Abstract

A resuscitation device for automatic compression of a victim'"'"'s chest using a compression belt which exerts force evenly over the entire thoracic cavity. The belt is constricted and relaxed through a motorized spool assembly that repeatedly tightens the belt and relaxes the belt to provide repeated and rapid chest compression.

Claims

1. A device for compressing the chest of a patient, said device comprising:

a belt which is adapted to extend at least partially around the chest of the patient;

a rotating member operatively connected to the belt such that rotation of the rotating member spools the belt upon the rotating member to constrict the belt about the chest of the patient; and

a motor for rotating the rotating member;

said rotating member being of a diameter chosen so as to require rotation exceeding one full rotation of the rotating member to effect resuscitative compression of the chest after any rotations necessary to effect take-up of slack in the belt.

2. The device of claim 1 wherein the rotating member has a diameter in the range of 0.5 cm to 2.5 cm.

3. The device of claim 1 further comprising:

a drive mechanism operably connected to the belt so that operation of the drive mechanism by the motor causes the belt to tighten around the chest of the patient;

a clutch operably connected to the motor and to the drive mechanism, wherein said clutch may be operated to connect the motor to the drive mechanism while the device is in operation; and

a control system operably connected to the clutch and capable of causing the clutch to disconnect the motor from the drive mechanism while the device is in operation.

4. The device of claim 1 further comprising:

a panel adapted for location near the patient'"'"'s chest, wherein drive spool is mounted on the panel;

a spindle mounted on the panel substantially parallel to the drive spool, said spindle engaging the belt to reverse travel of the belt from transverse direction relative to the chest and toward the drive spool to the opposite transverse direction relative to the chest away from the drive spool.

5. The device of claim 4 wherein the panel comprises a left panel and a right panel, and the right panel and left panel are hinged together so as to allow them to fold together and unfold to a substantially flat configuration.

6. The device of claim 5 wherein the left panel and the right panel are hinged together near the location of the spindle, thereby permitting substantially complete enclosure of the chest by the belt when the left and right panels are not completely unfolded to a substantially flat configuration.

7. The device of claim 1 further comprising:

a brake operably connected to the rotating member and capable of holding the belt rotating member such that the belt is held in a tightened state about the chest of the patient;

a clutch between the motor and the rotating member, said clutch capable of connecting and disconnecting the motor from the rotating member, said clutch being capable of engagement and disengagement while the motor is in operation; and

a controller for controlling operation of the motor, brake and clutch, said controller programmed to operate the motor, brake and clutch to cause repeated cycles of tightening of the belt to set threshold of tightness, momentarily hold the belt at this threshold of tightness, and release of the belt.

8. The device of claim 1 further comprising:

a clutch operably connected to the rotating member and to the motor, said clutch operable while the device is operating to connect and disconnect the motor to the rotating member;

wherein the rotating member is capable of drawing a portion of the belt to one side of the thorax, thereby to tighten the belt to produce intra-thoracic pressures sufficient to propel the blood of the patient into the extrathoracic regions, the rotating member being operable at a cardiac resuscitative rate to alternatively tighten and then loosen the belt to permit the elastic tissues of the body of the patient to return blood to the intrathoracic region.

9. The device of claim 1 further comprising:

a brake operably connected to the&

rotating member and capable of holding the rotating member in a tightened state about the chest of the patient;

a controller for controlling the operation of the motor and brake, said controller programmed to operate the motor and brake to cause repeated cycles of tightening of the belt to a set threshold of tightness, momentarily hold the belt at this threshold of tightness, and release of the belt.

10. The device of claim 1 wherein the rotating member comprises a drive spool.

11. The device of claim 1 further comprising:

a brake operably connected to the rotating member and capable of holding the rotating member in a tightened state about the chest of the patient;

a controller for controlling operation of the motor and brake, said controller programmed to operate the motor and brake to rotate the rotating member to cause repeated cycles of tightening of the belt and prevent the rotating member from rotating at selected points in the repeated cycles.

12. The device of claim 1 further comprising:

a current sensor operably connected to the motor, said current sensor adapted to sense current drawn by the motor and transmit a corresponding current signal to a controller;

wherein said controller is programmed to operate the motor and the rotating member to take up slack in the belt after initial placement of the belt on the chest of the patient by rotating the motor in a tightening rotation until the current signal provided by the current sensor rapidly increases;

wherein said controller is further programmed to operate the motor and rotating member to cause repeated cycles of tightening the belt about the chest of the patient and loosening of the belt about the chest of the patient.

13. The device of claim 12 further comprising a drive train operably connected to the rotating member and to the motor.

14. The device of claim 12 further comprising:

an encoder scale on the belt and an encoder scanner located in apposition to the scale, said encoder transmitting an encoder position to the controller;

said controller further programmed to associate the encoder position to the point of rapid increase in the current signal during initial placement thereby defining a slack-limit position of the belt, and to operate the motor to limit loosening of the belt during cycles to the slack-limit position.

