Abstract

The present invention discloses substantially more efficient and effective methods for delivering healthcare whereby patient care can be coordinated effectively and efficiently through changes in different physical settings and levels of care. Coordinated care helps ensure that patients seeking healthcare, and in particular chronically ill patients, with comorbidities get timely care to avoid unnecessary ER visits and admissions. Through risk and disease stratification, member profiling, inter disciplinary team follow-up and patient education, the methods of the present invention are operative to effectively allocate available resources, formulate and execute individualized care plans and successfully manage high risk members.

Claims

1. A system, comprising:

a user interface, comprising a network connection and internet access, operable to display data related to triage of individuals;

at least one application loaded on a server, the server comprising a processor, memory and a network connection with internet access, the at least one application providing data that is made available via the user interface, and programmed to identify a sub-population comprising selected individuals within a population, the sub-population being afflicted with a complex condition, wherein said complex condition comprises at least two chronic conditions selected from the group consisting of asthma, depression, dementia, congestive heart failure, chronic obstructive pulmonary disease, diabetes, obesity, AIDS/HIV, anticoagulation, hypertension, peripheral artery disease, coronary artery disease, end-stage renal disease, and chronic kidney disease;

a cloud-hosted access management application for providing user authentication and authorization loaded on the server running the at least one application;

at least one user input device with a network connection and internet access;

a plurality of databases comprising HL7 and DICOM, located on a plurality of servers connected to the internet in a plurality of facilities comprising dialysis centers, facilities for performing radiology, and laboratory facilities;

wherein at least two of the plurality of databases use different data storage formats;

a central database, stored on at least one server, the at least one server comprising a processor and memory, the at least one server in communication, via the network or internet, with the server running the at least one disease management application and the plurality of databases, wherein the central database transfers data from the plurality of databases and transforms the data into a single format for storage in the central database, and wherein the central database is adapted to be routinely updated, by a single source or multiple sources of data, to provide up-to-date information for each individual within the population; and

wherein the at least one user input device comprises video teleconference software to video teleconference a provider at the user interface, and the user interface is configured to display the video teleconference and simultaneously display the data stored on the central database regarding the individual, and wherein the at least one application is configured to access the data stored in the central database.
