Abstract

An electrosurgical instrument comprises an instrument shaft and an electrode assembly at one end of the shaft. The electrode assembly comprises a tissue treatment electrode and a return electrode which is insulated from the treatment electrode by means of an insulation member. The tissue treatment electrode has an exposed end for treating tissue, and the return electrode has a fluid contact surface which is spaced from the tissue treatment electrode in such a manner as to define a conductive fluid path that completes an electrical circuit between the tissue treatment electrode and the return electrode. The electrode assembly is provided with a plurality of apertures in the region of the tissue treatment electrode through which apertures vapor bubbles and/or particulate material can be aspirated from the region surrounding the tissue treatment electrode.

Claims

1. An electrosurgical instrument for the treatment of tissue in the presence of an electrically-conductive fluid medium, the instrument comprising an instrument shaft, and, an electrode assembly at one end of the shaft, the electrode assembly comprising a tissue treatment electrode and a return electrode which is electrically insulated from the tissue treatment electrode by means of an insulation member, the tissue treatment electrode having an exposed end for treating tissue, and the return electrode having a fluid contact surface which is spaced proximally from the tissue treatment electrode in such a manner as to define, in use, a conductive fluid path that completes an electrical circuit between the tissue treatment electrode and the return electrode, wherein the electrode assembly is provided with a plurality of apertures in the region of the tissue treatment electrode and distal to the return electrode, through which apertures vapour bubbles and/or particulate material can be aspirated from the region surrounding the tissue treatment electrode.

An electrosurgical instrument as claimed in claim 1, further comprising a pump for subjecting the distal end portion of the instrument shaft to a sub-atmospheric pressure thereby to aspirate, in use, vapour bubbles and/or particulate material through said apertures from the region surrounding the tissue treatment electrode.

3. An electrosurgical instrument as claimed in claim 2 wherein the pump is a cyclically activated pump whereby matter is aspirated in a pulsed fashion.

4. An electrosurgical instrument as claimed in claim 2 wherein the pump is connected to the RF generator and thereby activated only when the tissue treatment electrode is powered by the RF generator for tissue vaporisation.

5. An electrosurgical instrument as claimed in claim 2, further comprising an RF generator having a bipolar output connected to the tissue treatment electrode and the return electrode.

6. An electrosurgical instrument as claimed in claim 5, wherein the pump is connected to the RF generator'"'"'s voltage output, whereby operation of the pump depends upon the voltage output of the RF generator.

7. An electrosurgical instrument as claimed in claim 5, wherein the pump is connected to the RF generator'"'"'s voltage output, whereby operation of the pump depends upon the voltage output of the RF generator.

8. An electrosurgical instrument as claimed in claim 5, wherein the pump is connected to the RF generator'"'"'s voltage output, whereby operation of the pump depends upon the voltage output of the RF generator.

9. An electrosurgical instrument as claimed in claim 2, further comprising a tube positioned within the instrument shaft for connecting said apertures to the pump.

10. An electrosurgical instrument as claimed in claim 9, wherein the tube is a multi-lumen tube.

11. An electrosurgical instrument as claimed in claim 10, wherein the multi-lumen tube defines a plurality of channels, the diameter of each of said channels being at least equal to the diameter of the apertures in the region of the tissue treatment electrode.

12. An electrosurgical instrument as claimed in claim 9, further comprising a filter at the distal end of the tube.

13. An electrosurgical instrument as claimed in claim 2 wherein the pump is a cyclically activated pump whereby matter is aspirated in a pulsed fashion.

14. An electrosurgical instrument as claimed in claim 2 wherein the pump is connected to the RF generator and thereby activated only when the tissue treatment electrode is powered by the RF generator for tissue vaporisation.

15. An electrosurgical instrument as claimed in claim 2 wherein the pump is a cyclically activated pump whereby matter is aspirated in a pulsed fashion.

