure finding, a diagnosis, a treatment, a treatment plan, or information provided by the provider;

generating, with the computer, a report containing information regarding the information regarding the individual or the patient, an examination finding, a procedure finding, a diagnosis, a treatment, a treatment plan, a wellness report, an exercise report, a fitness report, a nutritional report, a diet report, a rehabilitation report, a therapy report, or information provided by the provider, wherein the report contains information regarding, a link to, or a hyperlink to, the electronic healthcare record of the individual or the patient, data or information contained in the electronic healthcare record of the individual or the patient, the note, the comment, or the message; and

transmitting, with or from the transmitter, a second transmitter, or the computer, the report to the first communication device, to the second communication device, to a third communication device, or to a fourth communication device, wherein the fourth communication device is associated with or is used by the individual, the patient, the caregiver, a second provider, or an insurer or payer of the individual, the patient, or the caregiver.

2. The computer-implemented method of claim 1, further comprising:

generating, with the computer, an insurance claim, a claim form, a request for payment, or a request for payment form; and

transmitting the insurance claim, the claim form, the request for payment, or the request for payment form, to the fourth communication device or to a fifth communication device, wherein the fifth communication device is associated with or is used by an insurer or payer of the individual, the patient, or the caregiver.

The computer implemented method of claim 2, wherein the insurance claim, the claim form, the request for payment, or the request for payment form, contains information contained or included in, or a link or a hyperlink to information contained in or included in, the electronic healthcare record of the individual or the patient.

4. The computer-implemented method of claim 1, further comprising:

storing the report in the electronic healthcare record of the individual or the patient.

5. The computer-implemented method of claim 2, further comprising:

transmitting the insurance claim, the claim form, the request for payment, or the request for payment form, to the first communication device, to the fourth communication device, or to a fifth communication device, wherein the fifth communication device is associated with or is used by the individual, the patient, or the caregiver.

6. The computer-implemented method of claim 2, further comprising:

transmitting the insurance claim, the claim form, the request for payment, or the request for payment form, to the second communication device, to the third communication device, or to a fifth communication device, wherein the fifth communication device is associated with or is used by provider or a second provider.

7. The computer-implemented method of claim 1, further comprising:

receiving a provider-provided note, comment, or message, provided by the provider, wherein the provider-provided note, comment, or message, is transmitted from the second communication device or from the third communication device; and

storing the provider-provided note, comment, or message, in the electronic healthcare record of the individual or the patient.

8. The computer-implemented method of claim 1, further comprising:

receiving an insurer-provided or a payer-provided note, comment, or message, provided by an insurer or a payer, insurer-provided or a payer-provided note, comment, or message, is transmitted from the fourth communication device or from a fifth communication device, wherein the fifth communication device is associated with or is used by an insurer or payer of the individual, the patient, or the caregiver.

storing the insurer-provided or a payer-provided note, comment, or message, in the electronic healthcare record of the individual or the patient.

The computer-implemented method of claim 1, wherein the electronic healthcare record of the individual or the patient contains one or more electronic healthcare records, electronic medical records, electronic dental records, electronic pharmacy records, or electronic behavioral health records, or contains a link or hyperlink to one or more electronic healthcare records, electronic medical records, electronic dental records, electronic pharmacy records, or electronic behavioral health records, or contains a link or a hyperlink to a computer or a communication device associated with or used by a provider, an insurer or a payer, an insurance company computer, or a healthcare records computer, or contains a link or a hyperlink to an insurance exchange computer, a social networking computer, or a media computer.

10. The computer-implemented method of claim 1, wherein the first communication device is a personal computer.

11. The computer-implemented method of claim 1, wherein the first communication device is a tablet or a tablet computer.

12. The computer-implemented method of claim 1, wherein the first communication device is a cellular telephone, a smart phone, or a personal digital assistant.

13. The computer-implemented method of claim 1, further comprising:

receiving information regarding or an update regarding a personal healthcare record associated with the individual or the patient;

processing the information regarding or the update regarding the personal healthcare record associated with the individual or the patient;

generating an updated personal healthcare record; and

transmitting the updated personal healthcare record to the first communication device.

14. The computer-implemented method of claim 13, further comprising:

storing the, or information contained or included in the, updated personal healthcare record in the electronic healthcare record of the individual or the patient.

15. The computer-implemented method of claim 1, wherein the report contains a link to wellness information.

16. The computer-implemented method of claim 1, further comprising:

receiving a request to create or establish the electronic healthcare record of the individual or the patient or a request to create or establish a second healthcare record of the individual or the patient; and

creating or establish the electronic healthcare record of the individual or the patient or a request to create or establish a second healthcare record of the individual or the patient.

17. The computer-implemented method of claim 1, further comprising:

processing a request by the individual or the patient, by a caregiver of the individual or patient, by a second individual or a second patient, or by a caregiver of the second individual or the second patient, to make or schedule an appointment with the provider or with the second provider; and

scheduling the appointment; and

storing information regarding the appointment; and

transmitting the information regarding the appointment to the first communication device or to a fourth communication device, wherein the fourth communication device is associated with the individual, the patient, the caregiver of the individual or the patient, the second individual, the second patient, the caregiver of the second individual or the second patient, the provider, or a second provider.

18. The computer-implemented method of claim 1, further comprising:

generating a notification message regarding an event or occurrence regarding the individual or the patient or a second individual or a second patient; and

transmitting the notification message to the second communication device, to the third communication device, to the fourth communication, or to a fifth communication device, wherein the fifth communication device is associated with or used by a provider of the second individual or the second patient.

19. The computer-implemented method of claim 1, further comprising:

determining whether the provider, a second provider, an insurer or payer, or a user, is allowed or authorized to access, obtain, modify, change, alter, or update, the electronic healthcare record of the individual or the patient or an electronic healthcare record of a second individual or a second patient; and

allowing the provider, the second provider, the insurer or payer, or the user, to access, obtain, modify, change, alter, or update, the electronic healthcare record of the individual or the patient or the electronic healthcare record of a second individual or a second patient if the provider, the second provider, the insurer or payer, or the user, is authorized or allowed.

20. The computer-implemented method of claim 19, further comprising:

processing information for undoing, negating, or voiding, an unauthorized modification, change, alteration, or update to the electronic healthcare record or to the second electronic healthcare record.
