Abstract

A pulse generator housing for enclosing and containing pulse generator defibrillation circuitry. The housing is formed entirely of electrically conductive metal defining an electrically conductive outer surface which is connected to the pulse generator circuitry for delivering defibrillating energy to the heart. The pulse generator housing is implanted in the pectoral region proximate the heart with the conductive surface facing the heart. Regions of the conductive outer surface may be electrically isolated and dedicated for separately sensing and shocking. The outer surface may be coated with platinum. Additional coiled segment electrodes may extend from the housing and be electrically connected to the conductive outer surface so as to increase the effective conductive surface area. A sensor is provided to determine whether the housing is inside or outside a body of a patient to disconnect the pulse generator housing conductive surface from the pulse generator circuitry when the unit is outside the body of a patient.

Claims

1. An apparatus, comprising:

first, second, and third electrodes for disposition near the heart;

an implantable pulse generator including pulse generator circuitry for delivering a defibrillation shock pulse to the heart;

a conductive pulse generator housing enclosing the pulse generator circuitry;

a programmable switch for connecting the first, second, and third electrodes and the conductive housing to the pulse generator circuitry, wherein the switch is configurable so that a shock pulse is either discharged between the first electrode and the second electrode, discharged between the first electrode and the second electrode connected in common with the conductive housing, or discharged from the first electrode connected in common with the third electrode to the second electrode connected in common with the conductive housing.

2. The apparatus of claim 1 wherein the second electrode is a subcutaneous electrode.

3. The apparatus of claim 2 wherein the subcutaneous electrode is a conductive mesh connected to the pulse generator housing.

4. The apparatus of claim 2 wherein the subcutaneous electrode is a helical coil which encircles the pulse generator housing.

5. The apparatus of claim 2 wherein the subcutaneous electrode is a plurality of coiled segment electrodes.

6. The apparatus of claim 1 wherein the pulse generator housing includes one or more conductive regions isolated by insulating material.

7. An apparatus, comprising:

first and second electrodes for disposition near the heart, wherein the second electrode is a subcutaneous electrode comprising a plurality of coiled segment electrodes;

an implantable pulse generator including pulse generator circuitry for delivering a defibrillation shock pulse to the heart;

a conductive pulse generator housing enclosing the pulse generator circuitry;

a programmable switch for connecting the first and second electrodes and the conductive housing to the pulse generator circuitry, wherein the switch is configurable so that a shock pulse is either discharged between the first electrode and the conductive housing or discharged between the second electrode and the conductive housing.

8. The apparatus of claim 7 wherein the first and second electrodes are proximal and distal electrodes incorporated into a single lead.

9. The apparatus of claim 7 further comprising a third electrode and wherein the switch is configurable so that a shock pulse is discharged from the first electrode connected in common with the third electrode to the second electrode connected in common with the conductive housing.

10. The apparatus of claim 7 wherein the pulse generator housing includes one or more conductive regions isolated by insulating material.

11. A method comprising:

sensing cardiac activity in order to detect an arrhythmia; and

,

upon detection of an arrhythmia, discharging a defibrillation shock pulse between a first ventricular electrode and a second electrode disposed in the superior vena cava and connected in common with a conductive pulse generator housing.

12. The method of claim 11 further comprising configuring a programmable switch so that the defibrillation shock pulse is delivered between the first ventricular electrode and the second electrode disposed in the superior vena cava and connected in common with a conductive pulse generator housing.

13. The method of claim 11 further comprising delivering a lower energy defibrillation shock and, if the arrhythmia is not terminated, delivering a higher energy defibrillation shock.

14. A method comprising:

sensing cardiac activity in order to detect an arrhythmia; and

,

upon detection of an arrhythmia, discharging a defibrillation shock pulse between a first ventricular electrode and a second electrode disposed subcutaneously and connected in common with a conductive pulse generator housing.

15. The method of claim 14 wherein the defibrillation shock pulse is discharged between the first ventricular electrode connected in common with a third electrode disposed in the superior vena cava and the second electrode connected in common with the housing.

16. The method of claim 15 further comprising configuring a programmable switch so that the defibrillation shock pulse is delivered between the first ventricular electrode connected in common with a third electrode disposed in the superior vena cava and the second electrode connected in common with the housing.

17. The method of claim 14 further comprising delivering a lower energy defibrillation shock and, if the arrhythmia is not terminated, delivering a higher energy defibrillation shock.

18. The method of claim 14 further comprising configuring a programmable switch so that the defibrillation shock pulse is delivered between the first ventricular electrode and the second electrode disposed subcutaneously and connected in common with a conductive pulse generator housing.
