Abstract

A localized cardiac measure, such as a localized measure of myocardial ischemia, is obtained by applying sensors to a subject, where the sensors are configured to produce electrical signals representative of cardiac activity of the subject. The subject'"'"'s heart is then physiologically stressed by, for example, exercise stress testing, and electrical signals are received from at least two of the sensors. The received signals then are processed to obtain a localized cardiac measure.

Claims

1. A method for obtaining a localized cardiac measure, the method comprising:

applying sensors to a subject, the sensors being configured and positioned to produce electrical signals representative of localized cardiac activity of the subject;

physiologically stressing the heart of the subject;

receiving electrical signals from at least two of the sensors;

processing the received signals to obtain a localized cardiac measure.

2. The method of claim 1 wherein the electrical signals include information related to myocardial ischemia and the processing step comprises generating a localized measure of myocardial ischemia as the localized cardiac measure.

3. The method of claim 1 wherein the processing step comprises generating a localized electrocardiogram as the localized cardiac measure.

4. The method of claim 3 wherein the processing step comprises generating the localized electrocardiogram as a localized measure of myocardial ischemia.

5. The method of claim 3, wherein the localized electrocardiogram includes an ST segment, the method further comprising determining ST segment change due to the physiologic stressing using the localized electrocardiogram.

6. The method of claim 3 wherein the localized electrocardiogram includes an ST segment and the processing step includes determining a normalized value of ST segment change due to the physiologic stressing.

7. The method of claim 3 wherein the localized electrocardiogram includes an ST segment and the processing step further includes determining characteristics of a body surface spatial distribution of ST segment changes due to the physiologic stressing.

8. The method of claim 1 wherein the processing step further comprises obtaining localized cardiac measures at multiple locations.

9. The method of claim 1 further comprising enhancing a signal-to-noise characteristic of the localized cardiac measure.

10. The method of claim 9 wherein the step of enhancing comprises using an averaging process based on a measure of statistical central tendency.

11. The method of claim 1 further comprising normalizing the localized cardiac measure.

12. The method of claim 11, further comprising generating a standard electrocardiogram and generating a parameter from the standard electrocardiogram, wherein the step of normalizing includes using the parameter generated from the standard electrocardiogram.

13. The method of claim 11, further comprising generating a localized electrocardiogram and generating a parameter from the localized electrocardiogram, wherein the step of normalizing includes using the parameter generated from the localized electrocardiogram.

14. The method of claim 1 wherein the processing step includes generating the localized cardiac measure by combining the received signals.

15. The method of claim 1, wherein the electrical signals include information related to myocardial ischemia, the method further comprising analyzing the localized cardiac measure to detect myocardial ischemia.

16. The method of claim 15 wherein the analyzing step includes visual interpretation of the localized cardiac measure.

17. The method of claim 15, generating a parameter from the localized cardiac measure and generating a graphical representation of the parameter, wherein the analyzing step includes visual interpretation of the graphical representation of the parameter of the localized cardiac measure.

18. The method of claim 1 wherein the physiological stressing of the heart includes stressing the heart according to a predetermined protocol.

19. The method of claim 1 wherein the physiological stressing of the heart includes exercise stress testing.

20. The method of claim 1 wherein the physiological stressing of the heart includes alteration of the physiologic condition of the subject by naturally occurring stress.

21. The method of claim 20, further comprising attaching the sensors to a portable recording device, wherein the received signals comprise signals received using the portable recording device.

22. The method of claim 1 wherein the physiological stressing of the heart includes stress testing using pharmacological agents.

23. The method of claim 1 wherein the processing step includes generating an approximation to a body surface spatial differential of an electrocardiogram as the localized cardiac measure.

24. The method of claim 23 wherein the processing step includes generating an approximation to a body surface Laplacian as the localized cardiac measure.

25. The method of claim 1 wherein the processing step includes generating a spatially-high-pass-filtered body surface potential as the localized cardiac measure.

26. The method of claim 1 further comprising processing the received signals to remove baseline noise using an impedance signal estimate.

27. The method of claim 1 wherein the processing step includes reconstruction of a standard electrocardiogram by transformation of the signals received from the sensors.

28. The method of claim 1 further comprising presenting a display of the localized cardiac measure.

29. The method of claim 28 wherein localized cardiac measures are obtained at multiple locations and the display includes a map of the localized cardiac measures at different locations.

30. The method of claim 29 wherein the map comprises a map that uses colors to represent ranges of values of the localized measures.

31. The method of claim 28 wherein localized cardiac measures are obtained at multiple locations and the display includes a graphical representation of the localized measures at positions corresponding to locations of the sensors.

32. The method of claim 28 wherein the display comprises a graphical image with superimposed cardiac measures.

33. The method of claim 1 further comprising displaying a plot of a time-dependence of a quantity derived from the localized cardiac measure.

34. The method of claim 1 further including estimating the localized cardiac measure at locations other than the locations of the received signals.

35. A system for obtaining a localized cardiac measure, the system including:

sensors configured and positioned to produce electrical signals representative of localized cardiac activity of the subject;

means for physiologically stressing the heart of the subject;

means for receiving electrical signals from at least two of the sensors; and

means for processing the received signals to obtain a localized cardiac measure.
