1. A system for onboarding a participant of a health services program, the system comprising:
- a data structure containing a plurality of unique identification codes, each of the unique identification codes being linked to a data element providing personally identifiable information of a participant; and
a document for providing to a participant, the document including;
(a) an indicium indicating a unique identification code from among the plurality of unique identification codes in the data structure;
(b) a personal identifier of the participant, the personal identifier being included in a data element providing personally identifiable information of the participant, the data element being linked to the unique identification code in the data structure; and
(c) a logistics detail for a biometric event,wherein, upon receiving the unique identification code from the document, the data element providing personally identifiable information of the participant is further linked to an electronic medical record of the participant.

Abstract
A registration card or similar document (such as a boarding pass) may be used to onboard participants of health services programs. A data structure may store a plurality of unique identification codes linked to data elements providing personally identifiable information of participants. A document may then be generated to include an indicium, such as a QR code and/or bar code, for indicating the unique identification code for the participant, a personal identifier and a logistics detail for a biometric event. Upon receiving the unique identification code from the document, such as at the biometric event, the data element in the data structure may then be linked to an electronic medical record of the participant.
