Abstract

A method and apparatus are disclosed of utilizing a source of arterial and/or arteriolar pulse waveform data from a patient for the purpose of measuring pulsus paradoxus. The arterial pulse waveform data source described is a pulse oximeter plethysmograph but can be any similar waveform data source, including intra-arterial transducer, blood pressure transducer, or plethysmograph. Through incorporation of the measurements of values, such as the area under the pulse waveform curve, that are time-domain functions of a change in height of the pulse waveform over at least a partial duration of the waveform, embodiments of the present invention represent a significant improvement upon previously described methods of measuring pulsus paradoxus and, further, produce improved accuracy in measurement of the multiple contributing variables generating pulsus paradoxus, in particular the contribution of diastolic events, to the extent that the physical signs so measured can be regarded as “Revised Pulsus Paradoxus.”

Claims

1. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

determining a first AUC from at least one component of the first pulse waveform;

determining a second AUC from at least one component of the second pulse waveform;

comparing the first AUC and the second AUC to measure pulsus paradoxus.

2. The method of claim 1, further comprising determining the presence or absence of pulsus paradoxus.

3. The method of claim 2, further comprising correlating a magnitude of the difference between the first AUC and second AUC with a magnitude of pulsus paradoxus.

4. The method of claim 1, wherein the comparing step is performed by a computer.

5. The method of claim 1, wherein the data indicative of pulsatile cardiovascular behavior is obtained from plethysmography.

6. The method of claim 5, wherein the data indicative of pulsatile cardiovascular behavior is obtained from pulse oximetry.

7. The method of claim 6, wherein the data indicative of pulsatile cardiovascular behavior is obtained from reflective pulse oximetry.

8. The method of claim 6, wherein the data indicative of pulsatile cardiovascular behavior is obtained from transmissive pulse oximetry.

9. The method of claim 1, wherein the at least one component of the first pulse waveform comprises a systolic portion of the first pulse waveform, and the at least one component of the second pulse waveform comprises a systolic portion of the second pulse waveform.

10. The method of claim 9, wherein the at least one component of the first pulse waveform further comprises a diastolic portion of the first pulse waveform, and the at least one component of the second pulse waveform further comprises a diastolic portion of the second pulse waveform.

11. The method of claim 1, wherein the comparing step comprises determining a difference between the first AUC and second AUC.

12. The method of claim 11, wherein an absolute value of the difference between the first AUC and second AUC is maximal for any two of a set of pulse waveforms obtained over a duration of at least one respiratory cycle.

13. The method of claim 11, further comprising providing a determination of the presence or absence of pulsus paradoxus in the mammal, the determination based upon whether a magnitude of the difference between the first AUC and second AUC is greater than a threshold value.

14. The method of claim 13, wherein pulsus paradoxus is determined to exist when the magnitude of the difference between the first AUC and second AUC is greater than 10 percent.

15. The method of claim 14, wherein pulsus paradoxus is determined to exist when the magnitude of the difference between the first AUC and second AUC is greater than 20 percent.

16. The method of claim 15, wherein pulsus paradoxus is determined to exist when the magnitude of the difference between the first AUC and second AUC is greater than 50 percent.

17. The method of claim 12, further comprising calculating an average for the difference in AUC.

18. The method of claim 17, further comprising calculating a standard deviation from the average difference in AUC.

19. The method of claim 18, further comprising calculating a variance from the standard deviation of the difference in AUC.

20. The method of claim 12, further comprising calculating a weighted signal average for the difference in AUC.

21. The method of claim 1, further comprising identifying and suppressing artifacts present within the data indicative of pulsatile cardiovascular behavior.

22. A system for measuring pulsus paradoxus in a mammal, the system comprising:

means for obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

means for determining a first AUC from at least one component of the first pulse waveform, and for determining a second AUC from at least one component of the second pulse waveform;

means for comparing the first AUC and the second AUC to measure pulsus paradoxus.

23. The system of claim 22, wherein the means for obtaining data indicative of pulsatile cardiovascular behavior comprises a plethysmograph.

24. The system of claim 23, wherein the plethysmograph comprises a reflective plethysmograph.

25. The system of claim 23, wherein the plethysmograph comprises a transmissive plethysmograph.

26. The system of claim 23, wherein the means for obtaining data indicative of pulsatile cardiovascular behavior comprises a pulse oximeter.

