1. A surgical suturing instrument comprising:
- an elongate handle having opposite proximal and distal ends;
a projecting coil on the distal end of the handle, the projecting coil extending from the handle distal end through a curve of the coil to a pointed tip of the coil, the pointed tip and curve being configured to enable penetration of the pointed tip through body tissue end passage of the coil through the penetration, the coil is a hollow tube having an interior bore that extends through the coil, the interior bore exits the coil through a bore opening at the pointed tip, a length of filament extends through the interior bore to a distal end of the filament adjacent the coil bore opening, the filament having a loop at its distal end for holding a length of suture, and means are connected to the length of filament for selectively extending the loop from the coil interior bore through the bore opening, and for selectively retracting the filament loop into the coil interior bore through the bore opening; and
,the handle has a hollow interior that communicates with the interior bore of the coil, and the means for selectively extending the loop from the coil interior bore and selectively retracting the loop into the coil interior bore includes a sliding block connected to the length of filament and received in the handle interior for reciprocating sliding movement therein, an oblong slot through the handle to the handle hollow interior, and a manual knob connected to the sliding block and extending through the oblong slot from the handle interior.

A surgical instrument for use in placing suture or fasteners in tissue at a remote location such as a laproscopic surgery is comprised of an elongated handle having a coiled projection at its distal end. The coiled projection is employed in penetrating and positioning a length of suture or fastener in tissue, for example, as in tissue proximation.

an elongate handle having opposite proximal and distal ends; a protecting coil on the distal end of the handle, the projecting coil extending from the handle distal end through a curve of the coil to a pointed tip of the coil, the pointed tip and curve being configured to enable penetration of the pointed tip through body tissue and passage of the coil through the penetration, the coil is a hollow tube having an interior bore that extends through the coil, the interior bore exits the coil through a bore opening at the pointed tip, a length of filament extends through the interior bore to distal end of the filament adjacent the coil bore opening, the filament having a loop at its distal end for holding a length of suture, and means are connected to the length of filament for selectively extending the loop from the coil interior bore through the bore opening, and for selectively retracting the filament loop into the coil interior bore through the bore opening; and

A method of inserting a ligature into body tissue using an instrument having an elongate handle with a center axis and opposite distal and proximal ends, and having a helically curved projection at the handle distal end that curves around an axis of the projection parallel to the handle axis to a tip of the projection, the projection having means for holding a ligature proximate to its tip, the method comprising:

inserting a ligature into the means for holding a ligature; penetrating the body tissue with the projection tip by rotating the handle in a first direction about its center axis; passing the curved projection and the ligature held by the means for holding a ligature through the tissue penetrated by the projection tip; and
,retracting the curved projection and the means for holding a ligature from the tissue penetration by rotating the handle in a second direction about its center axis while leaving the ligature in the tissue penetration; the means for holding a ligature includes an opening in the curved projection and a filament loop that is selectively extendable from and retractable into the opening, the ligature is a length of suture, and the step of inserting a ligature into the means for holding a ligature includes extending the filament loop from the opening, inserting at least a portion of the length of suture into the filament loop, and retracting the filament loop back into the opening. - View Dependent Claims (9, 10)

A method of inserting a ligature into body tissue using an instrument having an elongate handle with a center axis and opposite distal and proximal ends, and having a helically curved projection at the handle distal end that curves around an axis of the projection parallel to the handle axis to a tip of the projection, the projection having means for holding a ligature proximate to its tip, the method comprising:

penetrating the body tissue with the projection tip by rotating the handle in a first direction about its center axis; passing the curved projection and the means for holding a ligature through the tissue penetrated by the projection tip; inserting a ligature into the means for holding a ligature; retracting the curved projection from the tissue penetration and simultaneously pulling the ligature through the tissue penetration by rotating the handle in a second direction about its center axis; and removing the ligature from the means for holding a ligature. - View Dependent Claims (12, 13, 14)
