Abstract

An apparatus for injecting a fluid into the heart and making temperature measurements within the pulmonary artery comprising an elongated flexible tube having a distal end portion, a temperature sensor mounted on the distal end portion, a peripheral wall, at lest one lumen extending longitudinally within the tube and an injectate port opening in the peripheral wall proximally of the temperature sensor. The tube can be advanced through a vein, the right atrium and the right ventricle to place the distal end portion in the pulmonary artery and the injectate port in the right atrium facing generally toward the inferior vena cave. The temperature sensor is oriented on the distal end portion so as to materially reduce the likelihood that it will contact the wall of the pulmonary artery during use.

Claims

1. An apparatus for making temperature measurements within the pulmonary artery of a patient comprising:

an elongated flexible tube sized to pass through a vein or an artery and into the heart, said tube having a longitudinal axis, proximal and distal ends and a distal end portion which includes said distal end;

said distal end portion being curved with such curve having an inside and an outside;

a temperature sensor;

means for mounting the temperature sensor on said distal end portion of the tube with at least a portion of the temperature sensor displaced radially from the longitudinal axis of the tube and being at a location in a zone which extends circumferentially of the tube for about 150 degrees and which extends into the outside of the curve for about 30 degrees whereby the temperature sensor can obtain temperature information about a fluid adjacent the temperature sensor; and

means for transmitting the temperature information proximally of the temperature sensor along the tube.

2. An apparatus as defined in claim 1 wherein said location is on the inside of the curve.

3. An apparatus as defined in claim 1 wherein said distal end portion has a dividing plane which divides the inside of the curve from the outside of the curve and the temperature sensor is closely adjacent said dividing plane.

An apparatus as defined in claim 1 wherein said tube includes a peripheral wall, at least one lumen extending longitudinally within the tube and an injectate port opening in said peripheral wall proximally of the temperature sensor whereby an injectate can be injected through the lumen and the injectate port, said tube having a dividing plane which separates the outside of the curve from the inside of the curve and which extends along the longitudinal axis of the tube to divide the tube longitudinally into first and second half sections which include the outside and the inside of the curve, respectively, and said injectate port has a central axis in the first half section of the tube.

5. An apparatus as defined in claim 4 wherein said injectate port is located proximally of the curved distal end portion and includes a plurality of openings and said location is on the second half section of the tube.

6. An apparatus as defined in claim 4 wherein said injectate port is displaced about 90 degrees circumferentially from the temperature sensor.

7. An apparatus as defined in claim 4 including proximal and distal electrodes mounted on said tube, said proximal electrode being located proximally of the injectate port and said distal electrode being located intermediate the injectate port and the temperature sensor.

8. An apparatus as defined in claim 4 wherein the tube includes an infusion lumen and an infusion port in the peripheral wall for providing communication to the infusion lumen, said infusion port being located proximally of the injectate port.

An apparatus as defined in claim 4 wherein said at least one lumen is a first lumen and said tube has a second lumen and a third lumen and said apparatus includes an inflatable balloon adjacent the distal end of the tube, said second lumen extends to the balloon to provide for balloon inflation and said third lumen is a through lumen and extends at least substantially to the distal end and opens at a distal port.

10. An apparatus as defined in claim 1 including a heparin coating on said tube including said distal end portion thereof.

11. An apparatus for making temperature measurements within the pulmonary artery of a patient comprising:

an elongated flexible tube having a longitudinal axis, proximal and distal ends and a distal end portion which includes the distal end;

said tube being sized to pass through a vein or an artery, the right atrium and the right ventricle to place at least the distal end portion in the pulmonary artery, said distal end portion being curved at least when it is in the pulmonary artery with such curve having an inside and an outside;

a temperature sensor;

means for mounting the temperature sensor on the distal end portion of the tube with at least a portion of the temperature sensor displaced radially from the longitudinal axis of the tube and with at least said portion of the temperature sensor being at a location in a zone which extends circumferentially of the tube for about 150 degrees, primarily faces anteriorly and extends into the outside of the curve about 30 degrees whereby the temperature sensor can obtain temperature information about fluid in the adjacent regions of the pulmonary artery; and

means for transmitting the temperature information proximally of the temperature sensor along the tube.

12. An apparatus as defined in claim 11 wherein said location is on the inside of the curve.

13. An apparatus as defined in claim 11 wherein said location is approximately at the junction between the inside of the curve and the outside of the curve.

An apparatus as defined in claim 11 wherein said tube includes a peripheral wall and at least one lumen extending longitudinally within the tube and an injectate port opening in said peripheral wall sufficiently proximally of the temperature sensor and in a direction so that with the temperature sensor in the pulmonary artery the injectate can be injected through the lumen and the injectate port generally toward the inferior vena cava.

15. A method of making temperature measurements within the pulmonary artery of a patient comprising:

providing an elongated flexible tube sized to pass through a vein and into the heart with the tube having a longitudinal axis, proximal and distal ends, a distal end portion which includes said distal end, and a temperature sensor mounted on the distal end portion with at least a portion of the temperature sensor displaced axially from the longitudinal axis of the tube;

inserting the tube through a vein, the right atrium and the right ventricle to place the distal end portion and the temperature sensor in the pulmonary artery with the distal end portion being curved in the pulmonary artery and with such curve having an inside and an outside;

orienting the distal end portion in the pulmonary artery so that the temperature sensor is in a zone which extends circumferentially of the tube for about 150 degrees, primarily faces anteriorly and extends into the outside of the curve no more than about 30 degrees whereby the temperature sensor is less likely to contact pulmonary artery wall and the temperature sensor can obtain temperature information about fluid in the pulmonary artery; and

transmitting the temperature information proximally of the temperature sensor along the tube.

16. A method as defined in claim 15 wherein said step of orienting positions the distal end portion to place the temperature sensor approximately at the junction of the inside of the curve and the outside of the curve.

17. A method as defined in claim 15 including injecting a cold injectate from a location in the right atrium closely adjacent the tricuspid valve in the general direction of the inferior vena cava and allowing such injectate to mix with the blood and utilizing the temperature sensor to measure the temperature change resulting from such injection of injectate.

18. A method as defined in claim 17 wherein said step of injecting is carried out utilizing said tube.
