1. A heart pacemaker comprising:
- real-time clock means for producing an electrical signal corresponding to elapsed time;
sensor means for detecting events correlated to physiologic needs of a patient;
a microprocessor comprising means for executing stored instructions and means for maintaining a model of predicted cyclic physiologic needs of a patient, the model comprising a plurality of periods, each period having a predicted duration and predicted minimum physiologic need, the model being maintained based on data from the real-time clock means and the sensor means; and
means for pacing the heart of a patient at a rate determined by said microprocessor based on data from the real-time clock means and on the model.

Abstract
A body function assistance device, capable of analyzing changing but cyclic physiologic needs of a patient and generating a response based on the analysis. A microprocessor in the body function assistance device controls the response of the device to various external events and internal timer events. The microprocessor recognized variations in patterns of recurrent events and adjusts the output of the body function assistance device in response to the detected variations. In a particular embodiment, an implantable cardiac pacer is responsive to a circadian wake-sleep cycle.

16. A heart pacemaker comprising:

(A) clock means for producing an electrical signal corresponding to elapsed time; (B) sensor means for detecting events correlated to physiologic needs of a patient; (C) a microprocessor comprising (1) means for executing stored instructions and (2) means for maintaining a model of predicted physiologic needs of a patient, based on data from the clock means and the sensor means, said model maintaining means comprising (a) means for adjusting the model responsive to prior status of the model; (b) means for damping the adjustment of the model; (c) means for limiting the maximum and minimum duration of at least one period of the model; and (d) means for eliminating response to changes in physiologic needs not associated with a cyclic condition by disregarding changes in physiologic needs for a predetermined duration after a selected event; and (D) means for pacing the heart of a patient at a rate determined by said microprocessor based on the data from the clock means and the sensor means and on the model.
