Abstract

The present invention discloses devices and methods for performing intravascular procedures without cardiac bypass. The devices include various embodiments of temporary filter devices, temporary valves, and prosthetic valves.

The temporary filter devices have one or more cannulae which provide access for surgical tools for effecting repair of the cardiac valves. A cannula may have filters of various configurations encircling the distal region of the cannula, which prevent embolitic material from entering the coronary arteries and aorta.

The temporary valve devices may also have one or more cannulae which guide the insertion of the valve into the aorta. The valve devices expand in the aorta to occupy the entire flow path of the vessel. In one embodiment, the temporary valve is a disc of flexible, porous, material that acts to filter blood passing therethrough. A set of valve leaflets extend peripherally from the disc. These leaflets can alternately collapse to prevent blood flow through the valve and extend to permit flow.

The prosthetic valves include valve fixation devices which secure the prosthetic valve to the wall of the vessel. In one embodiment, the prosthetic valves have at least one substantially rigid strut, at least two expandable fixation rings located about the circumference of the base of the apex of the valve, and one or more commissures and leaflets. The prosthetic valves are introduced into the vascular system a compressed state, advanced to the site of implantation, expanded and secured to the vessel wall.

39. A valve prosthesis having a longitudinal axis, comprising:

a valve fixation device comprising;

a plurality of struts that run in a direction substantially parallel to the longitudinal axis;

a first circumferential row of cells coupled to the plurality of struts; and

a second circumferential row of cells coupled to the plurality of struts;

the valve fixation device being compressible to a compressed state for delivery to an implantation site, and expandable to an expanded state for deployment at the implantation site;

the plurality of struts being substantially rigid such that the plurality of struts do not change dimensions between the compressed state and the expanded state; and

a valve comprising a plurality of leaflets and a plurality of commissures, the valve being coupled to the valve fixation device such that the plurality of commissures are radially aligned with respective struts of the plurality of struts.

40. The valve prosthesis of claim 39, wherein the valve comprises tissue.

41. The valve prosthesis of claim 39, wherein the plurality of struts comprises three struts.

42. The valve prosthesis of claim 41, wherein the three struts are circumferentially spaced apart by about 120 degrees.

43. The valve prosthesis of claim 39, wherein the valve fixation device is self-expandable.

44. The valve prosthesis of claim 39, wherein the valve fixation device is balloon-expandable.

45. The valve prosthesis of claim 39, wherein:

the first circumferential row of cells is coupled to the plurality of struts substantially at a base end of the struts; and

the second circumferential row of cells is coupled to the plurality of struts substantially at an apical end of the struts.

46. The valve prosthesis of claim 39, wherein the valve is coupled to the valve fixation device at least at the plurality of struts.

47. The valve prosthesis of claim 46, wherein the valve fixation device further comprises a material at an upstream end of the valve fixation device, the material being configured to prevent perivalvular leakage.

48. A valve prosthesis having a longitudinal axis, comprising:

a valve fixation device configured to be circumferentially compressed to a compressed state for delivery to an implantation site, and circumferentially expandable to an expanded state for deployment at the implantation site, the valve fixation device comprising;

a plurality of struts that run in a direction substantially parallel to the longitudinal axis; and

a plurality of expandable cells between the plurality of struts;

wherein the plurality of struts are substantially rigid such that the plurality of struts do not change dimensions between the compressed state and the expanded state; and

a valve comprising a plurality of leaflets and a plurality of commissures, the valve being coupled to the valve fixation device such that the plurality of commissures are radially aligned with respective struts of the plurality of struts, the valve being attached to the valve fixation device at least at the plurality of struts.

49. The valve prosthesis of claim 48, wherein the valve comprises tissue.

50. The valve prosthesis of claim 48, wherein the plurality of struts comprises three struts.

51. The valve prosthesis of claim 50, wherein the three struts are circumferentially spaced apart by about 120 degrees.

52. The valve prosthesis of claim 48, wherein the valve fixation device is self-expandable.

53. The valve prosthesis of claim 48, wherein the valve fixation device is balloon-expandable.

54. The valve prosthesis of claim 48, wherein the plurality of struts run along substantially an entire length of the valve fixation device.

55. The valve prosthesis of claim 48, wherein the valve fixation device further comprises a material at an upstream end of the valve fixation device, the material being configured to prevent perivalvular leakage.
