Abstract

A device is provided for closing a wound, such as a percutaneous access site. The device is inserted into the wound, and a helically shaped pusher is actuated to penetrate a wall of the wound. The pusher extends a suture through the tissue wall. After the suture is extending through the tissue wall, the pusher is retracted out of the tissue, while the suture remains extending through the tissue. The suture may then be used to close the wound.

Claims

1. A wound closure device, comprising:

a pusher comprising a distal portion with a helical shape;

a housing supporting said pusher, said distal portion of said pusher being extendable along a helical path exterior from said housing;

a sharp tip disposed at a distal end of said distal portion of said pusher; and

a suture portion engaged with said pusher;

wherein said pusher is configured to push said sharp tip along said helical path to penetrate body tissue, said suture thereby being pulled through said body tissue along said helical path, said suture thereafter being separable from said pusher, and said pusher being retractable from said body tissue along said helical path to leave said suture extending through said body tissue.

The wound closure device according to claim 1, further comprising another pusher, another sharp tip, and another suture portion, said another pusher being oriented relative to said pusher to penetrate body tissue generally opposite from said pusher, said suture portion and said another suture portion thereby being adapted to pull together generally opposite walls of said body tissue defining a wound.

3. The wound closure device according to claim 2, wherein said pusher and said another pusher rotate in opposite directions.

4. The wound closure device according to claim 2, wherein said helical paths of said pusher and said another pusher circumferentially overlap each other.

5. The wound closure device according to claim 2, wherein said distal ends of said pusher and said another pusher are longitudinally offset from each other.

6. The wound closure device according to claim 2, wherein said helical shapes of said distal portions of said pusher and said another pusher each define an axis, said axes being nonparallel and being angled outward with respect to each other toward said distal ends.

7. The wound closure device according to claim 6, wherein an included angle between said axes is about 8°

to about 16°

.

The wound closure device according to claim 2, wherein said pusher and said another pusher rotate in opposite directions, said helical paths of said pusher and said another pusher circumferentially overlap each other, and said helical shapes of said distal portions of said pusher and said another pusher each define an axis, said axes being nonparallel and being angled outward with respect to each other toward said distal ends.

9. The wound closure device according to claim 1, wherein said pusher rotates about 180°

to about 240°

as said sharp tip penetrates said body tissue and as said pusher retracts from said body tissue.

10. The wound closure device according to claim 1, wherein said suture portion is attached to said sharp tip and said sharp tip is separable from said pusher.

11. The wound closure device according to claim 10, wherein said sharp tip comprises a first catch and said housing comprises a second catch configured to retain said sharp tip after penetrating said body tissue, said sharp tip thereby separating from said pusher as said pusher is retracted.

The wound closure device according to claim 1, wherein said housing comprises a first port, said sharp tip being disposed within said housing before penetrating said body tissue, and said sharp tip and said distal portion of said pusher extending through said first port to penetrate said body tissue.

The wound closure device according to claim 12, wherein said suture portion is attached to said sharp tip and said sharp tip is separable from said pusher, said housing comprises a second port, said sharp tip being received within said second port after penetrating said body tissue, said sharp tip remaining in said second port after penetrating said body tissue, and said sharp tip thereby separating from said pusher as said pusher is retracted.

14. The wound closure device according to claim 12, wherein said suture portion extends through said first port as said pusher extends through said body tissue.

15. The wound closure device according to claim 1, wherein said suture portion extends away from said distal portion of said pusher along an exterior of said housing.

16. The wound closure device according to claim 1, wherein said distal portion of said pusher comprises a longitudinal cavity, said suture portion being disposable within said longitudinal cavity as said pusher extends through said body tissue.

The wound closure device according to claim 16, wherein said distal portion of said pusher comprises a longitudinal opening along an outer edge thereof in communication with said longitudinal cavity, said suture portion passing out of said longitudinal cavity through said longitudinal opening as said pusher is retracted.

18. The wound closure device according to claim 1, wherein said housing comprises a tapered portion, said helical path of said pusher extending along at least a portion of said tapered portion.

The wound closure device according to claim 1, further comprising another pusher, another sharp tip, and another suture portion, said another pusher being oriented relative to said pusher to penetrate body tissue generally opposite from said pusher, said suture portion and said another suture portion thereby being adapted to pull together generally opposite walls of said body tissue defining a wound, wherein said pusher and said another pusher each rotate about 180°

to about 240°

as said sharp tip and said another sharp tip penetrate said body tissue and as said pusher and said another pusher retract from said body tissue, said suture portion is attached to said sharp tip and said sharp tip is separable from said pusher, and said another suture portion is attached to said another sharp tip and said another sharp tip is separable from said another pusher.

The wound closure device according to claim 19, wherein said pusher and said another pusher rotate in opposite directions, said housing comprises a first port, said sharp tip being disposed within said housing before penetrating said body tissue, and said sharp tip and said distal portion of said pusher extending through said first port to penetrate said body tissue, and said housing comprises another first port, said another sharp tip being disposed within said housing before penetrating said body tissue, and said another sharp tip and said distal portion of said another pusher extending through said another first port to penetrate said body tissue.

21. The wound closure device according to claim 20, wherein said housing comprises a second port, said sharp tip comprises a first catch and said housing comprises a second catch in said second port, said second catch retaining said sharp tip after penetrating said body tissue, said sharp tip thereby separating from said pusher as said pusher is retracted, said housing comprises another second port, said another sharp tip comprises another first catch and said housing comprises another second catch in said another second port, said another second catch retaining said another sharp tip after penetrating said body tissue, said another sharp tip thereby separating from said another pusher as said another pusher is retracted, said distal portions of said pusher and said another pusher each comprise a longitudinal cavity with a longitudinal opening along an outer edge thereof, said suture portion and said another suture portion being disposable within said longitudinal cavities as said pusher and said another pusher extend through said body tissue, and said suture portion and said another suture portion passing out of said longitudinal cavities through said longitudinal openings as said pusher and said another pusher are retracted, and said housing comprises a tapered portion, said helical paths of said pusher and said another pusher extending along at least a portion of said tapered portion.
