Abstract

The present invention provides a method and system for characterizing one beat of a patient'"'"'s supraventricular rhythm. A plurality of templates is provided and updated using a plurality of qualified beats. Updating occurs by temporally aligning the shock channel waveforms of the template beats using rate channel fiducial points. The template beats are combined by point-by-point addition of the shock channel waveforms. The resultant updated template characterizes one of the patient'"'"'s supraventricular conducted cardiac beats.

Claims

1. A method of characterizing a patient'"'"'s supraventricular rhythm, comprising:

providing a plurality of templates;

detecting, using an implantable lead system, rate channel signals and shock channel signals of a plurality of beats;

determining if the beat signals meet predetermined criteria;

in a body implantable system, selectively updating the plurality of templates, comprising;

selecting a template of the plurality of templates for updating based on a similarity between the template and a beat of the plurality of beats; and

updating the selected template using the beat; and

characterizing, in the body implantable system, the patient'"'"'s supraventricular rhythm by identifying a particular template of the plurality of selectively updated templates as representative of the patient'"'"'s supraventricular rhythm.

2. The method of claim 1, wherein providing the plurality of templates further comprises retrieving at least a first template from memory and forming one or more other templates using the beats.

3. The method of claim 1, wherein selecting the template of the plurality of templates for updating, comprises:

determining if the beat is correlated to the template; and

updating the selected template with the correlated beat.

The method of claim 3, wherein updating the selected template further comprises combining the beat and the selected template by any method selected from point-by-point addition of the beat to the selected template, performing a running average of the beat and the selected template, performing a weighted running average of the beat and the selected template, and using a median filtering technique to combine the beat and the selected template.

5. The method of claim 3, further comprising incrementing a counter associated with the selected template each time the selected template is updated.

6. The method of claim 3, wherein determining if the beat is correlated to the template further comprises:

determining the fiducial points of the beat and the template, respectively;

aligning shock channel signals of the beat and the template;

identifying a plurality of features of shock channel signals of the beat and the template; and

using the plurality of identified features to determine if the beat is correlated to the template.

7. The method of claim 1, wherein characterizing the patient'"'"'s supraventricular rhythm using the particular template of the plurality of updated templates further comprises storing the particular template.

8. The method of claim 1, further comprising:

providing a plurality of counters associated with the plurality of templates;

incrementing one of the plurality of counters when the template associated with the counter is updated; and

using the template which has an associated counter with the highest count as a characterization of the patient'"'"'s supraventricular rhythm.

9. The method of claim 1, wherein updating the plurality of templates further comprises replacing one of the plurality of templates with a beat when the beat does not correlate to any of the plurality of templates and only one previous beat has been used to form the template being replaced.

10. A method of characterizing a patient'"'"'s supraventricular rhythm, comprising:

providing a first template and a second template;

detecting, using an implantable lead system, rate channel signals and shock channel signals of a plurality of beats;

determining if the beat signals meet predetermined criteria;

in a body implantable system, selectively updating a template based on a similarity between the template and a beat of the plurality of beats, comprising;

updating the first template using the beat if the first template is selected;

updating the second template using to the beat if the second template is selected;

counting a first number of beats used to update the first template;

counting a second number of beats used to update the second template;

identifying and storing the first updated template as a particular template characterizing the patient'"'"'s supraventricular rhythm if the first number of beats used to update the first template reaches a predetermined count before the second number of beats used to update the second template reaches the predetermined count; and

identifying and storing the second updated template as a particular template characterizing the patient'"'"'s supraventricular rhythm if the second number of correlated beats reaches the predetermined count before the first number of correlated beats reaching the predetermined count, the current template representing one beat of the patient'"'"'s supraventricular conducted beats.

11. The method of claim 10, wherein the predetermined count is about six.

12. The method of claim 10, further comprising updating one or the first or second templates using beats uncorrelated to the other of the first or second templates.

13. The method of claim 10, wherein:

providing the first template comprises retrieving a stored template and using the retrieved template as the first template; and

providing the second template comprises forming the second template using a beat which is uncorrelated to the first template.

14. The method of claim 10, wherein:

providing the first template comprises forming the first template using a first beat; and

providing the second template comprises forming the second template using a beat uncorrelated to the first template.

15. The method of claim 10, wherein selecting the template for updating comprises:

identifying a plurality of features of the first or second template;

temporally aligning the beat and the first or second template;

identifying a plurality of features of the beat;

determining if the beat is correlated to the first or second template using the plurality of features of the beat and the plurality of features of the first or second template.

The method of claim 15, wherein the plurality of features of the first or second template comprise values of the first or second template at a plurality of times t_i, where i=1 to N, and the plurality of features of the beat comprise values of the beat at the plurality of times t_i, where i=1 to N.

17. The method of claim 15, wherein:

updating the first or second template using the beat if the first or second template is selected comprises updating the first or second template by adding a value of the beat at each of the times t_j to a value of the first or second template at each of the times t_j, where j=1 to M;

counting the first number of beats used to update the first template comprises counting the beats correlated to the first template; and

counting the second number of beats used to update the second template comprises counting the beats correlated to the second template.

18. The method of claim 10, wherein storing the first updated template or the second updated template as the current template further comprises normalizing the first updated template or the second undated template.

19. The method of claim 10, further comprising replacing one of the first or second templates using the beat if the beat does not correlate to the first or second templates and no beats are correlated to the template.

