125. A system for providing continuity of care management, the system comprising:
a member device configured to communicate with one or more remote devices over a network, at least one of the member device and remote devices comprises a health service management computing system, having at least one processor, configured to; provide a care plan for a user of the member device, the care plan defining a personalized course of care for a health-related condition of the user; monitor user interaction with the member device and receive a user-related health event from the member device; determine user compliance in real- or near real-time with the care plan based, at least in part, on a comparison between the received user-related health event and one or more parameters of the care plan; identify a violation and user non-compliance with the care plan if the comparison falls outside of a defined compliance boundary; initiate a multi-tiered escalation and workflow process of the care plan in response to the identified violation; transmit, at a first tier level, an alert of the violation only to the member device; transmit, at a second tier level, the violation alert to a first remote device associated with a non-healthcare professional in a trusted care network if the violation remains after the first tier level escalation; and transmit, at a third tier level, the violation alert to a second remote device associated with a healthcare professional if the violation remains after the second tier level escalation. - View Dependent Claims (126, 127, 128, 129, 130, 131, 132, 133, 134, 135, 136, 137, 138, 139, 140)

a member device configured to communicate with one or more remote devices over a network, at least the member device comprises a health service management computing system, having at least one processor, configured to; provide a care plan for a user of the member device, the care plan defining a personalized course of care for a health-related condition of the user; monitor user interaction with the member device and receive a user-related health event from the member device; determine user compliance in real- or near real-time with the care plan based, at least in part, on a comparison between the received user-related health event and one or more parameters of the care plan; identify a violation and user non-compliance with the care plan if the comparison falls outside of a defined compliance boundary; initiate a multi-tiered escalation and workflow process of the care plan in response to the identified violation; transmit, at a first tier level, an alert of the violation only to the member device; transmit, at a second tier level, the violation alert to a first remote device associated with a non-healthcare professional in a trusted care network if the violation remains after the first tier level escalation; and transmit, at a third tier level, the violation alert to a second remote device associated with a healthcare professional if the violation remains after the second tier level escalation; and a medical device for providing treatment to the user and an associated sensor communicatively coupled to the member device and configured to monitor user characteristics selected from the group consisting of user physiological characteristics, user vital signs, user blood analysis, and user movement; wherein the health service management computing system is configured to compare sensed user characteristics with a set of defined characteristics associated with the care plan and control operation of the medical device to provide a treatment based on the comparison.
