Abstract

Methods, systems, and computer-storage media are provided for facilitating the care of a patient by a multi-disciplinary care team. A graphical user interface presents one or more clinical problems associated with a patient. For each clinical problem, actionable care team icons are presented that, among other things, identify care teams responsible for caring for the clinical problem and any actions taken by the care teams to address the clinical problem.

Claims

1. One or more computer-storage media having computer-useable instructions embodied thereon that, when executed by a computing device, cause the computing device to display a graphical user interface (GUI) useable for facilitating the care of a patient by a multi-disciplinary care team, the GUI comprising:

a declared clinical problem display area configured to display one or more declared clinical problems associated with the patient; and

a care team display area configured to display for each of the one or more declared clinical problems one or more actionable care team icons, each care team icon identifying a care team associated with the declared clinical problem.

2. The GUI of claim 1, wherein interaction with a care team icon of the one or more care team icons initiates presentation of a set of information comprising an identity of a clinician on the respective care team who is responsible for addressing the respective declared clinical problem.

3. The GUI of claim 2, wherein the set of information further comprises new orders initiated by the clinician for the respective declared clinical problem, new clinical documentation created by the clinician for the respective declared clinical problem, and rounding information related to the clinician and the patient.

4. The GUI of claim 3, wherein the set of information further comprises one or more options for facilitating communication with the clinician, wherein the one or more options comprise at least instant messaging, electronic mail, paging, or telephone.

One or more computer-storage media having computer-useable instructions embodied thereon that, when executed by a computing device, cause the computing device to display a graphical user interface (GUI) useable for facilitating the care of a patient by a multi-disciplinary care team, the GUI comprising:

a declared clinical problem display area comprising one or more declared clinical problems associated with the patient, for each declared clinical problem in the declared clinical problem display area, the GUI further comprising;

(A) an alert display area configured to display a visual indicator indicating the existence of items on a to-do list for the declared clinical problem;

(B) a recommendation display area configured to display one or more recommendations related to the declared clinical problem;

(C) a clinical action display area configured to display one or more clinical actions icons useable for initiating an action with respect to the declared clinical problem;

(D) a care team display area configured to display one or more actionable care team icons, each care team icon identifying a care team associated with the declared clinical problem; and

(E) a clinical information display area configured to display one or more clinical information icons useable for interacting with clinical information related to the declared clinical problem.

6. The GUI of claim 5, wherein information presented on the GUI is customized based on an identity of a viewer of the GUI.

7. The GUI of claim 6, wherein for each declared clinical problem in the declared clinical problem display area, the GUI further comprises an annotation display area configured to present annotations associated with the declared clinical problem.

8. The GUI of claim 6, wherein the one or more recommendations displayed in the recommendation display area are generated using clinical-support guidelines.

9. The GUI of claim 6, wherein the one or more clinical action icons comprise at least one of:

a review action icon, interaction with which initiates at least one of a summary of information related to the declared clinical problem, or an option for selecting an item of clinical information to review,

an order action icon, interaction with which enables the viewer to initiate an order for the declared clinical problem,

a document action icon, interaction with which enables the viewer to create a clinical document related to the declared clinical problem, or

a communication action icon, interaction with which initiates the presentation of a message screen to facilitate communication with one or more clinicians.

10. The GUI of claim 9, wherein the clinical document comprises a clinical note.

11. The GUI of claim 6, wherein interaction with a care team icon initiates the presentation of information comprising an identity of a clinician on the respective care team who is responsible for addressing the declared clinical problem.

The GUI of claim 11, wherein the information presented in response to the interaction with the care team icon further comprises new orders initiated by the clinician for the declared clinical problem, new clinical documentation created by the clinician for the declared clinical problem, and rounding information related to the clinician and the patient.

13. The GUI of claim 6, wherein the one or more clinical information display icons comprise at least one of a medication icon, a note icon, a lab icon, a radiology icon, a surgery icon, and a message icon.

14. The GUI of claim 13, wherein each of the one or more clinical information display icons is presented in one or more states, the one or more states comprising:

a tagged state indicating that information associated with the clinical information display icon has been tagged by another clinician for review by the viewer of the GUI,

a review state indicating that information associated with the clinical information display icon has not been reviewed by the viewer of the GUI,

a previously-viewed state indicating that information associated with the clinical information display icon has previously been reviewed by the viewer of the GUI, and

a no-information state indicating that no information associated with the clinical information display icon has been documented.

15. The GUI of claim 5, further comprising:

an identified findings display area configured to present one or more selectable clinical findings related to the patient; and

a possible diagnosis area configured to present one or more possible diagnoses based on clinical findings selected by a user.

16. The GUI of claim 15, wherein the one or more clinical findings are determined by accessing the patient'"'"'s electronic medical record.

17. The GUI of claim 15, wherein at least a portion of the one or more possible diagnoses are flagged as high risk.

18. The GUI of claim 15, wherein interaction with at least one of the one or more possible diagnoses initiates a summary of information concerning the reason the at least one possible diagnosis was presented.

19. One or more computer-storage media having computer-useable instructions embodied thereon that, when executed by a computing device, cause the computing device to display a graphical user interface (GUI) useable for facilitating the care of a patient by a multi-disciplinary care team, the GUI comprising:

a declared clinical problem display area configured to present one or more declared clinical problems associated with the patient;

a care team display area configured to present care team icons for each of the one or more declared clinical problems, each care team icon indicating an identity of a care team associated with the respective declared clinical problem;

an action display area configured to present one or more clinical action icons, interaction with which enables a user to initiate a clinical action with respect to at least one of the one or more declared clinical problem;

a clinical information display area configured to present one or more clinical information display icons, interaction with which enables the user to view or interact with clinical information related to at least one of the one or more declared clinical problems;

an identified findings display area configured to display one or more selectable identified clinical findings; and

a possible diagnosis area configured to display one or more possible diagnoses based on selected identified clinical findings.

20. The GUI of claim 19, wherein the one or more possible diagnoses are categorized into a high risk category and a possible category.