15. The device of claim 14 wherein the encoder is a linear encoder.

16. The device of claim 14 wherein the encoder is an angular encoder.

17. The device of claim 14 wherein the encoder is a non-linear encoder.

18. A method of performing chest compressions on a patient, said method comprising the steps of:

providing a device for compressing the chest of a patient, said device comprising;

a belt which is adapted to extend at least partially around the chest of the patient;

a rotating member operatively connected to the belt such that rotation of the rotating member spools the belt upon the rotating member to constrict the belt about the chest of the patient;

a motor for rotating the rotating member;

said rotating member being of a diameter chosen so as to require rotation exceeding one full rotation of the rotating member to effect resuscitative compression of the chest after any rotations necessary to effect take-up of slack in the belt; and

performing chest compressions.

19. The method of claim 18 comprising the further step of operating the rotating member to perform a compression step to tighten the belt in order to compress the chest to a first state of compression, then perform a first holding step to hold the belt tight momentarily, then perform a release step to release the belt to allow the belt to loosen and allow the chest to relax.

20. The method of claim 18 comprising the further step of:

after releasing the belt to allow the belt to loosen and allow the chest to relax, performing a second holding step to hold the belt taut before the chest has fully relaxed to obtain a second state of compression.

21. The method of claim 20 comprising the further steps of:

after releasing the belt to allow the belt to loosen and allow the chest to relax, performing a second holding step to hold the belt taut before the chest has fully relaxed to obtain a second state of compression;

repeatedly performing compression cycles comprising the compression step, the first holding step, the releasing step and second holding step.

22. The method of claim 20 comprising the further steps of:

performing compressions in sets of compression cycles, said sets comprising a plurality of compression cycles;

between sets of compression cycles, releasing the belt completely to allow the chest to completely relax.

23. The method of claim 18 comprising the further step of:

operating the rotating member to perform several cycles of compressions including the steps of tightening the belt in order to compress the chest to a high state of compression, then holding the belt tight momentarily to maintain the high state of compression, then releasing the belt to allow the belt to loosen and allow the chest to relax, and holding the belt taut to maintain a low state of compression; and

after several compression cycles, releasing the belt to allow substantially complete relaxation of the chest without compression.

24. The method of claim 18 comprising the further steps of:

providing a first means for sensing the tightness of the belt around the chest of the patient based on a first sensed parameter;

providing a second means for sensing the tightness of the belt around the chest of the patient based on a second sensed parameter; and

providing a control system for operating the rotating member to tighten the belt in order to compress the chest, and programming said control system to limit the tightness of the belt based on either of the first sensed parameter or the second sensed parameter.

25. The method of claim 18 comprising the further steps of:

passing the belt around the thorax of the patient so that the belt is adapted, when tightened, to conform substantially to the contour of the thorax around a major portion of its periphery;

tightening the belt so that the force components produce intrathoracic pressures sufficient to propel the blood of the patient into the extrathoracic regions;

loosening the belt so as to allow the elastic tissues of the body of the patient to return blood to the intrathoracic regions;

continuing to alternately tighten and loosen the belt in a resuscitative rhythm; and

accomplishing the tightening and loosening steps by providing a clutch operatively connected to the motor and the rotating member and operating the clutch while the device is operating to connect the motor and the rotating member;

is operating to connect the motor and the rotating member.

26. The method of claim 18 comprising the further steps of:

passing the belt around the thorax of the patient so that the belt is adapted, when tightened, to conform substantially to the contour of the thorax around a major portion of its periphery;

tightening the belt so that the force components produce intrathoracic pressures sufficient to propel the blood of the patient into the extrathoracic regions;

loosening the belt so as to allow the elastic tissues of the body of the patient to return blood to the intrathoracic regions;

continuing to alternately tighten and loosen the belt in a resuscitative rhythm; and

accomplishing the tightening and loosening steps by providing a clutch operatively connected to the motor and the rotating member and operating the clutch while the device is operating to connect and disconnect the motor and the rotating member.

27. The method of claim 18 comprising the further steps of:

operating the rotating member to tighten the belt in order to compress the chest to a first state of compression, then releasing the belt to allow the belt to loosen and allow the chest to relax; and

sensing the state of compression of the chest, and braking the movement of the belt before the belt is completely slack about the chest of the patient.

28. A method of treating patients, said method comprising the steps of:

providing a blood flow increasing apparatus, said apparatus comprising;

a belt which is adapted to extend at least partially around the chest of the patient;

a rotating member operatively connected to the belt such that rotation of the rotating member spools the belt upon the rotating member to constrict the belt about the chest of the patient;

a motor for rotating the rotating member;

a bladder disposable between the belt and the patient, said bladder being filled with a pressure transmitting medium;

said rotating member being of a diameter chosen so as to require rotation exceeding one full rotation of the rotating member to effect resuscitative compression of the chest after any rotations necessary to effect take-up of slack in the belt;

a clutch operably connected to the motor and to the rotating member;

seating the blood flow increasing apparatus on the chest of a patient;

wrapping the belt around the chest of the patient;

fastening first and second opposite extremities of the belt to the apparatus;

operating the apparatus to apply a chest compressing force to the patient; and

operating the clutch while the device is operating to connect the motor to the rotating member.

29. The method of claim 28 comprising the further step of operating the clutch while the device is in operation to disconnect the motor from the clutch.

30. The method of claim 29 comprising the further step of providing a controller which controls operation of the motor and the clutch, said controller being programmed to operate the motor and the clutch to cause the rotating member to repetitively tighten and loosen the belt.
