A beam pattern is focused on a reflective film of the optical disk in an information signal region on which data is recorded. The optical disk is rotated to form the barcode.

while forming step b) to form said barcode, pulsed laser light from a light source is made into a rectangular beam pattern by using a rectangular mask and said rectangular pattern is intensity-modulated by a signal for making said barcode and focused on a reflective film in an information signal region in a prescribed radius portion of the optical disk. -
3. A method of forming a barcode on an optical disk according to claim 1, wherein
said optical disk is constructed from two disk-substrates laminated together. -
4. A method of forming a barcode on an optical disk according to claim 1, wherein
said barcode is made by removing the reflective film of said optical disk.
