A method to diagnose the presence of a disorder associated with connective tissue metabolism abnormalities, which method comprisescomparing the level of native peptide-free crosslinks derived from collagen in a non-hydrolyzed biological fluid of a human subject with the level of said crosslinks in normal subjects so as to diagnose a subject having an enhanced level of said crosslinks as showing the presence of said disorder,wherein said crosslinks are selected from native non-glycosylated peptide-free pyridinoline (N-Pyd), native peptide-free deoxypyridinoline (N-Dpd), or a combination thereof.

Abstract
The invention is directed to methods to assess connective tissue, especially bone, metabolism in disease or to monitor therapy, which method comprises assessing the levels of native free collagen-derived crosslinks in biological fluids, especially urine. The method can be enhanced by concomitantly determining the levels of an indicator of bone formation in biological fluids of the same individual and assessing the differences between the degradation marker and the formation indicator. Antibodies which are specifically immunoreactive with forms of crosslinks which occur free in biological fluids are also disclosed.

comparing the level of native peptide-free crosslinks derived from collagen in a non-hydrolyzed biological fluid of a human subject with the level of said crosslinks in normal subjects so as to diagnose a subject having an enhanced level of said crosslinks as showing the presence of said disorder, wherein said crosslinks are selected from native non-glycosylated peptide-free pyridinoline (N-Pyd), native peptide-free deoxypyridinoline (N-Dpd), or a combination thereof.

15. A method to monitor the effects of a therapy protocol in a human subject, which method comprises
comparing the level of native peptide-free crosslinks derived from collagen in a non-hydrolyzed biological fluid of the subject at various time points within and/or after the protocol with the level of said crosslinks prior to administration of the protocol so as to evaluate a subject having a decreased level of said crosslinks as showing the effectiveness of said protocol wherein said crosslinks are selected from native non-glycosylated peptide-free pyridinoline (N-Pyd), native peptide-free deoxypyridinoline (N-Dpd), or a combination thereof.

A method to determine the breakdown of connective tissue, which method comprises determining the ratio of Pyd/Dpd in a non-hydrolyzed biological fluid of a subject and comparing said ratio to that in normal controls, wherein an increase in said ratio in said subject over normal controls indicates connective tissue breakdown in said subject.
