1. A method of treating ventricular fibrillation, comprising:
- a) monitoring a first cardiac signal across a left ventricular cardiac region to start a coupling interval time period once a contraction of cardiac tissue is detected in the left ventricular cardiac region using the first cardiac signal, and monitoring a second cardiac signal across a right ventricular cardiac region to obtain an up-slope portion of a coarse ventricular fibrillation complex as detected in the right ventricular cardiac region using the second cardiac signal; and
b) delivering a defibrillation shock during the occurrence of both the coupling interval time period and the up-slope portion of the coarse ventricular fibrillation complex.

Abstract
A method and system for ventricular defibrillation by coordinating the delivery of defibrillation shocks with sensed ventricular fibrillation complexes in a way which improves the probability of success of the defibrillation shock. Ventricular electrical activity is monitored in two ventricular locations during ventricular fibrillation to detect coarse ventricular fibrillation complexes and contractions of the ventricular cardiac tissue. The defibrillation shock is delivered in coordination with the occurrence of coarse ventricular fibrillation complexes and the contractions of ventricular cardiac tissue, and specifically to occur on the up-slope portion thereof, for optimal probability of success.

1. A method of treating ventricular fibrillation, comprising:

a) monitoring a first cardiac signal across a left ventricular cardiac region to start a coupling interval time period once a contraction of cardiac tissue is detected in the left ventricular cardiac region using the first cardiac signal, and monitoring a second cardiac signal across a right ventricular cardiac region to obtain an up-slope portion of a coarse ventricular fibrillation complex as detected in the right ventricular cardiac region using the second cardiac signal; and
b) delivering a defibrillation shock during the occurrence of both the coupling interval time period and the up-slope portion of the coarse ventricular fibrillation complex. - View Dependent Claims (2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16)
providing a predetermined threshold value;
providing a coarse complex threshold value;
obtaining a coarse ventricular fibrillation complex having an amplitude; and
wherein delivering a defibrillation shock occurs during the coupling interval time period started once the first cardiac signal exceeds the predetermined threshold value and occurs when the coarse ventricular fibrillation complex amplitude is greater than the coarse complex threshold value.

The method of claim 7, further including counting coarse ventricular fibrillation complexes detected in the second cardiac signal and coordinating the delivery of the defibrillation shock with the up-slope portion of an nth counted coarse ventricular fibrillation complex detected in the second cardiac signal, wherein the nth counted coarse ventricular fibrillation complex is a number greater than or equal to 2 and less than or equal to about 9.

The method of claim 1, wherein monitoring comprises monitoring a cardiac electrogram signal for the first cardiac signal from a first lead implanted into a heart, and monitoring a cardiac electrogram signal for the second cardiac signal across a first and a second defibrillation coil electrode of a second lead implanted into at least one chamber of the heart, and wherein delivering a defibrillation shock includes applying a pulse of electrical energy between the first and the second defibrillation coil electrodes.

10. The method of claim 1, further including:

providing a predetermined threhold value; and
starting the coupling interval time period once the first cardiac signal exceeds the predetermined threshold value.

11. The method of claim 10, wherein the providing the predetermined threshold value includes programming the predetermined threshold value in the range of 0.1 to 10 millivolts.

12. The method of claim 1, further comprising programming the coupling interval time period in the range of 0 to 200 milliseconds.

13. The method of claim 1, further including delivering pacing level pulses of electrical energy at the left ventricular cardiac region of the heart.

14. The method of claim 13, wherein the pacing level pulses of electrical energy provide normal sinus pacing to the heart.

15. The method of claim 13, wherein the pacing level pulses of electrical energy provide overdrive pacing to the heart.

The method of claim 1, further including delivering at least one asynchronous defibrillation shock if the ventricular fibrillation is not terminated by the delivering a defibrillation shock during the occurrence of both the coupling interval time period and the up-slope portion of the coarse ventricular fibrillation complex.
