First Claim
1. An expandable fastener for joining adjacent bone portions, comprising:
- a) a sleeve having a flange portion and a plurality of prongs;
each of said prongs having a proximal end proximate said flange portion and a distal end distant from said flange portion;
each of said prongs further having at least one outwardly extending protrusion disposed near said distal end, said protrusions having an upper bearing surface generally sloped outwardly away from said flange portion;
b) a ram moveable between a first position and a second position with said ram disposed substantially within said sleeve in said second position; and
c) wherein, with said sleeve disposed between two adjacent bone portions, moving said ram from said first position to said second position expands said sleeve and forces at least one of said upper bearing surfaces of said protrusions into firm contact with at least one of said bone portions so as to constrain the relative motion between said bone portions.

Abstract
An expandable fastener is used to join adjacent portions of bones for surgical recovery. The fastener typically includes a sleeve having an expandable section and a ram that is moveable between a ready position and a deployed position. When the ram is moved to the deployed position, the ram forces the sleeve to expand against at least one of the bone sections. Preferably, the sleeve includes a plurality of prongs in the expandable section that are forced outwardly when the ram is moved to the deployed position. The prongs may include protrusions having bearing surfaces that curve outwardly from the bore of the sleeve. When the sleeve is expanded, the protrusion bearing surfaces apply both a lateral and a radial force, with the radial force acting to clamp one or both of the bone portions between the prong and a flange on the sleeve. In some embodiments, the fastener is designed to be shorter than the surrounding bone is thick so that the fastener may be used to secure adjacent bone sections without extending into the cranial cavity. A method of using the fastener is also disclosed wherein a fastener is inserted into at least one craniotomy burr hole and thereafter expanded, such as by driving the ram of the typical fastener down into to sleeve by screwing the ram into the sleeve. All burr holes may be filled by a fastener, thereby providing multi-point fixation of the bone flap to the surrounding cranial bone.

6. An expandable fastener for joining adjacent bone portions, said bone portions substantially defining a burr hole having a first diameter, comprising:

a) a sleeve having a bore, a plurality of prongs, and a flange portion;
each of said prongs having a proximal end proximate said flange portion and a distal end distant from said flange portion;
each of said prongs further having at least one outwardly extending protrusion disposed near said distal end, said protrusions having an upper bearing surface generally sloped outwardly away from said flange portion, each of said prongs further having a spur extending into said bore generally opposite the corresponding protrusion;b) a ram moveable between a first position and a second position, wherein the majority of said ram is external to said sleeve in said first position and wherein said ram is disposed substantially within said sleeve and said ram does not extend above said flange portion in said second position; c) wherein, with said sleeve disposed between two adjacent bone portions, moving said ram from said first position to said second position i) expands said sleeve and forces at least one of said upper bearing surfaces of said protrusions into firm contact with at least one of said bone portions so as to constrain the relative lateral and radial motion between said bone portions, and ii) generates a compressive force between at least one of said prongs and said flange portion, clamping at least a portion of at least one of said bone portions therebetween; d) wherein said prongs, before said sleeve expansion, fit within a cylindrical space having a diameter smaller than said first diameter; and e) wherein said flange portion has a lateral dimension larger than said first diameter. - View Dependent Claims (7)

8. A method of joining first and second bone portions using an expandable fastener, comprising:

a) positioning the first and second bone portions so as to be adjacent on along at least one edge; b) forming at least one burr hole substantially defined by the adjacent bone portions; c) inserting the expandable fastener into said burr hole;
said fastener having an expandable section; andd) thereafter, with said bone portions adjacent to one another, expanding said expandable section into firm contact at least one of said first and second bone sections so as to constrain the relative motion between said first and second bone sections. - View Dependent Claims (9, 10, 11, 12, 13, 14, 15)

16. A method of closing a craniotomy using an expandable fastener, comprising:

a) positioning a bone flap in an opening formed in surrounding cranial bone so as to be adjacent along at least one edge with said surrounding cranial bone, said bone flap substantially filling said opening, said bone flap and said surrounding cranial bone collectively substantially defining at least one burr hole; b) situating an expandable fastener at least partially in said burr hole;
said fastener having at least one flange, an expansion driver, and an expandable section having at least one protrusion associated therewith;
said fastener disposed so that said flange rests outside the cranial cavity; andc) thereafter, expanding said expandable section of said fastener by moving said expansion driver from a first position to a second position so as to clamp at least a portion of said surrounding bone between said protrusion and said flange, thereby constraining the relative motion between said bone flap and said surrounding cranial bone. - View Dependent Claims (17, 18, 19, 20)