16. An electrosurgical instrument as claimed in claim 2 wherein the pump is connected to the RF generator and thereby activated only when the tissue treatment electrode is powered by the RF generator for tissue vaporisation.

17. An electrosurgical instrument as claimed in claim 1, wherein the exposed end of the tissue treatment electrode extends laterally through a cut-out provided in the insulation member at the distal end portion of the instrument, the fluid contact surface of the return electrode overlying the insulation member in the region of the cut-out.

18. An electrosurgical instrument as claimed in claim 17, wherein a single coiled filament constitutes the tissue treatment electrode.

19. An electrosurgical instrument as claimed in claim 18, wherein said apertures are formed in the insulation member, the apertures being positioned around, and adjacent to, the tissue treatment electrode.

20. An electrosurgical instrument as claimed in claim 18, wherein the filament has a diameter lying within the range of from 0.05 mm to 1.0 mm.

21. An electrosurgical instrument as claimed in claim 17, wherein an apertured plate constitutes the tissue treatment electrode, the apertures of the plate constituting the apertures through which vapour bubbles and/or particulate material can be aspirated.

22. An electrosurgical instrument as claimed in claim 21, wherein the outer surface of said plate is formed with recesses adjacent to the apertures.

23. An electrosurgical instrument as claimed in claim 1, wherein the tissue treatment electrode is made of tungsten.

24. An electrosurgical instrument is claimed in claim 1, wherein the tissue treatment electrode is made of an alloy of tungsten or platinum.

25. An electrode unit for an electrosurgical instrument for the treatment of tissue in the presence of an electrically-conductive fluid medium, the electrode unit comprising a shaft having at one end means for connection to an instrument handpiece, and, mounted on the other end of the shaft, an electrode assembly comprising a tissue treatment electrode and a return electrode which is electrically insulated from the tissue treatment electrode by means of an insulation member, the tissue treatment electrode having an exposed end for treating tissue, and the return electrode having a fluid contact surface which is spaced proximally from the tissue treatment electrode in such a manner as to define, in use, a conductive fluid path that completes an electrical circuit between the tissue treatment electrode and the return electrode, wherein the electrode assembly is provided with a plurality of apertures in the region of the tissue treatment electrode and distal to the return electrode, through which apertures vapour bubbles and/or particulate material can be aspirated from the region surrounding the tissue treatment electrode.

An electrode unit as claimed in claim 25, further comprising a pump for subjecting the distal end portion of the instrument shaft to a sub-atmospheric pressure thereby to aspirate, in use, vapour bubbles and/or particulate material through said apertures from the region surrounding the tissue treatment electrode.

27. An electrode unit as claimed in claim 26, further comprising an RF generator having a bipolar output connected to the tissue treatment electrode and the return electrode.

28. Electrosurgical apparatus comprising a radio frequency generator and an electrosurgical instrument for the treatment of tissue in the presence of an electrically-conductive fluid medium, the instrument comprising an instrument shaft, and an electrode assembly at one end of the shaft, the electrode assembly comprising a tissue treatment electrode and a return electrode which is electrically insulated from the tissue treatment electrode by means of an insulation member, the tissue treatment electrode having an exposed end for treating tissue, and the return electrode having a fluid contact surface which is spaced proximally from the tissue treatment electrode in such a manner as to define, in use, a conductive fluid path that completes an electrical circuit between the tissue treatment electrode and the return electrode, and the radio frequency generator having a bipolar output connected to the electrodes, wherein the electrode assembly is provided with a plurality of apertures in the region of the tissue treatment electrode and distal to the return electrode, through which apertures vapour bubbles and/or particulate material can be aspirated from the region surrounding the tissue treatment electrode.

29. Electrosurgical apparatus as claimed in claim 28, further comprising a pump for subjecting the distal end portion of the instrument shaft to a sub-atmospheric pressure thereby to aspirate, in use, vapour bubbles and/or particulate material through said apertures from the region surrounding the tissue treatment electrode.
