1. In a medical instrument having a longitudinally extending outer tube having a proximal end, a distal end, and a longitudinal axis, a push rod extending through the outer tube and having a proximal and distal ends, an actuating apparatus engaging the utter tube and the push rod and imparting reciprocal axial motion to the push rod relative to the outer tube, end effector means coupled to the push rod at the distal end of the push rod and pivotally coupled to said outer tube, and a clevis coupled to the outer tube, whereby said reciprocal axial motion is translated to pivotal movement of said end effector means, an improvement in said actuating apparatus comprising:
- a) a sleeve member surrounding a first portion of said outer tube intermediate said proximal end and said distal end of said outer tube, said sleeve member being movable with respect to said outer tube along said longitudinal axis and said sleeve member being restrained from rotation about said longitudinal axis, said sleeve member having a proximal end having a peripheral rim portion at least partially surrounding said push rod, said peripheral rim portion of said sleeve member having first engagement means;
b) resilient means adjacent said sleeve member and adjacent said outer tube for resiliently resisting movement of said sleeve member along said longitudinal axis in a direction away from said proximal end of said outer tube;
c) handle means rotatably engaging said outer tube, said handle means having a first hollow distal portion surrounding a second portion of said outer tube adjacent and proximal said sleeve means, said first hollow distal portion of said handle means having a peripheral rim portion at least partially surrounding said outer tube and having a second engagement means for a disengageable mating engagement with said first engagement means; and
d) lever arm means pivotally engaging said handle means, and coupled to and rotatable about said proximal end of said push rod, whereby when said first and second engagement means are in an engaged position, said handle means and said lever arm are rotationally fixed relative to said sleeve member, said outer tube, and said end effector means, and when said first and second engagement means are in a disengaged position, said handle means and said lever arm freely rotate relative to said sleeve member, said outer tube, and said end effector means.

Abstract
Medical instruments are provided having end effectors and end effector actuator apparatus which are rotatable relative to the end effectors. The instruments generally include a tube which is typically coupled to the end effectors via a clevis around which the end effectors pivot, a push rod extending through the tube and coupled to the end effectors, an actuator apparatus engaging the tube and the push rod and imparting reciprocal axial motion to the push rod relative to the tube, and end effectors. The actuator apparatus includes a sleeve, spring, handle, and lever arm. The sleeve is movable along the longitudinal axis of the tube against a biasing force of the spring, but is restrained from rotating about the tube. The sleeve has a locking element on its proximal end. The handle surrounds and rotatably engages the proximal end of the tube and has a locking element on its distal end which mates with the locking element of the sleeve and disengages therewith when the sleeve is advanced along the tube. Upon disengagement, the tube, push rod, and end effectors can be rotated. The lever arm pivotally engages the handle and is coupled to the push rod such that pivotal movement of the lever arm relative to the handle effects movement of the push rod relative to the tube and pivotal movement of the end effectors. To decouple rotation of the push rod from the lever arm, a generally spherical push rod engaging element is provided in a generally cylindrical bore of the lever arm.

15. A medical instrument, comprising:

a) a longitudinally extending hollow outer tube having a proximal end, a distal end, and a longitudinal axis; b) a push rod extending through said outer tube and having proximal and distal ends; c) an actuating means for imparting reciprocal axial motion to said push rod relative to said outer tube, said actuating means comprising a sleeve member movable with respect to said outer tube for a predetermined distance along said longitudinal axis and restrained from rotation about said longitudinal axis, said sleeve member having a promixal end having a peripheral rim portion at least partially surrounding said push rod, said peripheral rim portion of said sleeve member having first engagement means; sleeve biasing means adjacent said sleeve member for keeping said sleeve member at a proximal-most position relative to said outer tube, whereby force is required to move said sleeve member distally from said proximal-most position, grip means rotatably engaging said outer tube, said grip means having a peripheral rim distal portion at least partially surrounding said push rod, said peripheral rim distal portion of said grip means having a second engagement means for a disengageable mating engagement with said first engagement means, a lever means pivotally engaging said grip means, and coupled to and rotatable about said proximal end of said push rod, whereby when said first and second engagement means are in an engaged position, said qrip means and said lever means are rotationally fixed relative to said sleeve member and said outer tube, and when said first and second engagement means are in a disengaged position, said grip means and said lever means freely rotate relative to said sleeve member and said outer tube; d) end effector means coupled to said push rod at said distal end of said push rod and pivotally coupled to said outer tube, whereby said reciprocal axial motion of said push rod is translated to pivotal movement of said end effector means. - View Dependent Claims (16, 17, 18, 19, 20, 21, 22, 23, 24, 25)
