store a first code and associate the first code with at least a portion of the first input;
after accepting the first input, accept a second input, the second input being from the patient and including the first code; and
responsively to the second input, associate the first healthcare provider and the patient in a database.

The present disclosure relates to the field of personal health data tracking, and in particular to an automated system and method for configuring and managing a patient'"'"'s account by a health care provider. One disclosed embodiment of the system includes a server and a remote interface for accessing the server through a network. The server may be a web server, and the remote interface may be a personal computer or smartphone device connected to the server via the Internet. The patient and the healthcare provider each connect to the server through their own remote interface. The healthcare provider completes an electronic form to identify the patient and select data elements for the patient to track. Upon completion of this form, a code is generated that the patient can use to automatically create a new patient account and configure its remote interface according to the healthcare provider'"'"'s selections.
