First Claim
1. A method of determining a patient'"'"'s cardiac stimulation threshold, comprising the steps of:
- (a) defining a capture detect window of predetermined duration and initiating said window at a predefined time following delivery of any stimulating pulse to a patient'"'"'s heart, during which a cardiac impedance signal is sensed and measured;
(b) delivering to a patient'"'"'s heart at least one control stimulating pulse having sufficient energy predetermined to ensure cardiac capture;
(c) deriving at least one control value which characterizes the morphology of said cardiac impedance signal sensed and measured during said predefined capture detect window following delivery of said at least one stimulating pulse in step (b);
(d) delivering to said patient'"'"'s heart at least one series of stimulating pulses having successively lower energy levels;
(e) for each one pulse of said series of stimulating pulses delivered in step (d) above, deriving at least one test value corresponding to said each one pulse, wherein said at least one corresponding test value characterizes the morphology of said cardiac impedance signal signal sensed and measured during said capture detect window following said each one of said series of stimulating pulses;
(f) for each test value derived in step (e), determining whether a difference between said each test value and said control value lies within a predetermined range;
(g) for each one pulse of said series of stimulating pulses delivered in step (d), determining whether a cardiac electrical event was sensed during said capture detect window following said each one of said series of stimulating pulses;
(h) discontinuing said at least one series of stimulating pulses of successively lower energy levels when any one of said determinations in step (f) indicates that said difference between said test value for a given one pulse of said series of stimulating pulses and said control value does not lie within said predetermined range and therefore that capture was not achieved by said given one pulse of said series of stimulating pulses;
(i) discontinuing said at least one series of stimulating pulses of successively lower energy levels when any one of said determinations in step (g) indicates a cardiac electrical event reflecting a loss of capture was sensed during said capture detect window following a given one pulse of said series of stimulating pulses; and
(j) identifying said patient'"'"'s pacing threshold as being at least a predetermined incremental amount greater than the energy level of the last in said series of test stimulation pulses delivered prior to discontinuance in step (h) or step (l).

Abstract
A method and apparatus for automatic determination of a pacemaker patient'"'"'s pacing stimulation threshold. Circuitry is provided in a pacemaker for obtaining a signal reflecting cardiac impedance, which is known to reliably reflect certain aspects of cardiac function. Circuitry is also provided for monitoring the cardiac impedance waveform during a predetermined capture detect window following delivery of stimulating pulses. One or more values are derived which characterize the morphology of the impedance waveform during the capture detect window associated with each stimulation pulse delivered. These values are compared to predetermined control values in order to assess whether a stimulation pulse has achieved cardiac capture. The assessment of whether cardiac capture has been achieved is also based partly upon the conventional sensing of atrial and/or ventricular cardiac signals occurring during the capture detect window. In one embodiment of the invention, the control values against which impedance waveform characterization values are compared are obtained by delivering a series of stimulation pulses having sufficient energy to ensure that capture is achieved, and by monitoring the impedance waveform during delivery of these pulses.

(a) delivering at least one control stimulating pulse having predetermined sufficient energy to ensure cardiac capture; (b) sensing and then based on that sense, providing at least one value that provide a means for characterizing the morphology of a cardiac impedance signal during a predetermined time period after said delivery of said test stimulating pulse in step (a); (c) delivering a series of test stimulating pulses having successively lower energy levels; (d) for each test stimulating pulse delivered in step (c), comparing the at least one value that provide a means for characterizing the morphology of said cardiac impedance signal with said at least one value that provide a means for characterizing the characterization of morphology for said control stimulating pulse; (e) for each test stimulating pulse delivered in step (c), sensing cardiac electrical events occurring during said predetermined time period following said test stimulating pulse; (f) for each test stimulating pulse delivered in step (c), assessing based on said comparison in step (d) and said sensing in step (e) whether cardiac capture was achieved by said each test stimulating pulse; (g) discontinuing said delivery of said series of test stimulating pulses when said assessment in step (f) indicates that capture was not achieved for a given test pulse; (g) determining that said patient'"'"'s pacing threshold lies at least a predetermined incremental level above the energy level of said given test pulse. - View Dependent Claims (10, 11, 12, 13, 14, 15, 16, 17)

(a) defining a capture detect window of predetermined duration and initiating said window at a predefined time following delivery of any stimulating pulse to a patient'"'"'s heart, during which a measurable physiological characteristic is monitored; (b) delivering to a patient'"'"'s heart at least one control stimulating pulse having sufficient energy to ensure cardiac capture; (c) deriving at least one control value which characterizes the physiological characteristic monitored during said predefined capture detect window following delivery of said at least one stimulating pulse in step (b); (d) delivering to said patient'"'"'s heart at least one series of stimulating pulses having successively lower energy levels; (e) for each one of said series of stimulating pulses delivered in step (d) above, deriving at least one test value corresponding to said each one pulse, wherein said at least one corresponding test value characterizes said physiological characteristic during said capture detect window following said each one of said series of stimulating pulses; (f) for each test value derived in step (e), determining whether the difference between said each test value and said control value lies within a predetermined range; (g) for each one of said series of stimulating pulses delivered in step (d), determining whether a cardiac electrical event was sensed during said capture detect window following said each one of said series of stimulating pulses; (h) discontinuing said at least one series of stimulating pulses of successively lower energy levels when any one of said determinations in step (f) indicates that said difference between said test value for a given one of said series of stimulating pulses and said control value does not lie within said predetermined range and therefore that capture was not achieved by said given one of said series of stimulating pulses; (i) discontinuing said at least one series of stimulating pulses of successively lower energy levels when any one of said determinations in step (g) indicates a cardiac electrical event reflecting a loss of capture was sensed during said capture detect window following a given one of said series of stimulating pulses; and (j) identifying said patient'"'"'s pacing threshold as being at least a predetermined incremental amount greater than the energy level of the last in said series of test stimulation pulses delivered prior to discontinuance in step (h) or step (i).
