Abstract

Devices, systems, and methods for performing an ophthalmic procedure in an eye are disclosed. The devices include a hand-held portion and a distal, elongate member coupled to the hand-held portion having a lumen operatively coupled to a vacuum source. A drive mechanism operatively coupled to the elongate member is configured to oscillate the elongate member. When in use, the device is configured to aspirate ocular material from the eye through the lumen. The drive mechanism retracts the elongate member with a retraction speed profile and advances the elongate member with an extension speed profile. The retraction speed profile is different from the extension speed profile.

Claims

1. A medical device for removing lens tissue from inside a capsular bag of an eye, the device comprising:

a cam assembly operatively coupled to at least one vacuum generation source, at least a first portion of the cam assembly capable of rotating about an axis to cause the at least one vacuum generation source to generate vacuum; and

an elongate member operatively coupled to the cam assembly, at least a distal portion of the elongate member positioned distally of the cam assembly, and wherein at least a second portion of the cam assembly is capable of rotating about the axis to cause the elongate member to oscillate.

2. The medical device of claim 1, wherein the causing the elongate member to oscillate comprises causing the elongate member to reciprocate along a longitudinal axis of the elongate member.

3. The medical device of claim 2, wherein the elongate member reciprocates in a distal direction with a first maximum speed, and reciprocates in a proximal direction with a second maximum speed, wherein the first maximum speed is greater than the second maximum speed.

4. The medical device of claim 2, wherein the longitudinal axis of the elongate member is coincident with the axis about which the cam assembly rotates.

5. The medical device of claim 1, wherein the first portion of the cam assembly comprises a first surface, the at least one vacuum generation source comprises a piston and a cylinder, and wherein rotation of the first surface causes the piston to reciprocate within the cylinder to generate vacuum.

The medical device of claim 1, wherein the second portion of the cam assembly comprises a second surface, wherein a proximal portion of the elongate member is coupled to a third portion of the cam assembly comprising a third surface, and wherein the second surface rotating over the third surface causes the elongate member to oscillate.

7. The medical device of claim 1, wherein the at least one vacuum generation source comprises a plurality of vacuum generation sources, and the rotation of the first portion of the cam assembly causes each of the plurality of vacuum generation sources to generate vacuum.

8. The medical device of claim 1, wherein the cam assembly is operatively rotated by a motor positioned within an interior of a housing containing the cam assembly.

9. The medical device of claim 1, further comprising a housing, wherein the housing comprises the cam assembly, the at least one vacuum generation source, the elongate member and a motor for rotating the first and second portions of the cam assembly.

10. The medical device of claim 1, wherein rotation of the first and second portions of the cam assembly causes generation of a plurality of pulses of vacuum synchronized with a plurality of oscillations of the elongate member.

11. The medical device of claim 10, wherein the plurality of oscillations of the elongate member generated has a greater frequency than the plurality of pulses of vacuum generated.

12. The medical device of claim 10, wherein the plurality of pulses of vacuum generated has a greater frequency than the plurality of oscillations of the elongate member generated.

13. The medical device of claim 1, wherein the cam assembly is configured to generate a period of vacuum during at least a portion of a distal stroke of the elongate member or at least a portion of a proximal stroke of the elongate member.

14. The medical device of claim 1, wherein the cam assembly is configured to generate a period of vacuum during at least a portion of a distal stroke of the elongate member and at least a portion of a proximal stroke of the elongate member.

15. The medical device of claim 1, wherein the cam assembly is configured to generate a period of vacuum during at least a first extension and a first retraction of the elongate member and at least a second extension of the elongate member.

16. The medical device of claim 1, wherein the elongate member undergoes a dwell time of no movement after a distal stroke and before a proximal stroke.

17. The medical device of claim 16, wherein a first period of vacuum occurs during the distal stroke and during the dwell time.

18. The medical device of claim 17, wherein the first period of vacuum is maintained during at least a second distal stroke and at least a second proximal stroke of the elongate member.

19. The medical device of claim 9, wherein a speed of the motor is variably controlled by an input on the housing.

20. A method of removing lens tissue from inside a capsular bag of an eye, the method comprising:

delivering a plurality of pulses of vacuum to a distal end of an elongate member to aspirate lens tissue into an inner chamber of the elongate member; and

oscillating the elongate member in a synchronized, predetermined manner with respect to the plurality of pulses of vacuum.

21. The method of claim 20, wherein the plurality of pulses of vacuum are generated by a plurality of vacuum generation sources, wherein the oscillating the elongate member comprises advancing the elongate member in a distal direction along an axis and retracting the elongate member in a proximal direction along the axis.

22. The method of claim 21, wherein the oscillating the elongate member is synchronized to cause the elongate member to advance in the distal direction before a subsequent pulse of vacuum is generated.

23. The method of claim 21, wherein the oscillating the elongate member is synchronized to cause the elongate member to advance in the distal direction during a generation of a pulse of vacuum by at least one of the plurality of vacuum generation sources.
