Abstract

A method and apparatus for endoscopic removal of compliant biological tissues utilizing an endoscopic ultrasonic aspirator comprising irrigation and aspiration means, a piezoelectric ultrasonic transducer, a first resonator such as a half-wave stepped velocity transformer, a probe including a second resonator such as a constant-stress velocity transformer, a blunt or modified working tip of open channel means or restricted tubular means for application of ultrasonic energy to cellular material, and a capacitive fluid sensor to detect the presence of irrigation fluid adjacent these transformers within the instrument. The surgery is advantageously performed by operating the transducer in the 10-20 kHz range to achieve maximum cavitation of the intracellular fluids in the tissues to be removed.

Claims

1. High-frequency vibration apparatus comprising:

high-frequency vibration source means for generating vibrations having a first amplitude,

first velocity transformer means having an input end and an output end, the input end being coupled to the vibration source means to be vibrated thereby, and the output end vibrating in response to such received vibrations with a second amplitude greater than such first amplitude,

second velocity transformer means having an input end and an output end, the input end being coupled to the output end of the first velocity transformer means to be vibrated thereby, and the output end vibrating in response to such received vibrations with a third amplitude greater than such second amplitude,

the second transformer means being elongated and when vibrating having a substantially constant mechanical stress level in substantially all of its length;

the output end of the first transformer velocity means and the second velocity transformer means forming a unitary component to minimize the production of transverse flexural vibrations.

2. Apparatus as in claim 1, wherein the second velocity transformer means has a cross-sectional area which gradually decreases between its input end and its output end to provide such substantially constant mechanical stress level when vibrating.

3. Apparatus as in claim 2, wherein the second velocity transformer means further includes a tip region close to its output end with a changing cross-sectional area for decreasing vibrational mechanical stress in the tip region to substantially zero at said output end.

4. Apparatus as in claim 2, wherein the second velocity transformer means has a cross-sectional area A which decreases as a function of the distance x from its input end substantially according to the formula

5. Apparatus as in claim 1, wherein the elastic constant of the material of the second velocity transformer means varies between its input end and its output end to provide such substantially constant mechanical stress level when vibrating.

6. Apparatus as in claim 5, wherein the density of the second velocity transformer means varies between its input end and its output end to provide such substantially constant mechanical stress level when vibrating.

7. Apparatus as in claim 1 wherein the vibration source means comprises piezoelectric transducer means.

8. Apparatus as in claim 1 wherein said first and second velocity transformer means comprise a unitary bar of titanium.

9. Apparatus as in claim 1 where said first and second velocity transformer means comprise a full wavelength resonator.

10. Apparatus as in claim 9 wherein the length of said first velocity transformer means input end is substantially 1/4 of a desired wavelength, the length of said first velocity transformer means output end is substantially 1/2 of said desired wavelength and the length of said second velocity transformer means is substantially 1/4 of said desired wavelength.

11. Vibration apparatus comprising

a handipiece

high-frequency vibration source means mounted in the handipiece for vibrating with a selected wavelength and with a first amplitude,

first velocity transformer means for amplifying vibration from the veibration source means, having an input section and an output section, the input section coupled to the vibration source means, and the output section being smaller in cross-sectional area than the input section, and

second velocity transformer means for amplifying vibrations from the first velocity transformer means, having an input end and an output end, the input end being unitary with the output section of the first velocity transformer means to receive vibrational energy and to minimize the production of transverse flexural vibrations, the output end vibrating in response to such received vibrational energy for further transmitting such vibrational energy.

12. Apparatus as in claim 11, wherein

the length of the input section of the first velocity transformer means is substantially 1/4 of such selected wavelength,

the length of the output section of the first velocity transformer means is substantially an integral multiple of 1/4 of such selected wavelength, and

the length of the second velocity

transformer means is substantially 1/4 of such selected wavelength.

13. Apparatus as in claim 12, wherein the length of the output section of the first velocity transformer means is substantially 1/2 of such selected wavelength.

14. Apparatus as in claim 11, wherein

the first and second velocity transformer means and the vibration source means are elongated and have longitudinal axes, and

at least part of the first velocity transformer means is curved to offset the longitudinal axis of the second velocity transformer means from the longitudinal axis of the vibration source means by a selected angle of offset.

15. Apparatus as in claim 14, wherein only the input section of the first velocity transformer means is curved.

16. Apparatus as in claim 14, wherein said curved part of said first velocity transformer means is curved with a selected radius of curvature such that said angle of offset is at least about 20 °

C.

17. Apparatus as in claim 16, wherein said selected angle of offset is about 40 °

C..

18. Apparatus as in claim 16, wherein said selected radius of curvature is about 0.1 to 0.5 times such selected wavelength.

19. Apparatus as in claim 18, wherein said selected radius of curvature is about 0.2 times such selected wavelength.

20. Apparatus as in claim 11, wherein

the vibration source means and the two velocity transformer means vibrate extensionally with alternating vibrational nodes and antinodes occurring herein,

the vibration source means is mounted in the handpiece by flexible means between the interior of the handpiece and the vibration source means at a vibrational node of the vibration source means, and

the first velocity transformer means is mounted in the handpiece by flexible means between the interior of the handpiece and the first velocity transformer means at a vibrational node of the first velocity transformer means, the latter vibrational node being near the point of connection between the input and the output sections of the first velocity transformer means.

