Abstract

An implantable lead is provided with at least one extendable member to position therapy delivery elements, which may be electrodes or drug delivery ports, after the lead has been inserted into the body. The lead may be formed as a resilient element which is contained in a retainer tube that may be removed to permit the lead to deploy. Alternatively, a non-resilient lead may be provided with a slotted retainer tube. A series of mechanical linkages for expanding and retracting the lead within the human body may be actuated with various mechanisms. A control system may be provided for closed-loop feedback control of the position of the extendable members. The invention also includes a method for expanding an implantable lead in situ.

Claims

1. An implantable lead system for providing therapy to a body comprising:

an elongate central portion;

at least one extendable member having an end, the extendable member depending from the central portion and being adapted to assume a compact position, wherein the end is disposed in close proximity to the central portion and an extended position, wherein the end is disposed at a location distal from the central portion;

at least one therapy delivery element disposed on the extendable member for delivering stimulation to the body;

an actuating assembly for permitting adjustment of the position of the extendable member in situ;

a sensor for sensing an effect of stimulation provided by the therapy delivery element; and

a closed-loop feedback controller for operating the actuating assembly to restore the effect of stimulation provided by the therapy delivery element to a predetermined value.

2. The implantable lead according to claim 1, wherein the extendable member is formed as a coaxial accessory tube mounted to the central portion and including a central slot forming at least two flexible leaf portions.

3. The implantable lead according to claim 2, wherein the at least two flexible leaf portions each have a bending joint formed therein.

4. The implantable lead according to claim 3, wherein the bending joint is facilitated by a recess formed in the leaf portion.

5. The implantable lead according to claim 3, further comprising at least one central electrode disposed on the central portion.

6. The implantable lead according to claim 5, wherein the central electrode is not exposed by the leaf portions in the compact position and exposed in the extended position.

7. The implantable lead according to claim 3, wherein the actuating assembly comprises an actuator movable relative to the central portion for moving a lower end of the coaxial accessory tube to cause the leaf portions to move to the extended position.

8. The implantable lead according to claim 7, wherein the actuator is coaxially mounted on the central portion.

9. The implantable lead according to claim 7, wherein the actuator is slidably mounted on the central portion.

10. The implantable lead according to claim 7, wherein threads are provided on the central portion and wherein the actuator is a screw actuator adapted to cooperate with the threads.

11. The implantable lead according to claim 10, further comprising an actuation driver for releasably engaging the screw actuator.

12. The implantable lead according to claim 2, wherein a lower end of the accessory tube is adapted to slide with respect to the central portion, the flexible leaf portions adapted to extend outward as the lower end slides.

13. The implantable lead according to claim 2, wherein a lower end of the accessory tube is adapted to slide with respect to the central portion.

14. The implantable lead according to claim 13, wherein the actuating assembly comprises a screw mechanism for adjusting the position of the lower end of the accessory tube.

15. The implantable lead according to claim 1, wherein the actuating assembly comprises a linkage assembly for expanding the extendable member.

16. The implantable lead according to claim 15, wherein the linkage assembly comprises first and second actuating members adapted to move in a direction substantially parallel to an axis of the central portion.

17. The implantable lead according to claim 16, further comprising a mechanism for adjusting the relative positions of the first and second actuating members.

18. The implantable lead according to claim 17, wherein the mechanism comprises a rotatable circular component cooperatively associated with the first and second actuating members.

19. The implantable lead according to claim 15, wherein the linkage assembly comprises a rotatable shaft.

20. An implantable lead for providing therapy to a body comprising:

an elongate central lead body;

at least one extendable member having a therapy delivery element disposed thereon for delivering therapy to the body, the extendable member depending from the lead body and being adapted to assume a compact position, wherein the therapy delivery element is disposed in close proximity to the lead body, and an extended position, wherein the therapy delivery element is disposed at a location distal from the lead body;

the extendable member being formed as a coaxial accessory tube mounted on the lead body and including a central slot forming at least two flexible leaf portions, each leaf portion having a bending joint formed therein; and

at least one central electrode disposed on the lead body;

wherein the central electrode is not exposed by the leaf portions in the compact position and exposed in the extended position.

21. An implantable lead for providing therapy to a body comprising:

an elongate central lead body;

at least one extendable member having a therapy delivery element disposed thereon for delivering therapy to the body, the extendable member depending from the lead body and being adapted to assume a compact position, wherein the therapy delivery element is disposed in close proximity to the lead body, and an extended position, wherein the therapy delivery element is disposed at a location distal from the lead body;

the extendable member being formed as a coaxial accessory tube mounted on the lead body and including a central slot forming at least two flexible leaf portions, each leaf portion having a bending joint formed therein;

at least one central electrode disposed on the lead body; and

at least one ratchet ring formed on the lead body to prevent downward movement of a lower end of the coaxial accessory tube.
