Abstract

A composite assembly includes a first catheter having a lead grasping mechanism extending through a channel thereof, a second catheter concentrically disposed over the first catheter and having a smooth, tapered end and a third catheter concentrically disposed over the second catheter and is used to remove an inoperative endocardial lead from a patient'"'"'s heart through a venous path. A free end of the lead is grasped and held securely to the first catheter by a grasping mechanism. The second and third catheters then are slid as a unit through the venous path over the grasped end of the lead as opposing tensile force is maintained on the first catheter to prevent movement of the lead. The second catheter is rotated to cause its smooth, tapered, leading edge with outward sharp serrations to separate scar tissue from the lead and dilate a path through the scar tissue. The third catheter, which also has a smooth leading edge and outward sharp serrations is further advanced a predetermined amount forward over the second catheter and is rotated to separate and/or dislodge a tip of the lead which is embedded in scar tissue of the heart. The composite assembly along with the lead therein then is removed as a unit from the heart via the venous path without causing excessive forces to be applied to the vein or the wall of the heart.

Claims

1. An apparatus for removing an endocardial lead from the heart of a patient via a venous path, said endocardial lead having a proximal end and a distal tip that is lodged in tissue in said heart, said apparatus comprising in combination:

(a) first catheter means for attachment to said proximal end of said endocardial lead, said first catheter means having a distal end and a proximal end;

(b) lead grasping means for grasping and securely attaching said distal end of said first catheter means to said proximal end of said endocardial lead;

(c) stylet means extending through said first catheter means and connected to said lead grasping means for allowing manipulating of said lead grasping means from said proximal end of said first catheter means and drawing of said lead grasping means and a grasped portion of said endocardial lead to said distal end of said first catheter means;

(d) second catheter means, having a proximal end and a distal end, and disposed concentrically about said first catheter means for advancement over said first catheter means and said proximal end of said endocardial lead and along said endocardial lead in said venous path and in said heart toward said tip;

(e) first means for effecting sliding of said second catheter means over said first catheter means and said endocardial lead toward said tip; and

(f) third catheter means, having a distal end and a proximal end, and disposed concentrically about said second catheter means for controlled advancement over said first end of said second catheter means for engaging the tissue surrounding said tip, and pushing said tissue away from said tip to separate said tissue from said tip, thereby dislodging said tip from said tissue without excessively damaging the wall of said heart near said tissue.

2. The apparatus of claim 1 wherein said distal end of said second catheter means has smooth leading edge means for avoiding cutting into said endocardial lead as said second catheter means is advanced over said endocardial lead.

3. The apparatus of claim 2 wherein said second catheter means includes a plurality of spaced, outwardly oriented tissue separating means disposed around said distal end of said second catheter means behind said smooth leading edge thereof for effecting efficient separation of scar tissue in said venous path from said endocardial lead as said second catheter means is advanced over said endocardial lead.

4. The apparatus of claim 3 wherein said first for effecting siding means also effects rotating of said second catheter means about said first catheter means to cause said tissue separating means to separate said scar tissue from said endocardial lead.

5. The apparatus of claim 4 wherein said plurality of tissue separating means includes a plurality of outwardly oriented serrations disposed on the outer surface of said distal end of said second catheter means.

6. The apparatus of claim 4 including second means for effecting sliding and rotation of said third catheter means over said second catheter means.

7. The apparatus of claim 6 wherein said first means for effecting sliding includes an elongated, first cylindrical knob disposed concentrically about said first catheter means and attached at one end to said proximal end of said second catheter means, and wherein said second means for effecting sliding and rotation includes a second cylindrical knob concentrically disposed about and longitudinally slidable about and rotatable about said first cylindrical knob.

8. The apparatus of claim 7 including a calibrated scale on said first cylindrical knob for gauging the distance said third catheter means is advanced over said second catheter means.

9. The apparatus of claim 7 including stop means attached to said first cylindrical knob for limiting the amount of advancement of said third catheter means relative to said second catheter means to prevent excessive penetration of said distal end of said third catheter into the wall of said heart.

10. The apparatus of claim 3 wherein said second catheter means includes a radiopaque portion to effect fluoroscopic visualization of the position of said distal end of said second catheter means as it is advanced along said endocardial lead.

11. The apparatus of claim 10 wherein said third catheter means has a radiopaque portion to effect fluoroscopic visualization of the position of said first and of said third catheter means as it is advanced over said second catheter means.

12. The apparatus of claim 1 including tension applying means attached to said proximal end of said first catheter means for applying a tensile force to said distal end of said first catheter means to maintain said first catheter means and said securely attached endocardial lead substantially stationary while said second catheter means is being advanced over said first catheter means.

13. The apparatus of claim 1 wherein said distal end of said third catheter means has smooth leading edge means for avoiding excessive damage to the wall of said heart as said third catheter means engages said tissue to dislodge said top of said endocardial lead therefrom.

14. The apparatus of claim 13 wherein a portion of said second catheter means adjacent to said distal end thereof is generally tapered to enhance dilating of scar tissue in said venous path by said second catheter means as it is advanced therein.

15. The apparatus of claim 1 wherein said first catheter means includes an enlarged chamber at its distal end into which the grasped free end of said endocardial lead can be drawn.

16. The apparatus of claim 1 including stylet tension maintaining means disposed at the proximal end portion of said first catheter means for maintaining sufficient tension on said stylet means to retain said lead grasping means and the grasped proximal end portion of said endocardial lead in securely attached relationship to said distal end of said first catheter means.

