Abstract

The present invention is an apparatus to apply an electrical pretreatment of pulses to a fibrillating heart that begins the process of organizing the action of the chaotically contracting myocardial cells, so that the defibrillating waveform applied after the pretreatment can accomplish its task with less energy than would otherwise be required. The same electrodes are employed for both pretreatment and defibrillation. In one preferred embodiment a long-duration, low-voltage pretreatment pulse is generated directly by battery and voltage-conversion circuitry, and is followed by the defibrillation pulse delivered from the previously charged capacitor. In variations on the embodiments, the capacitor is charged during the pretreatment pulse, or between the pretreatment and defibrillation pulses. In other embodiments, pretreatment includes a train of pulses of appreciably lower energy than the defibrillation pulse, and with inter-pulse time adjusted so that necessary capacitor recharging can be accomplished, so that the same capacitor (a component of dominant size) can be used for all pretreatment pulses and for the defibrillation pulse as well. Wide-ranging variations of pulse duration, height spacing and waveform are included, as well as selected polarity inversions.

Claims

1. An improved implantable defibrillator apparatus for producing a capacitor-discharge defibrillation pulse, the implantable defibrillator apparatus being a self-contained human implantable device including a pulse-generating capacitor means for storing an electrical charge, means for internally charging the pulse-generating capacitor means, and means for selectively discharging the electrical charge in the pulse-generating capacitor means through electrodes adapted to be implanted in a human patient in response to a sensing of a fibrillating heart, the improvement comprising:

pulse means for providing a plurality of low-energy pretreatment pulses for application through the electrodes to the heart before application of the defibrillation pulse, the plurality of pretreatment pulses being a train of low-energy pulses spaced away from the defibrillation pulse by a time interval.

2. Apparatus according to claim 1 wherein each pretreatment pulse employed has an energy less than 30% that of the defibrillation pulse.

3. Apparatus according to claim 1 wherein the electrical pretreatment comprises at least one pulse having a duration that is long compared to that of a conventional monophasic defibrillation pulse and a voltage that is low compared to that of a conventional monophasic defibrillation pulse.

5. Apparatus according to claim 1 wherein at least one of pulse length, pulse height, pulse spacing and pulse waveform is varied within the duration of the train.

4. Apparatus according to claim I wherein the pulse means comprises a battery and voltage-conversion circuitry that directly generates the pretreatment pulses.

6. Apparatus for the electrical pretreatment of a fibrillating heart, comprising:

a. pulse means for providing at least two low-energy pretreatment pulses for application to the heart before application of a defibrillation pulse; and

b. at least one electrical conductor means to conduct the pretreatment pulses to the heart.

7. Apparatus according to claim 6 which includes defibrillation pulse means for providing a defibrillation pulse and at least one electrical conductor to conduct the defibrillation pulse to the heart.

8. Apparatus according to claim 7 wherein each pretreatment pulse employed has an energy less than 30% that of the defibrillation pulse.

9. Apparatus according to claim 7 wherein at least one pretreatment pulse has a duration that is long compared to that of a conventional monophasic defibrillation pulse and a voltage that is low compared to that of a conventional monophasic defibrillation pulse.

10. Apparatus according to claim 7 wherein the pulse means comprises a battery and voltage-conversion circuitry that directly generates the pretreatment pulses and the defibrillation pulse means comprises a previously charged capacitor which is used to generate the defibrillation pulse.