27. A system for measuring pulsus paradoxus in a mammal, the system comprising:

a pulse oximeter that obtains data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

a measurer that measures a first AUC from at least one component of the first pulse waveform, and measures a second AUC from at least one component of the second pulse waveform;

an analyzer that compares the first AUC and the second AUC to measure pulsus paradoxus.

28. The system of claim 27, wherein a computer comprises the analyzer.

29. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

determining a first value that is a time-domain function of a change in height of the first pulse waveform over at least a partial duration of the first pulse waveform;

determining a second value that is a time-domain function of a change in height of the second pulse waveform over at least a partial duration of the second pulse waveform; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are slopes.

30. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

determining a first value that is a time-domain function of a change in height of the first pulse waveform over at least a partial duration of the first pulse waveform;

determining a second value that is a time-domain function of a change in height of the second pulse waveform over at least a partial duration of the second pulse waveform; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are average slopes.

31. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

determining a first value that is a time-domain function of a change in height of the first pulse waveform over at least a partial duration of the first pulse waveform;

determining a second value that is a time-domain function of a change in height of the second pulse waveform over at least a partial duration of the second pulse waveform; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are maximal slopes.

32. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising at least a first and a second pulse waveform;

determining a first value that is a time-domain function of a change in height of the first pulse waveform over at least a partial duration of the first pulse waveform;

determining a second value that is a time-domain function of a change in height of the second pulse waveform over at least a partial duration of the second pulse waveform; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first value is a first AUC and the second value is a second AUC.

33. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising a first plethysmographic or pressure amplitude obtained at a first time interval after an onset of a first pulse, and a second plethysmographic or pressure amplitude obtained at a second time interval after the onset of the first pulse;

determining a first value that is a time-domain function of a difference between the first and second plethysmographic or pressure amplitudes;

obtaining a third plethysmographic or pressure amplitude obtained at the first time interval after an onset of a second pulse, and a fourth plethysmographic or pressure amplitude obtained at the second time interval after the onset of the second pulse;

determining a second value that is a time-domain function of a difference between the third and fourth plethysmographic or pressure amplitudes; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are slopes.

34. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising a first plethysmographic or pressure amplitude obtained at a first time interval after an onset of a first pulse, and a second plethysmographic or pressure amplitude obtained at a second time interval after the onset of the first pulse;

determining a first value that is a time-domain function of a difference between the first and second plethysmographic or pressure amplitudes;

obtaining a third plethysmographic or pressure amplitude obtained at the first time interval after an onset of a second pulse, and a fourth plethysmographic or pressure amplitude obtained at the second time interval after the onset of the second pulse;

determining a second value that is a time-domain function of a difference between the third and fourth plethysmographic or pressure amplitudes; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are average slopes.

35. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising a first plethysmographic or pressure amplitude obtained at a first time interval after an onset of a first pulse, and a second plethysmographic or pressure amplitude obtained at a second time interval after the onset of the first pulse;

determining a first value that is a time-domain function of a difference between the first and second plethysmographic or pressure amplitudes;

obtaining a third plethysmographic or pressure amplitude obtained at the first time interval after an onset of a second pulse, and a fourth plethysmographic or pressure amplitude obtained at the second time interval after the onset of the second pulse;

determining a second value that is a time-domain function of a difference between the third and fourth plethysmographic or pressure amplitudes; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are maximal slopes.

36. A method for measuring pulsus paradoxus in a mammal, the method comprising:

obtaining data indicative of pulsatile cardiovascular behavior from the mammal, the data comprising a first plethysmographic or pressure amplitude obtained at a first time interval after an onset of a first pulse, and a second plethysmographic or pressure amplitude obtained at a second time interval after the onset of the first pulse;

determining a first value that is a time-domain function of a difference between the first and second plethysmographic or pressure amplitudes;

obtaining a third plethysmographic or pressure amplitude obtained at the first time interval after an onset of a second pulse, and a fourth plethysmographic or pressure amplitude obtained at the second time interval after the onset of the second pulse;

determining a second value that is a time-domain function of a difference between the third and fourth plethysmographic or pressure amplitudes; and

comparing the first value and the second value to measure pulsus paradoxus;

wherein the first and second values are slopes;

wherein the first value is a first AUC and the second value is a second AUC.
