Grasping an object, e.g., a calculus in the biliary tract, using a grasper, such as a basket, that can be opened and closed without mechanical advantage and also rotated about the device axis to make it easier to capture the object. The basket can then be closed further with mechanical advantage to crush the object.

a mechanical actuator for moving a control element axially along a long axis and with mechanical advantage; and
a manual actuator for moving the control element axially along the axis, and for rotating the control element about the axis relative to said mechanical actuator wherein said mechanical actuator comprises a jam element for engaging the control element to apply axial force to the control element.

2. An operating assembly for a medical instrument, comprising:

a mechanical actuator for moving a control element axially along a long axis and with mechanical advantage; and
a manual actuator for moving the control element axially along the axis, and for rotating the control element about the axis relative to said mechanical actuator wherein said mechanical actuator comprises an engaging piece for bearing on the control element and applying axial force to withdraw the control element, said engaging piece including a deforming portion that deforms when a predetermined tension is applied to the control element to prevent excessive tension being placed on the control element.

3. A method for crushing an object in a body, the steps comprising:

providing a medical instrument comprising (a) a grasper actuateable between an open and a closed position, and (b) an operating assembly including (b1) a mechanical actuator for moving a control element axially along a long axis and with mechanical advantage, and (b2) a manual actuator for moving the control element axially along the axis and for rotating the control element about the axis relative to said mechanical actuator, said grasper attached to a distal end of the control element, said operating assembly capable of actuating said grasper between the open and closed positions either with or without mechanical advantage and of rotating said grasper about the axis relative to said mechanical actuator;
positioning said grasper in proximity to the object;
actuating said grasper with said operating assembly to open said grasper;
rotating said grasper using said operating assembly to orient said grasper in a desired orientation to allow capture of the object; and
actuating said grasper with said operating assembly to close said grasper with mechanical advantage to crush the captured object.
