Abstract

A method and apparatus that provides an expert system for determining respiratory phase during ventilatory support of a subject. Discrete phase states are partitioned and prior probability functions and observed probability functions for each state are defined. The probability functions are based upon relative duration of each state as well as the flow characteristics of each state. These functions are combined to determine phase probabilities for each state using Bayes'"'"' theorem. The calculated probabilities for the states may then be compared to determine which state the subject is experiencing. A ventilator may then conform respiratory support in accordance with the most probable phase. To provide a learning feature, the probability functions may be adjusted during use to provide a more subject specific response that accounts for changing respiratory characteristics.

Claims

1. A method for determining a subject'"'"'s respiratory phase during ventilatory support comprising the steps of:

deriving airflow-related measures from a subject during successive respiratory cycles;

using said airflow-related measures to calculate probabilities of a plurality of phase states; and

determining a phase in the subject'"'"'s respiratory cycle from the calculated probabilities of said phase states.

2. The method of claim 1 wherein said calculating step includes the sub-steps of:

calculating prior probabilities of said phase states; and

calculating observed probabilities of said phase states from said airflow-related measures.

3. The method of claim 2 wherein said prior probabilities are functions of phase state durations relative to respiratory cycle duration.

4. The method of claim 3 wherein said prior probabilities are functions of previously calculated phase-state probabilities.

5. The method of claim 3 wherein said observed probabilities are based on relationships between said phase states and sub-ranges selected from a range of airflow-related measures.

6. The method of claim 5 wherein said relationships change as a result of changing airflow cycles of the subject.

7. The method of claim 3 wherein in said determining step said calculated phase-state probabilities are compared with each other to identify a calculated phase-state probability that is more probable than other calculated phase-state probabilities.

8. The method of claim 3 wherein said phase states are characterized by partitioning a respiration cycle in relation to a peak flow.

9. The method of claim 3 wherein said phase states are repeatedly adjusted to account for the subject'"'"'s changing respiratory characteristics as assessed by said airflow-related measures over time.

10. The method of claim 3 wherein said step of calculating phase-state probabilities involves an evaluation of prior probabilities and observed probabilities as a function of Bayes'"'"' theorem.

11. The method of claim 3 further comprising the step of adjusting ventilatory support based upon results from said phase determining step.

12. The method of claim 3 wherein said step of calculating phase-state probabilities includes adjusting phase-state probabilities based upon state successions.

13. The method of claim 3 wherein said phase determining step includes the sub-step of calculating a continuous phase variable from said phase-state probabilities.

14. The method of claim 1 wherein said step of deriving airflow-related measures includes the sub-step of smoothing airflow measures by low pass filtering.

15. An apparatus for determining a subject'"'"'s respiratory phase during ventilatory support provided by a ventilator that furnishes a controlled supply of breathable gas to the subject'"'"'s airway comprising:

a transducer for generating an airflow signal representative of respiratory airflow in the subject; and

a processor with programmed instructions for controlling the steps of;

deriving airflow-related measures from a subject during successive respiratory cycles;

using said airflow-related measures to calculate probabilities of a plurality of phase states; and

determining a phase in the subject'"'"'s respiratory cycle from the calculated probabilities of said phase states.

16. The apparatus of claim 15 wherein said calculating step includes the sub-steps of:

calculating prior probabilities of said phase states; and

calculating observed probabilities of said phase states given said airflow-related measures.

17. The apparatus of claim 16 wherein said prior probabilities are functions of phase state durations relative to respiratory cycle duration.

18. The apparatus of claim 17 wherein said prior probabilities are functions of previously calculated phase-state probabilities.

19. The apparatus of claim 17 wherein said observed probabilities are based on relationships between said phase states and sub-ranges selected from a range of airflow-related measures.

20. The apparatus of claim 19 wherein said relationships change as a result of changing airflow cycles of the subject.

21. The apparatus of claim 17 wherein in said determining step said calculated phase-state probabilities are compared with each other to identify a calculated phase-state probability that is more probable than other calculated phase-state probabilities.

22. The apparatus of claim 17 wherein said phase states are characterized by partitioning a respiration cycle in relation to a peak flow.

23. The apparatus of claim 17 wherein said phase states are repeatedly adjusted to account for the subject'"'"'s changing respiratory characteristics as assessed by said airflow-related measures over time.

24. The apparatus of claim 17 wherein said step of calculating phase-state probabilities involves an evaluation of prior probabilities and observed probabilities as a function of Bayes'"'"' theorem.

25. The apparatus of claim 17 wherein said processor further has programmed instructions for controlling the step of adjusting ventilatory support based upon results from said phase determining step.

26. The method of claim 17 wherein said step of calculating phase-state probabilities includes adjusting phase-state probabilities based upon state successions.

27. The method of claim 17 wherein said phase determining step includes the sub-step of calculating a continuous phase variable from said phase-state probabilities.

28. The method of claim 15 wherein said step of deriving airflow-related measures includes the sub-step of smoothing airflow measures by low pass filtering.