21. Apparatus as in claim 11, wherein the cross-sectional area of said second velocity transformer means varies between its input end and its output end for maintaining the mechanical stress level in substantially all of said second velocity transformer means substantially constant when vibrating.

22. Apparatus as in claim 21, wherein the second velocity transformer means further includes a tip region close to its output end whose cross-sectional area varies for decreasing vibrational mechanical stress in the tip region to substantially zero at said output end.

23. Apparatus as in claim 11 wherein the second velocity transformer means has a cross-sectional area A that decreases as a function of the distance x from its input end substantially according to the formula

24. Apparatus as in claim 11, further comprising

viewing means mounted in the handpiece for viewing from the handpiece a work site adjacent the output end of the second velocity transformer means.

25. Apparatus as in claim 24, wherein said viewing means is elongated, extends from the handpiece to such work site along a path closely parallel to said second velocity transformer means and said output section of said first velocity transformer means, and passes through a groove formed in said input section of said first velocity transformer means.

26. Apparatus as in claim 11 wherein the vibration source means comprises piezoelectric transducer means.

27. Apparatus as in claim 11 wherein said first and second velocity transformer means comprise a unitary bar of titanium.

28. High frequency vibration apparatus comprising:

transducer means for generatinig high frequency vibration of a selected wavelength and having a first amplitude;

vibration amplifying means for receiving the vibrations generated by the transducer means and amplifying said vibrations to ultrasonic vibrations of a frequency of between 10 and 20 KHz, said vibration amplifying means comprising;

first velocity transformer means of an elongated member having an input end and an output end, the input end being coupled to the transducer means and the output end vibrating in response to such received vibrations with a second amplitude greater than said first amplitude,

second velocity transformer means of an elongated member having an input end and an output end, the input end being unitary with the output end of the first transformer to be vibrated thereby and to minimize the production of transverse flexural vibrations, the output end vibrating in response to such received vibrations with the desired frequency while further having a substantially constant mechanical stress level in substantially all its length.

Apparatus as in claim 28 wherein the length of the input section of the first velocity transformer means is substantially 1/4 of the generated vibrational wavelength, and the length of the output section of the first velocity transformer means is at least 1/2 or multiples thereof of the generated vibrational wavelength.

30. Apparatus as in claim 29 wherein at least part of the first velocity transformer means is curved to offset the longitudinal axis of the second velocity transformer means from the longitudinal axis of the transducer means by a selected angle of offset of at least 20 °

C.

31. Apparatus as in claim 30 further comprising a handpiece and wherein the first velocity transformer means is mounted in said handpiece by flexible means between the interior of the handpiece and a vibrational node of the first velocity transformer means.

32. Apparatus as in claim 31 further comprising viewing means mounted in said handpiece for viewing a work site adjacent the output end of the second velocity transformer means.

33. Apparatus as in claim 32, wherein said viewing means is elongated, extends from the handpiece to such work site along a path closely parallel to said second velocity transformer means and said output section of said first velocity transformer means, and passes through a groove formed in said input section of said first velocity transformer means.

34. Apparatus as in claim 33, wherein the second velocity transformer means further includes a tip region close to its output end whose cross-sectional area varies to decrease vibrational mechanical stress in the tip region to substantially zero at said output end.

35. Apparatus as in claim 34 wherein said first and second velocity transformer means comprise a unitary bar of titanium. substantially 1/2 of said desired wavelength and the length of said second velocity transformer means is substantially 1/4 of said desired wavelength.

36. High-frequency vibration apparatus comprising:

high frequency vibration source means for generating vibrations having a first amplitude;

first velocity transformer means having an input end and an output end, the input end being coupled to the vibration source means to be vibrated thereby, and the output end vibrating in response to such received vibrations with a second amplitude greater than such first amplitude; and

second velocity transformer means having an input end and an output end, the input end being coupled to the output end of the first velocity transformer means to be vibrated thereby, and the output end comprising open channel means and vibrating in response to such received vibrations with a third amplitude greater than such second amplitude;

the second velocity transformer means being unitary with said first velocity transformer means elongated, and having a substantially constant mechanical stress level in substantially all of its length when vibrating.

37. Apparatus as in claim 36 wherein said open channel means tapers and diminishes along its length.

38. Apparatus as in claim 36 wherein the open channel means comprises a base portion and two side portions.

39. Apparatus as in claim 36 wherein the open channel means has a cross sectional area in the shape of a U or V.

40. Apparatus as in claim 36 wherein said open channel means is of substantially uniform cross section.

41. Apparatus as in claim 36 wherein said open channel means further comprises tip means of smaller cross section than said channel means.

42. Vibration apparatus comprising:

a handpiece,

high frequency vibration source means mounted in the handpiece for vibrating with a selected wavelength and with a first amplitude;

first velocity transformer means for amplifying vibrations from the vibration source means having an input section and an output section, the input, section being relatively large in cross-sectional area than the output section and coupled to the vibration source means; and

second velocity transformer means for amplifying vibrations from the first velocity transformer means having an input end and an output end, the input end being coupled to the output section of the first velocity transformer means to from unitary member which recieves vibrational energy, and the output end comprising open channel means and vibrating in response to such received vibrational energy and having a substantially constant mechanical stress level in substantially all of its length when vibrating for further transmitting such vibrational energy.

43. Apparatus as in claim 42 where said open channel means tapers and diminishes along its length.
