Abstract

A surgical implant comprising a receiver member and deployment member for clamping and sealing tissue of variable thickness, a corresponding applicator tool and surgical system for connecting, sealing, fastening and/or attaching tissue to itself, to other tissue and/or to non-tissue structures, and/or sealing the flow of fluid at and/or between structures such as tissue and vessel structures.

44. A surgical implant configured to accommodate a range of tissue thicknesses, the implant comprising first and second fasteners and first and second compressible members, the first fastener engageable with the first compressible member and the second fastener engageable with the second compressible member, wherein the first compressible member is compressible in accordance with a distance of advancement of the first fastener and the second compressible member is compressible in accordance with a distance of advancement of the second fastener, the distances of advancement of the first and second fasteners dependent on a tissue thickness through which the fasteners advance.

45. The surgical implant according to claim 44, wherein the first and second compressible members apply a force to the respective first and second fasteners in a direction opposite a direction in which the first and second fasteners advance through tissue.

46. The surgical implant according to claim 44, further comprising a receiver member having first and second shields, the shields shielding penetrating tips of the first and second fasteners.

47. The surgical implant according to claim 44, further comprising a receiver member, wherein the first and second fasteners are supported on a fastener supporting member and the extent of compression of the first and second compressible members is dependent on the thickness of tissue positioned between a first tissue contacting surface of the fastener supporting member and a second tissue contacting surface of the receiver member.

48. The surgical implant according to claim 45, wherein the first and second compressible members comprise tower-like structures having a base engageable by a base of one of the fasteners.

49. The surgical implant according to claim 48, wherein the base of the fastener presses against the base of the tower-like structure to deform the tower-like structure.

50. The surgical implant according to claim 45, wherein the first fastener straddles the first compressible member and the second fastener straddles the second compressible member.

51. The surgical implant according to claim 44, further comprising a retainer, the first and second fasteners engaging the retainer.

52. The surgical implant according to claim 51, wherein the retainer includes a plurality of openings and a plurality of strands, wherein a first barb of the first fastener is receivable in one of the openings and a second barb of the second fastener is receivable in another of the openings, the first and second barbs moving apart the plurality of strands during deployment of the first and second fasteners into the openings, at least a portion of the plurality of openings formed by the plurality of strands.

53. The surgical implant of claim 51, wherein the first and second fasteners are deployable from a fastener supporting member toward a receiver member to penetrate tissue and to engage the retainer supported by the receiver member to couple the fastener supporting member and the receiver member together.

54. The surgical implant according to claim 44, wherein the distance of advancement of the first fastener is the same as the distance of advancement of the second fastener.

55. The surgical implant according to claim 44, wherein the distance of advancement of the first fastener is different than the distance of advancement of the second fastener.

56. A surgical implant comprising:

a fastener supporting member having a first surface;

a plurality of fasteners supported by the fastener supporting member, the plurality of fasteners having a penetrating tip and deployable in a first direction;

a receiver member configured to receive the plurality of fasteners; and

a plurality of counterforce members extending from the fastener supporting member to apply a counterforce on the plurality of fasteners when the plurality of fasteners have been deployed, the counterforce members applying a force against the plurality of fasteners in a second direction opposite a first direction of deployment of the plurality of fasteners.

57. The surgical implant according to claim 56, wherein deployment of the plurality of fasteners compresses the counterforce members.

58. The surgical implant according to claim 56, wherein the fasteners of the plurality of fasteners are unattached to each other.

59. The surgical implant according to claim 56, wherein the plurality of fasteners are arranged in rows substantially parallel to a longitudinal axis of the implant.

60. The surgical implant according to claim 56, wherein the counterforce members apply a force to the plurality of fasteners to force the receiver member toward the fastener supporting member after the fastener supporting member and the receiver member are coupled together by the plurality of fasteners.

61. A surgical implant comprising a receiver member, a fastener supporting member and a plurality of fasteners supported on the fastener supporting member and arranged along a longitudinally extending row substantially parallel to a longitudinal axis of the fastener supporting member, the plurality of fasteners engageable with the receiver member, the fastener supporting member and the receiver member having a width and a length exceeding the width, and the plurality of fasteners coupling the fastener supporting member and the receiver member together and pulling the receiver member toward the fastener supporting member after coupling the fastener supporting member and receiver member.

62. The surgical implant according to claim 61, further comprising a plurality of biasing members biasing the plurality of fasteners away from the receiver member when the fastener supporting member and receiver member are coupled together.

63. The surgical implant according to claim 62, wherein a retainer with openings formed therein is positioned in the receiver member, and tips of the plurality of fasteners extend through the openings in the retainer when the plurality of fasteners are advanced into the receiver member.