17. An apparatus for removing an endocardial lead from the heart of a patient via a venous path, said endocardial lead having a free end and a distal end with a tip that is lodged in tissue in said heart, said apparatus comprising in combination:

(a) first catheter means, having a distal end and a proximal end, for insertion into a venous path to said heart;

(b) lead grasping means for grasping a portion of said endocardial lead distant from said tip;

(c) stylet means extending entirely through the length of said first catheter means and connected to said lead grasping means for manipulating of said lead grasping means from said proximal end of said first catheter means and drawing of said lead grasping means and a grasped portion of said endocardial lead into said distal end of said first catheter means; and

(d) stylet tension maintaining means disposed at the proximal end portion of said first catheter means for maintaining sufficient tension on said stylet means to retain said lead grasping means and said grasped portion of said endocardial lead in said distal end of said first catheter means and in securely attached relationship to said distal end of said first catheter means;

(e) second catheter means disposed concentrically about said first catheter means for controlled advancement over said first catheter means and said endocardial lead toward said tip, said second catheter means having a distal end and a proximal end;

(f) first means for effecting controlled advancing of said second catheter means over said first catheter means and said endocardial lead toward said tip;

(g) third catheter means, having a distal end and a proximal end, disposed concentrically about said second catheter means for controlled advancement over said distal end of said second catheter means for engaging the tissue surrounding said tip and pushing said tissue surrounding said tip away from said tip to separate that tissue from said tip; and

(h) second means for effecting controlled advancing of said distal end of said third catheter means over said distal end second catheter means into said tissue surrounding said tip and for effecting controlled rotation of said third catheter means relative to said second catheter means, to further effectuate said separation of said tissue from said tip.

18. The apparatus of claim 17 further including radially protruding tissue engaging means on said distal end of said third catheter means for improving engagement of said tissue surrounding said lodged tip with said distal end of said third catheter means.

19. A method of removing an endocardial lead from the heart of a patient via a venous path, said endocardial lead having a free end and a distal end with a tip that is lodged in tissue in said heart, said method comprising the steps of:

(a) providing a lead grasping assembly including a first catheter having a proximal end and a distal end, lead grasping means for grasping a portion of said endocardial lead, and stylet means extending entirely through the length of said first catheter for controlling said lead grasping means, the portion of said stylet means extending from said distal end of said first catheter being connected to said lead grasping means;

(b) providing a second catheter concentrically about said first catheter, said second catheter having a proximal end and a distal end, and also providing a third catheter concentrically about said second catheter, said third catheter having a proximal end and a distal end;

(c) passing said lead grasping assembly through a venous path to advance said lead grasping means to said free end of said endocardial lead;

(d) manipulating said stylet means at said proximal end of said first catheter to cause said lead grasping means to grasp said free end of said endocardial lead;

(e) manipulating said proximal end of said second catheter to advance said distal end of said second catheter over said distal end of said first catheter, said lead grasping means, and said free end of said endocardial lead and toward said tip;

(f) manipulating said proximal end of said third catheter to advance its distal end over said distal end of said second catheter to cause said distal end of said third catheter to engage tissue surrounding said lodged tip and push said tissue away from said tip to separate said tissue from said tip; and

(g) withdrawing said lead grasping assembly to remove said endocardial lead from said heart.

20. The method of claim 19 wherein said endocardial lead is a composite multi-level endocardial lead including a channel lead and a core lead extending out of a side port in said channel lead, said method including, prior to step (a), utilizing an introducer apparatus to position an open channel sheath leading from an external opening into said venous path to a first location adjacent to said core lead extending out of said side port of said channel lead, introducing a fourth catheter apparatus having thereon a lead cutting apparatus and controlling a cutting element of said lead cutting apparatus from a distal end of said fourth catheter apparatus to affect cutting said core lead in half, and then performing steps (a) through (g) to dislodge the tip of the said core lead and extract the lower portion of said core lead and also separately performing steps (a) through (g) to extract said channel lead after withdrawing the upper portion of said core lead into the channel of said channel lead.

21. The method of claim 19 including, after step (d), applying tension to said stylet means to draw said lead grasping means and said free end of said endocardial lead into said distal end of said first catheter.

22. The method of claim 21 including effectively locking said stylet means at said proximal end of said first catheter to maintain sufficient tension to said stylet means to maintain said free end of said endocardial lead in securely attached relationship with said distal end of said first catheter.

23. The method of claim 19 wherein step (e) is performed by means of a first knob means attached to said proximal end of said second catheter, said manipulating of step (e) including rotating said second catheter to aid in advancing said distal end of said second catheter through any scar tissue in the venous path through which said second catheter is being advanced.

24. The method of claim 23 including providing a tapered smooth leading edge on the distal end of said second catheter to avoid cutting of insulation of said endocardial lead and to effect dilating of the venous path through which said second catheter is being advanced.

25. The method of claim 24 including providing outward protrusions or serrations on the shoulder of said distal end of said second catheter adjacent to said leading edge thereof to effect separation of obstructions in that venous path from said endodcardial lead.

26. The method of claim 23 wherein step (f) is performed by means of a second knob means concentric with said first knob means and attached to said proximal end of said third catheter, said manipulating of step (f) including rotating said second knob means relative to said first knob means to further effectuate said pushing of said tissue away from said tip.

27. The method of claim 26 including providing a smooth, uniform leading edge on the distal end of said third catheter to avoid excessive concentration of outward force on the wall of said heart.

28. The method of claim 27 including providing a plurality of outward protrusions on the shoulder of said distal end of said third catheter adjacent to said smooth, uniform leading edge of said distal end of said third catheter to effect improved engagement of said distal end of said third catheter with said tissue surrounding said tip.

29. The method of claim 26 including providing a stop on said first knob means relative to said first knob means to limit the extent to which said distal end of said third catheter can extend beyond said distal end of said second catheter.
