Abstract

Apparatus for supplying liquid to a body cavity during an endoscopic procedure includes a feedback loop-controlled liquid supply device inherently capable of supplying liquid at a substantially constant pressure substantially independent of the flow rate of liquid delivered by the liquid supply device within a relatively wide range of flow rate. Also disclosed are a disposable plastic pump cassette having an inflow pump in a housing, and an operative cannula. The surgical procedure is performed with continuous control over the body cavity pressure, regardless of the outflow flow rate.

Claims

1. Apparatus for supplying liquid to a body cavity during an endoscopic procedure, which comprises:

a) liquid supply means for supplying liquid under pressure;

b) conduit means, including an inlet coupled to said liquid supply means and an outlet for delivering said liquid under pressure to said body cavity;

c) pressure signal generating means for generating a body cavity pressure signal representing a pressure in said body cavity, including;

pressure sensor means in communication with said liquid under pressure at a location upstream of said conduit means outlet for producing a sensed pressure signal indicative of a sensed pressure at said upstream location; and

pressure signal processing means responsive to said sensed pressure signal for generating said body cavity pressure signal from said sensed pressure signal as a function of the pressure drops between said liquid supply means to said pressure sensor means and between said pressure sensor means to said body cavity; and

d) control means coupled to said liquid supply means and responsive to said body cavity pressure signal for increasing or decreasing the pressure of said liquid under pressure delivered by said liquid supply means to thereby maintain said pressure in said body cavity at a predetermined value.

3. Apparatus according to claim 1 or claim 2, wherein said liquid supply means comprises a dynamic pump.

4. Apparatus according to claim 3, wherein said dynamic pump comprises a centrifugal inflow pump.

5. Apparatus according to claim 3, wherein said location of said pressure sensor means is adjacent an outlet of said dynamic pump.

Apparatus according to claim 5, wherein said pressure sensor means comprises a chamber having an open end, a thin, flexible diaphragm closing said open end, a channel for directing liquid delivered from said pump outlet to said chamber, and a pressure transducer in operative contact with said flexible diaphragm.

7. Apparatus according to claim 4, wherein said location of said pressure sensor means is adjacent an outlet of said centrifugal inflow pump.

Apparatus according to claim 7, wherein said pressure sensor means comprises a chamber having an open end, a thin, flexible diaphragm closing said open end, a channel for directing liquid delivered from said pump outlet to said chamber, and a pressure transducer in operative contact with said flexible diaphragm.

Apparatus according to claim 4, including a motor for driving said centrifugal inflow pump and a tachometer for generating a tachometer signal indicating the actual speed of said motor, wherein said pressure signal processing means comprises input means for receiving said tachometer signal, said sensed pressure signal, and a signal indicating a desired set pressure, a processor for generating from said tachometer signal, said sensed pressure signal and said desired set pressure signal a target tachometer signal as said calculated body cavity signal as a function of the pressure drops between said liquid supply means to said sensor means and between said sensor means to said body cavity, said target tachometer signal representing the motor speed necessary to increase or decrease the pressure of liquid delivered by said centrifugal inflow pump to thereby maintain said body cavity pressure at said predetermined value.

10. Apparatus of claim 1 or claim 2, further comprising a display for displaying said body cavity pressure in response to said body cavity pressure signal.

11. Apparatus of claim 1 or claim 2, wherein said liquid supply means includes a pump and means producing a pump speed signal indicative of a speed of operation of said pump,

wherein said pressure signal processing means is further responsive to said pump speed signal for generating said body cavity pressure signal as a function of said sensed pressure signal and said pump speed signal, and

wherein said control means is structured to increase or decrease the pressure of said liquid under pressure delivered by said liquid supply means by increasing or decreasing said speed of operation of said pump.

12. Apparatus according to claim 1 or claim 2, wherein a liquid outflow cannula is provided for insertion into said body cavity and a source of suction is provided communicating with said outflow cannula for aspirating liquid from said body cavity via said outflow cannula.

13. Apparatus according to claim 12, wherein said source of suction is a suction pump.

Apparatus according to claim 12, wherein said conduit means further includes tubing connecting said source of suction to said liquid outflow cannula and a crimping mechanism is provided selectively to crimp said tubing and thus selectively restrict flow of liquid aspirated from said body cavity.

Apparatus according to claim 12, wherein said pressure sensor means comprises a pressure transducer and an operative pressure-sensing diaphragm, and said pressure signal generating means includes a channel for directing a column of liquid from liquid flowing through said conduit means to said diaphragm.

Apparatus according to claim 12, wherein said pressure sensor means comprises a chamber having an open end, a thin, flexible diaphragm closing said open end, a channel for directing liquid delivered from said liquid supply means to said chamber, and a pressure transducer in operative contact with said flexible diaphragm.

17. Apparatus of claim 11, wherein said pressure signal processing means further generates said body cavity pressure signal as a function of said sensed pressure signal, said pump speed signal and at least one empirically predetermined constant.

18. Apparatus of claim 11, wherein said pressure signal processing means generates said body cavity pressure signal according to the relation:

where P_s represents the sensed pressure, P_c represents the body cavity pressure, A₁ and A₂ represent empirically predetermined constants, and Q_in represents a flow rate of said liquid under pressure, wherein Q_in is determined as a function of P_s and said pump speed signal.

2. Apparatus for supplying liquid to a body cavity during an endoscopic procedure, which comprises:

a) liquid supply means for supplying liquid under pressure;

b) conduit means, including an inlet coupled to said liquid supply means and an outlet for delivering said liquid under pressure to said body cavity;

c) pressure signal generating means for generating a body cavity pressure signal representing a pressure in said body cavity, including;

pressure sensor means in communication with said liquid under pressure at a location upstream of said conduit means outlet for producing a sensed pressure signal indicative of a sensed pressure at said upstream location; and

pressure signal processing means responsive to said sensed pressure signal for generating said body cavity pressure signal as a function of at least said sensed pressure signal; and

d) control means coupled to said liquid supply means and responsive to said body cavity pressure signal for increasing or decreasing the pressure of said liquid under pressure delivered by said liquid supply means to thereby maintain said pressure in said body cavity at a predetermined value.
