1. A method for stereotactic surgery on a body comprising:
- providing a surgical instrument;
providing a guidance fixture that includes (a) a mounting base having a central axis, (b) a controllable instrument drive for moving the surgical instrument along a constrained trajectory in response to a drive control signal, (c) an actuated adjustment mechanism coupled between the mounting base and the controllable instrument drive and (d) a sensor system coupled to the adjustment mechanism;
attaching the mounting base to the body;
accepting a three-dimensional image of the body;
determining a location of the mounting base in the three-dimensional image of the body; and
generating and transmitting sensor signals encoding an orientation of the constrained trajectory and encoding a position of the surgical instrument along the constrained trajectory.

Abstract
A method and apparatus for stereotactic surgery on a body, such as surgery on the brain or spine. A guidance fixture is used that includes a mounting base having a central axis, a controllable instrument drive for moving a surgical instrument along a constrained trajectory in response to a drive control signal, an actuated adjustment mechanism coupled between the mounting base and the controllable instrument drive, and a sensor system coupled to the adjustment mechanism. The mounting base of the guidance fixture is attached to the body and a location of the mounting base in a three-dimensional image of the body is determined, for example, using a remote sensing device, such as a camera, and tracking markers, such as LEDs, attached to the guidance fixture. Sensor signals encoding the orientation of the constrained trajectory and encoding a position of the surgical instrument along the constrained trajectory are generated and transmitted from the guidance fixture. An adjustment control signal is received and the orientation of the constrained trajectory relative to the mounting base is adjusted in response to the received adjustment control signal.

determining a location of the mounting base in the three-dimensional image of the body; and

receiving the sensor signals;
determining a location of the surgical instrument in the three-dimensional image using the sensor signals and the determined location of the mounting base in the three-dimensional image;
accepting a control input from an operator;
determining the adjustment control signal from the control input; and
transmitting the adjustment control signal.

5. The method of claim 4 further comprising:

generating a force feedback signal in response to adjustment of the guidance fixture; and
transmitting the force feedback signal to the operator.

6. The method of claim 4 wherein determining the adjustment control signal includes execution of an automated procedure for determining the adjustment control signal from the accepted control input and the determined location of the surgical instrument.

7. The method of claim 1 further comprising displaying a view of the three-dimensional image in conjunction with a representation of the determined location of the surgical instrument.

8. An apparatus for stereotactic surgery on a body comprising:

a guidance fixture including (a) a mounting base for attachment to the body, (b) a controllable instrument drive for moving a surgical instrument along a constrained trajectory in response to a drive control signal, (c) an actuated adjustment mechanism coupled between the mounting base and the controllable instrument drive for adjusting an orientation of the constrained trajectory relative to the mounting base in response to an adjustment control signal, and (d) a sensor system for providing signals encoding the orientation of the constrained trajectory and encoding a position of a surgical instrument driven along the constrained trajectory by the controllable instrument drive; and
a controller coupled to the guidance fixture for accepting signals provided by the sensor system, accepting a control input, computing drive control and adjustment control signals in response to the signals provided by the sensor system and the control input, and providing the drive control signal and the adjustment control signal to the guidance fixture. - View Dependent Claims (9, 10, 11)

12. A guidance fixture for stereotactic surgery on a body, comprising:

an instrument guide for moving a surgical instrument along a constrained trajectory relative to the instrument guide;
an adjustable base supporting the instrument guide, including (a) a mounting base for attachment to the body having a central axis and (b) an adjustment mechanism coupled between the mounting base and the instrument guide, wherein a configuration of the adjustment mechanism determines an orientation of the instrument guide relative to the central axis of the mounting base; and
