Abstract

Pumping mechanisms for performing peritoneal dialysis and the like employ a pumping mechanism comprising a diaphragm that operate in response to applied fluid pressure. Flexible tubing is attached to the pumping mechanism for carrying liquid to and from the patient'"'"'s peritoneal cavity. An actuating station for pumping mechanism receives the pumping mechanism and attached tubing. A pressure transfer element conveys fluid pressure to the diaphragm for moving liquid through the cassette. An occluder element contacts and crimps closed the tubing attached to the cassette. A first reservoir inflates with positive fluid pressure for contacting the pressure transfer element to hold it in operative contact against the diaphragm. A second reservoir inflates with positive fluid pressure for contacting the occluding element to hold it away from crimping contact with the tubing. At least one of the first and second reservoirs also transports fluid pressure to the pressure transfer element for conveyance to the diaphragm.

Claims

1. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber, and

means for establishing flow communication between the pumping mechanism and the patient'"'"'s peritoneal cavity,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support,

a source element carried within the housing to generate positive and negative fluid pressure, and

a pressure conveying element carried within the housing and communicating with the source element for applying positive and negative fluid pressure to the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity.

2. A system according to claim 1 wherein the pressure conveying element comprises

a pressure transfer element in contact against the diaphragm of the pump chamber to apply positive and negative fluid pressure to the diaphragm to move liquid through the pump chamber,

a reservoir that inflates in response to the introduction of positive fluid pressure from the source element for contacting the pressure transfer element to hold it in contact against the diaphragm,

conduit means for transporting positive fluid pressure from the source element to the reservoir element, and

pressure regulator means communicating with the reservoir element and the pressure transfer element for maintaining positive fluid pressure in the reservoir element above a predetermined amount to maintain contact between the pressure transfer element and the diaphragm while transporting positive fluid pressure from the reservoir element to the pressure transfer element for application to the diaphragm.

3. A system according to claim 2 and further including a carrier for moving the pressure transfer element between an at rest position away from the diaphragm and an operating position in contact against the diaphragm, and

and wherein the reservoir element, during inflation, moves the carrier from the at rest position toward the operative position.

4. A system according to claim 3

and further including means for biasing the carrier toward the at rest position.

5. A system according to claim 2

wherein the housing includes a door movable between an opened position permitting access to the support and a closed position blocking access to the support.

6. A system according to claim 2

wherein the pressure transfer element conveys both positive fluid pressure from the reservoir element and negative fluid pressure from the source element.

7. A system according to claim 2

wherein the pressure transfer element applies pneumatic pressure.

8. A system according to claim 1

wherein the means for establishing flow communication between the pumping mechanism and the patient'"'"'s peritoneal cavity includes flexible tubing attached to the cassette,

and further including

an occluding element movable when the cassette and tubing are received in the support between a normally biased flow disabled position contacting and crimping closed the tubing and a flow enabled position out of contact with the tubing,

an occluder bladder that inflates in response to the introduction of a predetermined amount of positive fluid pressure from the source element for contacting the occluding element and holding the occluding element in its flow enabled position as long as the predetermined positive pressure is maintained in the occluder bladder, and

occlusion control means operative in a first control condition for conveying positive fluid pressure from the source element to the occluder bladder to maintain the predetermined positive pressure, the occlusion control means being further operative in a second control condition for venting the positive pressure in the occluder bladder to cause the occluder element to move into its normally biased flow disable position.

9. A system according to claim 8

wherein the occlusion control means is also operative for conveying position fluid pressure from the bladder to the pressure transfer element for conveyance to the diaphragm.

10. A system according to claim 8

wherein the tubing includes patient tubing attachable in communication with the patient'"'"'s peritoneal cavity and second tubing attachable to an external component, and

wherein, when in its flow disabled position, the occluding element simultaneously contacts and crimps closed both the patient tubing and the second tubing.

11. A system according to claim 8

wherein the tubing includes patient tubing attachable in communication with the patient'"'"'s peritoneal cavity and a plurality of additional tubing attachable to external components, and

wherein, when in its flow disabled position, the occluding element simultaneously contacts and crimps closed the patient tubing and the plurality of additional tubing.

12. A system according to claim 1

and further including membrane means located between the pressure conveying elements and the diaphragm and being operative for transmitting position and negative fluid pressure while blocking liquid.

13. A system according to claim 1

wherein the pressure applies position and negative elements conveys pneumatic pressure.

