- an implantable hemodynamic monitor capable of measuring physiologic patient data and immediately and continuously uploading the patient data on an essentially real-time basis in response to a data request signal received by the implantable hemodynamic monitor;
a network means for immediately conveying the patient data to a remote database; and
at least two independent, dynamically reconfigurable network portals coupled to the remote database each having a customizable graphical user interface for displaying at least a portion of a one of the following;

Continuous remote monitoring of patients based on data obtained from an implantable hemodynamic monitor provides an interactive patient management system. Using network systems, patients are remotely monitored to continuously diagnose and treat heart-failure conditions. A screen displayable summary provides continuous feedback and information to physicians, patients and authorized third parties. The quick look summary includes various sites and presentation tailored to match the patients'"'"' and physicians'"'"' needs. An interactive system according to claim 2, wherein the pulmonary systolic pressure value, the pulmonary diastolic pressure value and the right atrial pressure value are derived from measurements obtained by a single absolute pressure sensor implanted in the right ventricle of the patient and coupled to the implantable hemodynamic monitor.

4. An interactive system according to claim 2, further comprising a secure database and an encryption system interconnected to the remote database.

5. An interactive system according to claim 4, further comprising a network search engine coupled to at least the remote database and the secure database.

An interactive system according to claim 5, wherein the patient data and other network content is divided according to a security rule or a privacy rule into at least three zones, and wherein said at least three zones include a public zone, a private zone and a confidential zone, and the at least three portals include a predefined user access authorization for said at least three zones.

7. An interactive system according to claim 6, wherein a one of the at least three portals is a physician portal and said physician portal has a highest level of predefined user access authorization.

8. An interactive system according to claim 7, further including a physician portal quick look greeting page, wherein said quick look greeting page displays at least a one of the following:
a physician identifier code or name, a number of new patient data records received by the remote database, a number of identified abnormal events contained in the number of new patient data records, a number of patients who require manual review by the physician, a tachyarrhythmia-triggered status, a bradycardia-triggered status, a patient-triggered status.

9. An interactive system according to claim 8, further including a quick look patient status page of content having at least a one of the following daily minimum values:
a heart rate, a right ventricle systolic pressure, a right ventricle diastolic pressure, an estimated pulmonary artery pressure, a right ventricle pulse pressure, a right ventricle differential pressure.

10. An interactive system according to claim 9, further including a quick look patient status page of content having at least a one of the following out-of-range values, as determined with reference to a set of predetermined in-range values:
a heart rate, a right ventricle systolic pressure, a right ventricle diastolic pressure, an estimated pulmonary artery pressure, a right ventricle pulse pressure, a right ventricle differential pressure, an ePAD pressure.

11. An interactive system according to claim 8 further including at least one content field for displaying:
an appointment schedule for the patient, an accounts payable summary for the patient, an accounts payable summary for the physician, a clinical study data set, a language preference field for the patient, a language preference for the physician, a trend summary for the patient, a graphical plot of the trend summary, a clinical indication for the patient, an indication whether or not the essentially real-time patient data represents a predefined repeatable data routine.

12. An interactive system according to claim 9 wherein at least one of the at least three portals displays a logo, tradename or trademark owned by Medtronic, Inc.

13. An interactive system according to claim 12, further including a data comparison engine means for producing a graphical display comparing the essentially real-time patient data and a prior patient data.

14. An interactive system according to claim 13, wherein the graphical display is based on either a percent change or an absolute difference between the essentially real-time patient data and the prior patient data.

An interactive system according to claim 14, further including a data excursion alert information message delivered for display to a one of the at least three portals, wherein said data excursion alert information message is delivered for display in response to a change in a measured value which exceeds an adjustable threshold value and wherein said measured value is a one of:
a heart rate, a right ventricle systolic pressure, a right ventricle diastolic pressure, an estimated pulmonary artery pressure, a right ventricle pulse pressure, a right ventricle differential pressure, an ePAD pressure.

An interactive system according to claim 4, wherein the implantable hemodynamic monitor ceases continuously uploading the patient data on an essentially real-time basis after a predetermined period of time following receipt of the data request signal received by the implantable hemodynamic monitor.

An interactive system according to claim 16, wherein the implantable hemodynamic monitor is capable of bi-directional communication and further including a programming protocol engine that in response to an accepted downloaded device programming code from the remote network the programming protocol engine changes an operational parameter of the hemodynamic monitor.

18. An interactive system according to claim 8, wherein at least one of the out-of-range values which is delivered for display is adjustably highlighted on a monitor, and wherein said values are highlighted using a one of the following:
a unique color, a unique font, an italicized font, a relatively larger font.

19. An interactive system according to claim 8, further including an out-of-network messaging utility, wherein said messaging utility provides a message related to each of the out-of-range values to at least a one of:
an e-mail address of the physician, an e-mail address of a designated care provider, an e-mail address of the patient, a fax message to a remote fax machine accessible to the physician, a fax message to the patient, a fax message to the designated care provider, a pager message to the physician, a pager message to the designated care provider, a pager message to the patient.
