Abstract

Various devices, systems, and methods that can be used in the treatment of obesity and related illnesses are disclosed. In some instances, the cecum of an obese patient is distended to a pathophysiological size for a therapeutically effective period. The distention may be achieved by introduction of an object that is of foreign origin relative to the body of the patient into the cecum of the patient. In some instances, the distention is achieved by a medical device that transitions from an undeployed state, in which the medical device is introduced into the cecum of the patient, to an expanded state in which the medical device distends the cecum by an amount sufficient to trigger a colo-gastric brake in the patient.

90. A method comprising:

treating obesity of a patient by distending the cecum of the patient to a pathophysiological size for a therapeutically effective period.

93. The method of claim 90, wherein the therapeutically effective period comprises an amount of time sufficient to achieve an excess weight loss of the patient of no less than 10 percent.

95. The method of claim 90, wherein the therapeutically effective period comprises no less than two weeks.

99. The method of claim 90, wherein distending the cecum of the patient reduces an appetite of the patient.

100. The method of claim 90, wherein distending the cecum of the patient triggers a colo-gastric brake in the patient.

104. The method of claim 90, further comprising distending the cecum without obstructing the ileocecal valve.

119. The method of claim 90, wherein distending the cecum comprises contacting an interior surface of the cecum with an object that is of foreign origin relative to the body of the patient.

120. The method of claim 119, further comprising:

advancing an endoscope into the patient toward the cecum; and

introducing the object into the cecum via the endoscope.

124. The method of claim 120, wherein the endoscope comprises a working channel, and wherein said introducing the object into the cecum via the endoscope comprises advancing the object through at least a portion of the working channel of the endoscope.

140. The method of claim 90, wherein distending the cecum comprises placing a medical device in contact with an inner surface of the cecum.

144. The method of claim 140, wherein the medical device comprises an expandable frame.

147. The method of claim 144, wherein the expandable frame comprises a resilient material, the method further comprising releasing the frame from a constricted orientation and permitting the frame to naturally expand outwardly into contact with the cecum.

148. The method of claim 144, further comprising transitioning the expandable frame from a substantially cylindrical shape to a substantially bulbous shape.

149. The method of claim 144, further comprising transitioning the expandable frame from a substantially linear shape to a substantially spiral shape.

161. The method of claim 90, wherein said distending the cecum of the patient comprises bearing outwardly against the cecum.

163. The method of claim 161, wherein said bearing outwardly against the cecum is performed in an amount and duration sufficient to achieve fibrosis of the cecum.

164. The method of claim 163, wherein the fibrosis contributes to said treating obesity of the patient.

175. The method of claim 90, wherein the pathophysiological size is a size that is naturally achievable by the cecum of the patient for relatively short time intervals, but that would be indicative of a pathological event if sustained for the therapeutically effective period.

176. The method of claim 175, wherein the therapeutically effective period is no less than 2 weeks.

introducing a medical device into the cecum of a patient;

distending the cecum to a pathophysiological size via the medical device to reduce an appetite of the patient; and

leaving the medical device within the cecum of the patient while the medical device at least intermittently maintains the cecum in distention.

treating obesity of a patient by expanding the cecum of the patient to a pathophysiological size to trigger a colo-gastric brake in the patient; and

leaving the medical device within the cecum of the patient for a therapeutically effective period to at least intermittently trigger the colo-gastric brake throughout the therapeutically effective period.