14. A system according to claim 13

wherein the pressure conveying elements applies position and negative pneumatic pressure that is both above and below atmospheric pressure.

15. A system according to claim 14

and further including membrane elements located between the pressure conveying means and the diaphragm and being operative for transmitting pneumatic fluid pressure while blocking liquid.

16. A system according to claim 1

wherein the actuator includes a pressure transfer element communicating with the pressure applies position and negation means and contacting the diaphragm to convey fluid pressure to the diaphragm to move liquid through the pump chamber.

17. A system according to claim 16

wherein the actuator includes a reservoir that inflates in response to the introduction of positive fluid pressure from the source element to contact the pressure transfer element and hold it in contact against the diaphragm.

18. A system according to claim 17

and further including means communicating with the reservoir for transporting position fluid pressure from the reservoir to the pressure transfer element for application to the diaphragm.

19. A system according to claim 16

wherein the pressure transfer element carries a membrane that contacts the diaphragm and transmits positive and negative fluid pressure while blocking liquid.

20. A system according to claim 1

wherein the actuator includes a carrier that moves the pressure transfer element between an at rest position away from the diaphragm and an operating position in contact against the diaphragm.

21. A system according to claim 20

wherein the actuator includes a reservoir that inflates in response to the introduction of positive pressure from the source element to move the carrier from the at rest position to the operating position.

22. A system according to claim 21

wherein the actuator includes means for biasing the carrier toward the at rest position.

23. A system according to claim 1

wherein the housing means includes a door that opens to allow access to the support and that closes to block access to the support.

24. A system according to claim 1

wherein the means for establishing flow communication between the pumping mechanism and the patient'"'"'s peritoneal cavity includes flexible tubing attached to the cassette,

and further including an occluding element operative when the cassette and tubing means are received in the support means for movement between a flow disabled position contacting and crimping closed the flexible tubing and a flow enabled position out of contact with the flexible tubing.

25. A system according to claim 24

and further including an occluder bladder that inflates in response to the introduction of a predetermined amount of positive fluid pressure from the source element for contacting the occluding element and holding the occluding element in its flow enabled position as long as the predetermined positive pressure is maintained in the occluder bladder.

26. A system according to claim 25

wherein the occluder bladder is normally biased toward its flow disabled position,

and further including a vent for venting the positive pressure in the occluder bladder to cause the occluder bladder to move toward its normally biased flow disabled position.

27. A system according to claim 25

wherein the actuator includes a pressure transfer element communicating with the pressure conveying means and contacting the diaphragm to apply positive and negative fluid pressure to the diaphragm to move liquid through the pump chamber, and

further including means communicating with the occluder bladder for transporting positive fluid pressure from the occluder bladder to the pressure transfer element for conveyance to the diaphragm.

28. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber,

means for establishing flow communication between the pumping mechanism and a patient'"'"'s peritoneal cavity,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support, and

a pressure conveying element carried within the housing for conveying fluid pressure to the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity, the pressure conveying element including

a pressure transfer element in contact against the diaphragm of the pump chamber to convey fluid pressure to the diaphragm to move liquid through the pump chamber,

a reservoir that inflates in response to the introduction of positive fluid pressure for contacting the pressure transfer element to hold it in contact against the diaphragm,

conduit means for transporting positive fluid pressure from a source to the reservoir element, and

pressure regulator means communicating with the reservoir element and the pressure transfer element for maintaining positive fluid pressure in the reservoir element above a predetermined amount to maintain contact between the pressure transfer element and the diaphragm while transporting positive fluid pressure from the reservoir element to the pressure transfer element for conveyance to the diaphragm.

29. A system according to claim 28

and further including a carrier for moving the pressure transfer element between an at rest position away from the diaphragm and an operating position in contact against the diaphragm, and

and wherein the reservoir element, during inflation, moves the carrier from the at rest position toward the operative position.

30. A system according to claim 29

and further including means for biasing the carrier toward the at rest position.

31. A system according to claim 28

wherein the housing includes a door movable between an opened position permitting access to the support and a closed position blocking access to the support.

32. A system according to claim 28

wherein the pressure transfer element conveys both positive fluid pressure from the reservoir element and negative fluid pressure from another source.

33. A system according to claim 28

wherein the pressure transfer element conveys pneumatic pressure.

34. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber, and

means for establishing flow communication between the pumping mechanism and a patient'"'"'s peritoneal cavity including flexible tubing attached to the cassette,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support, and

a pressure conveying element carried within the housing for conveying fluid pressure to the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity,

an occluding element movable when the cassette and tubing are received in the support between a normally biased flow disabled position contacting and crimping closed the tubing and a flow enabled position out of contact with the tubing,

an occluder bladder that inflates in response to the introduction of a predetermined amount of positive fluid pressure for contacting the occluding element and holding the occluding element in its flow enabled position as long as the predetermined positive pressure is maintained in the occluder bladder, and

occlusion control means operative in a first control condition for conveying positive fluid pressure from a source to the occluder bladder to maintain the predetermined positive pressure, the occlusion control means being further operative in a second control condition for venting the positive pressure in the occluder bladder to cause the occluder element to move into its normally biased flow disable position.

35. A system according to claim 34

wherein the occlusion control means is also operative for conveying fluid pressure from the bladder to the pressure transfer element for conveyance to the diaphragm.

36. A system according to claim 34

wherein the tubing includes patient tubing attachable in communication with the patient'"'"'s peritoneal cavity and second tubing attachable to an external component, and

wherein, when in its flow disabled position, the occluding element simultaneously contacts and crimps closed both the patient tubing and the second tubing.

37. A system according to claim 34

wherein the tubing includes patient tubing attachable in communication with the patient'"'"'s peritoneal cavity and a plurality of additional tubing attachable to external components, and

wherein, when in its flow disabled position, the occluding element simultaneously contacts and crimps closed the patient tubing and the plurality of additional tubing.

38. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber,

means for establishing flow communication between the pumping mechanism and a patient'"'"'s peritoneal cavity,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support, and

a pressure conveying element carried within the housing for conveying fluid pressure including a pressure transfer element movable into contact with the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity, and

a reservoir that inflates in response to the introduction of fluid pressure to contact the pressure transfer element and hold it in contact against the diaphragm.

39. A system according to claim 38

and further including means communicating with the reservoir for transporting fluid pressure from the reservoir to the pressure transfer element for conveyance to the diaphragm.

40. A system according to claim 38

wherein the pressure transfer element carries a membrane that contacts the diaphragm and transmits fluid pressure while blocking liquid.

41. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber,

means for establishing flow communication between the pumping mechanism and a patient'"'"'s peritoneal cavity,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support, and

a pressure conveying element carried within the housing for conveying fluid pressure including a pressure transfer element and a carrier that moves the pressure transfer element between an at rest position away from the diaphragm and an operating position in contact against the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity, and

a reservoir that inflates in response to the introduction of positive pressure to move the carrier from the at rest position to the operating position.

42. A system according to claim 41

wherein the actuator includes means for biasing the carrier toward the at rest position.

43. A system according to claim 41

wherein the pressure transfer element carries a membrane that contacts the diaphragm and transmits fluid pressure while blocking liquid.

44. A system for performing peritoneal dialysis comprising

a liquid distribution cassette including

a pumping mechanism comprising a pump chamber, a diaphragm associated with the pump chamber and at least one valve communicating with the pump chamber,

means for establishing flow communication between the pumping mechanism and a patient'"'"'s peritoneal cavity including flexible tubing attached to the cassette,

an actuator for the cassette including

a support for the cassette body,

a housing enclosing the support, and

a pressure conveying element carried within the housing for conveying fluid pressure including a pressure transfer element contacting the diaphragm to operate the pump chamber and valve to move dialysis liquid either to or from the patient'"'"'s peritoneal cavity, and

an occluding element operative when the cassette and flexible tubing are received in the support for movement between a flow disabled position contacting and crimping closed the flexible tubing and a flow enabled position out of contact with the flexible tubing, including an occluder bladder that inflates in response to the introduction of a predetermined amount of positive fluid pressure for contacting the occluding element and holding the occluding element in its flow enabled position as long as the predetermined positive pressure is maintained in the occluder bladder.

45. A system according to claim 44

wherein the occluder bladder is normally biased toward its flow disabled position,

and further including a vent for venting the positive pressure in the occluder bladder to cause the occluder bladder to move toward its normally biased flow disabled position

46. A system according to claim 44

wherein the actuator includes a pressure transfer element communicating with the pressure conveying means and contacting the diaphragm to convey fluid pressure to the diaphragm to move liquid through the pump chamber, and

further including means communicating with the occluder bladder for transporting fluid pressure from the occluder bladder to the pressure transfer element for conveyance to the diaphragm.
