1. An implantable device for monitoring a patient'"'"'s heart rate variability over time to develop an index of heart rate variability, comprising:
- a sensing electrode to detect a cardiac depolarization;
a sense amplifier coupled to the sensing electrode to produce a sigal indicative of a detected cardiac depolarization;
means for measuring intervals between detected cardiac depolarizations;
first timing means for defining successive monitoring periods;
second timing means for defining successive shorter time periods during each monitoring period;
a memory storing measured cardiac depolarization intervals occurring during a shorter time period;
means for calculating a median of the measured cardiac depolarization intervals occuring during a preceding shorter time period; and
means for calculating standard deviations of the calculated median measured cardiac depolarization intervals for a preceding monitoring period to produce an index of heart rate variability.

Abstract
An implantable monitoring device for monitoring a patient'"'"'s heart rate variability over time. The device includes a cardiac electrogram amplifier, a sensing electrode coupled to an input of the amplifier, timing circuitry, processing circutry and a memory. The timing circuitry defines successive monitoring periods each extending over a period of hours, the monitoring periods together extending at least over a period of weeks and also defines successive shorter time periods during each monitoring period. The memory stores heart intervals between depolarizations of the patient'"'"'s heart sensed by the amplifier during the shorter time periods. The processing circuitry calculates median intervals between depolarizations of the patient'"'"'s heart sensed by the amplifier during the shorter time periods and calculates standard deviations of the median intervals calculated during each monitoring period. The processing circuitry may also reject heart intervals occurring during tachyarrhythmias and calculate median intervals based only on heart intervals not rejected.

defining successive monitoring periods each extending over a first period of time;
defining successive shorter time periods during each monitoring period;
detecting cardiac depolarizations of the patient'"'"'s heart;
measuring intervals between detected cardiac depolarizations;
storing the measured cardiac depolarization intervals occurring during the shorter time periods;
calculated a median measured cardiac depolarization interval for a preceding shorter time period; and
calculating standard deviations of the median cardiac depolarization intervals calculated during a preceding monitoring period to produce an index of heart rate variability. - View Dependent Claims (6, 7, 8)
rejecting measured cardiac depolarization intervals occurring during tachyarrhythmias and wherein the calculating step comprises calculating median measured cardiac depolarization intervals based only on measured cardiac depolarization intervals not rejected.

A method according to claim 6 wherein the calculating step comprises determining a total duration of the non-rejected measured cardiac depolarization intervals during the shorter time periods and preventing calculation of a median measured cardiac depolariation interval if the total duration of non-rejected heart intervals during a shorter time period is less that a required duration.

8. A method according to claim 5 further comprising:

binning stored measured cardiac depolarization intervals to form a histogram having multiple bins with each bin being associated with a measured cardiac depolarization interval range, successively adding together the number of measured cardiac depolarization intervals stored in successive interval range bins until a sum is reached which exceeds one half of a total number of the measured cardiac depolarization intervals stored in the bins, and responsive to the sum exceeding one half of the total number of measured cardiac depolarization intervals in the bins, designating as the median measured cardiac depolarization interval the interval range of the bin having the number of intervals stored therein most recently added to the sum that causes the sum to exceed one half of the total number of measured cardiac depolarization interval.
