Abstract

A cardiac pacer features plural adressable digital memory elements each containing different pulse parameters for the pacing stimulation, address - selecting digital logic circuitry for choosing the characteristics of the pulses to be generated in response to signals derived from plural physiological sensors detecting different exercise - related body function to improve control of the heart during various levels exertion.

Claims

1. A cardiac pacemaker comprising:

at least first and second means for detecting at least respective first and second attributes of a patient'"'"'s body and for transmitting respective first and second signals representative of said at least respective first and second attributes;

said first and second attributes being representative of exertion of the patient'"'"'s body;

circuitry means for varying at least one pacing parameter as a function of said first and second signals detected in the patient'"'"'s body as input variables, said circuitry means including means for supplying pulses in accordance with said pacing parameter, said input variables being correlated with physical exertion of the patient'"'"'s body;

said pacing parameter being the pacing rate; and

additional circuitry means for uniting said at least two signals to produce a further signal for determining said pacing rate, and which is dependent on said at least two signals.

A pacemaker as defined by claim 1, wherein said additional circuitry means performs an evaluation of said first and second two signals for producing said further output signal, said evaluation being based upon at least one of an amplitude-dependent evaluation and a time-dependent evaluation for each of said first and second two signals, the amplitude of said further signals forming a standard for current cardiac output in accordance with cardiac performance demand, said pacing parameter being determined by said further signal.

A pacemaker as defined by claim 2, wherein said first signal represents a standard value for the current physical exertion of the patient'"'"'s body for short-term compensation of an increase in exertion and an integral of said second signal represents previous exertion of the patient'"'"'s body in a predetermined immediately preceding period of time, for causing said pacing parameter to undergo a relatively long term increase of the patient'"'"'s heart rate.

A pacemaker as defined by claim 3, wherein said first signal for short-term compensation of an increase in exertion is determined by said circuitry means by one of (a) systolic intervals detected in a ventricle and (b) the difference between a measured value pickup disposed on a pacemaker housing and a measured value pickup disposed in a pacemaker electrode;

while said second signal for relatively long term compensation for an increase in exertion is determined by at least one of the blood temperature, the blood oxygen saturation, the blood pH value, the respiration rate, the variation of electroplethysmographically picked up values, and LVET values which are indicative of an increased stroke volume of the patient'"'"'s heart.

5. A pacemaker as defined by claim 3, wherein said circuitry means decreases a rate variation range of said pacing parameter based upon said first signal in comparison with an entire variation range of the patient'"'"'s heart rate.

A pacemaker as defined by claim 1, further comprising a third means for detecting one of the orientation of said pacemaker in space and accelerations of a predetermined minimum intensity, as a criterion for the activity of the patient, said third means supplying a signal to said circuitry means, said circuitry means being enabled by said signal from said third means to cause said pacing parameter to have a substantial rate increase.

7. A pacemaker as defined by claim 1, further comprising third means for detecting the orientation of the pacemaker in space and for supplying an output signal to said circuitry means, said circuitry means lowering a value indicative of the stroke volume of the patient'"'"'s heart.

A pacemaker as defined by claim 7, wherein said third means supplies signals useable as selection data by said circuitry means which, as a function of orientation of the patient'"'"'s body in space, whereby in response to detection of said signals from said third means said circuitry means associates higher stroke volume values with a value characterizing the stroke volume when the patient'"'"'s body is disposed in a horizontal position.

A pacemaker as defined by claim 1, wherein said circuitry means further comprises a closed-loop control means for controlling said pacing parameter, and said first and second attributes are heart rate and stroke volume, said closed-loop control means being responsive to said hear rate and said stroke volume as the cardiac output indicative of the current physical exertion, for causing said circuitry means to vary a basic pacing rate of said pacing parameter.

A pacemaker as defined by claim 6, wherein said circuitry means further comprises calibration means for calibrating at least one of (a) one of the detected signals and (b) a united signal of a plurality of detected signals such that said output signal of said third means is processed simultaneously with said first and second signals from said first and second means for detecting for a predetermined period of time, and further comprising compensation means, which during said predetermined period of time are connected to the output side of said third detecting means such that said first signal obtained from said first means for detecting including any compensation of said first signal by said compensation means, and said second means for detecting, are substantially identical;

said compensation means remaining substantially unchanged for following predetermined operating period of time.

A pacemaker as defined by claim 1, further comprising a triggerable linking element means connected with said circuitry means for the selection of at least one of (a) different processing routes used by said circuitry means and (b) said means for detecting the measured variable which is dependent on the patient'"'"'s physical exertion, wherein triggering of said linking element is caused by at least one of:

external communication means, current signal values, signal values that are stored in memory, and elapsed time.

A pacemaker as defined by claim 1, wherein said circuitry means further comprises a transmission element which has linear and time-dependent characteristics which include (a) at least one time constant and (b) further coefficients, in accordance with a simulation of a linear physical system, described by a one of (a) linear differential equation, (b) a time delay, and (c)a predeterminable time-dependent course, as an evaluation factor.

A pacemaker as defined by claim 1, wherein said circuitry means for varying said at least one pacing parameter is controllable by means of input of data contained in at least one of a plurality of memory locations of a memory means, wherein addressing of said memory locations is effected in accordance with an address signal derived from an input variable and at least one further address signal, which is one of (a) a signal picked up in the patient'"'"'s body, (b) a signal derived thereform, and (c) an independent signal;

the data contained in said memory means is a characteristic data field containing data used by said circuitry means for causing variation of the pacing rate, said memory means being addressable as a data unit and which is linkable with further memory means containing further corresponding characteristic data fields.
