Abstract

Remote patient management involves receiving an enrollment request to enroll a patient, identification of vitals of the patient to be measured based on the enrollment request, providing an equipment request indicating the vitals of the patient to be measured, receiving equipment information corresponding to one or more health measurement devices for measuring the vitals of the patient, and associating the patient with the equipment information corresponding to the one or more health measurement devices. In addition, a user device establishes communication with a health measurement device and determines whether it is associated with the patient, and if so, receives measurements from the health measurement device and provides the measurements to a healthcare management server. Otherwise, if not associated with the patient, no measurements are received from the health measurement device.

Claims

1. A computer hardware system comprising at least one processing unit coupled to a memory, wherein the memory is encoded with computer executable instructions that when executed cause the at least one processing unit to:

receive an enrollment request to enroll a patient in remote patient management;

identify vitals of the patient to be measured based on the enrollment request;

provide an equipment request indicating the vitals of the patient to be measured;

receive equipment information corresponding to one or more health measurement devices for measuring the vitals of the patient; and

associate the patient with the equipment information corresponding to the one or more health measurement devices.

2. The system of claim 1, wherein the one or more health measurement devices includes at least one of a scale, a glucose monitor, an oxygen monitor, a blood pressure monitor, a thermometer, or a heart rate monitor.

3. The system of claim 1, wherein the computer executable instructions that when executed cause the at least one processing unit to provide an equipment request indicating the vitals of the patient to be measured include instructions that when executed cause the at least one processing unit to provide an equipment request to a third party server.

4. The system of claim 1, wherein the equipment information includes at least one of a serial number, a model number, a version number, or an identification number.

5. The system of claim 1, wherein the memory is further encoded with computer executable instructions that when executed cause the at least one processing unit to:

identify the one or more health measurement devices based on the vitals of the patient to be measured.

6. The system of claim 1, wherein the memory is further encoded with computer executable instructions that when executed cause the at least one processing unit to:

authenticate a practitioner device; and

authenticate a user device.

7. The system of claim 1, wherein the memory is further encoded with computer executable instructions that when executed cause the at least one processing unit to:

provide an account for the patient responsive to receipt of the enrollment request.

8. The system of claim 1, wherein the memory is further encoded with computer executable instructions that when executed cause the at least one processing unit to:

receive a measurement from a user device associated with the patient; and

determine whether the measurement was provided by the one or more health measurement devices.

9. The system of claim 8, wherein the memory is further encoded with computer executable instructions that when executed cause the at least one processing unit to:

determine whether the measurement is within a range.

10. A method, comprising:

establishing, using a user device, communication with a health measurement device, wherein the user device is associated with a patient;

determining, using the user device, whether the health measurement device is associated with the patient;

if the health measurement device is associated with the patient;

receiving one or more measurements from the health measurement device; and

providing the one or more measurements to a healthcare management server; and

if the health measurement device is not associated with the patient;

receiving no measurements from the health measurement device.

11. The method of claim 10, wherein determining, using the user device, whether the health measurement device is associated with the patient comprises:

receiving first equipment information from the health measurement device; and

determining whether the first equipment information corresponds to second equipment information associated with the patient.

12. The method of claim 11, wherein determining whether the first equipment information corresponds to second equipment information associated with the patient comprises:

receiving the second equipment information from the healthcare management server; and

determining whether the first equipment information matches the second equipment information.

13. The method of claim 10, further comprising:

before establishing communication with the health measurement device, authenticating the patient with the user device.

14. A method, comprising:

receiving an equipment request;

identifying a plurality of health measurement devices based on the equipment request; and

for each of the plurality of health measurement devices, providing equipment information associated with the health measurement device, wherein the equipment information associated with the health measurement device includes a serial number, a model number, an identification number, a version number, or a combination thereof.

15. The system of claim 14, further comprising:

causing the health measurement device and a user device to be provided to the patient.

16. The system of claim 15, further comprising:

prior to causing the health measurement device and a user device to be provided to the patient, configuring the user device to communicate with the health measurement device.

17. The system of claim 14, wherein receiving an equipment request comprises:

receiving the equipment request, the equipment request indicative of a plurality of vitals associated with the patient.
