Abstract

A method and device for the continuous real-time monitoring of relative blood volume change, based on registration of blood hemoglobin concentration, during long periods of time, such as dialysis session. A method and device for cardiac output measurement, based on optical dilution technique, during dialysis, surgeries, intensive care procedure. The effects of blood electrolyte composition change which result in a change of light beam geometry are eliminated by the relative orientation between a light source and a single light or photodetector. An illumination axis and a detection axis are oriented in an offset, non colinear configuration at a sufficient angle to substantially eliminate the scattering properties of the blood. A primary implementation of the device is extracorporeal paths, such as hemodialysis tubing systems, artery-vein extracorporeal artificial shunts, other extracorporeal systems. It also may be applied to vessels, tissues or to body parts being capable of transillumination.

Claims

1. An apparatus for measuring blood parameters in a blood system, comprising:

(a) an extracorporeal portion having a first end adapted to be connected to a vascular portion of the blood system at an up stream end and a second end adapted to be connected to the vascular portion at a downstream end, the extracorporeal portion having a light transmissive section;

(b) an optical sensor coupled to the light transmissive section for detecting a photometric property of diluted blood flowing within the extracorporeal portion; and

(c) means for determining a selected blood characteristics corresponding to the detected photometric property of the diluted blood.

2. The apparatus of claim 1, further comprising means for introducing an indicator into the blood system upstream of the optical sensor.

3. The apparatus of claim 2, wherein the extracorporeal portion includes an arterio-veneous shunt.

4. The apparatus of claim 2, wherein the means for introducing an indicator includes one of a first injection port in the extracorporeal portion and a second injection port in the vascular portion.

5. The apparatus of claim 1, further including a flow measuring ultrasound sensor in the extracorporeal portion.

6. The apparatus of claim 1, wherein the optical sensor is configured to correct for scatter effect of blood caused by variation in one of electrolytes, composition of the blood and variations in blood flow.

7. The apparatus of claim 1, wherein the optical sensor is sensitive to scatter effect.

8. The apparatus of claim 1, wherein the optical sensor employs an isobestic wavelength.

9. The apparatus of claim 1, wherein the selected blood characteristic corresponding to the detected photometric property of the diluted blood is one of a cardiac output and a flow parameter.

10. A method for measuring blood parameters in a blood system having a vascular portion, the method comprising:

(a) connecting an extracorporeal portion to the vascular portion at an upstream end and connecting the extracorporeal portion to the vascular portion at a downstream end, the extracorporeal portion including a light transmissive section;

(b) introducing an indicator bolus upstream of an optical sensor in the extracorporeal portion;

(c) determining a photometric property of diluted blood in the extracorporeal portion; and

(d) determining a blood characteristic corresponding to the diluted photometric property of the blood.

11. The method of claim 10, further comprising introducing an indicator into the blood system upstream of the optical sensor.

12. The method of claim 10, further comprising correcting said determined blood characteristic for a scatter effect of blood caused by a variation in one of electrolytes, composition of the blood and variations in blood flow.

13. The method of claim 10, further comprising locating an ultrasound sensor in the extracorporeal portion, the ultrasound sensor selected to measure flow in the extracorporeal portion.

14. The method of claim 10, further comprising determining a photometric property of diluted blood in the extracorporeal portion with an isobestic wavelength.

15. The method of claim 10, further comprising introducing an indicator into one of the vascular portion and the extracorporeal portion upstream of the optical sensor.