20. The method of claim 10, further comprising replacing the first template with the beat if one or more beats are correlated with the second template and no beats are correlated with the first template and the first template was formed by retrieving a stored template.

21. A body implantable system for characterizing a patient'"'"'s supraventricular rhythm, comprising:

a lead system, the lead system comprising electrodes and extending into a heart;

a detector system, coupled to the lead system, that detects rate channel signals and shock channel signals of beats sensed by the lead system electrodes; and

a control system coupled to the detector system, wherein the control system provides a plurality of templates, determines if the beat signals meet predetermined criteria, selectively updates the plurality of templates using a plurality of beats, the control system configured to select a template for updating based on a similarity between the template and a beat of the plurality of beats and to update the selected template using the beat, the control system further configured to characterize a patient'"'"'s supraventricular rhythm by identifying a particular template of the plurality of selectively updated templates as representative of the patient'"'"'s supraventricular rhythm.

22. The system of claim 21, wherein the control system performs an automatic gain control adjustment for the shock channel signals prior to characterizing a patient'"'"'s supraventricular rhythm.

23. The system of claim 22, wherein the automatic gain control adjustment is performed by measuring a peak value in a predetermined number of regular beats and adjusting the shock channel gain such that an average peak value is a predetermined percentage of a maximum A/D converter value.

24. The system of claim 23, wherein the predetermined number of regular beats is four and the predetermined percentage of the maximum A/D converter value is 50%.

25. The system of claim 21, wherein the control system retrieves at least a first template from memory and forms one or more other templates using the beats.

26. The system of claim 21, wherein the control system

determines if the beat is correlated to the template of the plurality of templates;

and

updates the selected template with the beat if the beat is correlated to the template.

The system of claim 26, wherein the control system updates the selected template by combining the beat with the selected template by any method selected from point by point addition of the beat to the selected template, performing a running average of the beat and the selected template, performing a weighted running average of the beat and the selected template and using a median filtering technique to combine the beat and the selected template.

28. The system of claim 26, wherein the control system:

aligns a shock channel signal of the beat and the template;

identifies a plurality of features of the shock channel signal of the beat and the template; and

uses the plurality of identified features to determine if the beat is correlated to the template.

29. The system of claim 21, wherein the control system increments a counter associated with the selected template each time the selected template is updated.

30. The system of claim 21, wherein the control system

provides a plurality of counters respectively associated with the plurality of templates;

increments a counter of the plurality of counters when a template associated with the counter is updated; and

identifies the particular template as representative of the patient'"'"'s supraventricular rhythm based on a counter with the highest count.

31. The system of claim 21, wherein the control system replaces one of the plurality of templates using the beat when the beat does not correlate to any of the plurality of templates and no beats are correlated to the template being replaced.

32. A body implantable system for characterizing a patient'"'"'s supraventricular rhythm, comprising:

a lead system, the lead system comprising electrodes and extending into a heart;

a detector system, coupled to the lead system, that detects rate channel signals and shock channel signals of beats sensed by the lead electrodes;

a control system coupled to the detector system, wherein the control system

provides a first template and a second template;

determines if the beat signals meet predetermined criteria;

selects the first template or the second template based on a similarity between the first template or the second template and a beat of the plurality of beats;

updates the first template using beats similar to the first template;

updates the second template using beats similar to the second template;

counts a first number of beats used to update the first template;

counts a second number of beats used to update the second template;

identifies and stores the first updated template as a current template representative of the patient'"'"'s supraventricular rhythm if the first number of beats reaches a predetermined count prior to the second number of correlated beats reaching the predetermined count; and

identifies and stores the second updated template as the current template representative of the patient'"'"'s supraventricular rhythm if the second number of beats reaches the predetermined count prior to the first number of beats reaching the predetermined count.

33. The system of claim 32, wherein the predetermined count is six.

34. The system of claim 32, wherein the control system updates the second template using beats not similar to the first template.

35. The system of claim 32, wherein the control system retrieves a stored template and uses the retrieved template as the first template and forms the second template using a beat which is not similar to the first template.

36. The system of claim 32, wherein the control system provides the first template using a first beat and provides the second template using a second beat that is not similar to the first template.

37. The system of claim 32, wherein the control system performs an automatic gain control adjustment for the shock channel signals.

38. The system of claim 37, wherein the automatic gain control adjustment is performed by measuring a peak value in a predetermined number of regular beats and adjusts the shock channel gain such that an average peak value is a predetermined percentage of a maximum A/D converter value.

39. The system of claim 38, wherein the predetermined number of regular beats is four and the predetermined percentage of the maximum A/D converter value is 50%.

40. The system of claim 32, wherein the control system

identifies a plurality of features of the first or second template,

temporally aligns a qualified beat and the first or second template,

identifies a plurality of features of the beat,

determines if the beat is similar to the first or second template using the plurality of features of the beat and the plurality of features of the first or second template, and

updates the first or second template if the beat is similar to the first or second template.

41. The system of claim 40, wherein the plurality of features of the first or second template comprise values of the first or second template at a plurality of times t_i, where i=1 to N, and the plurality of features of the beat comprise values of the qualified beat at the plurality of times t_i, where i=1 to N.

42. The system of claim 40, wherein the control system updates the first or second template by adding a value of the beat at each of the times t_j to a value of the first or second template at each of the times t_j, where j=1 to M.

43. The system of claim 32, wherein the control system normalizes the first or second updated template before storing the first or second updated template as the current template.

44. The method of claim 32, wherein the control system replaces the second template using the beat if the beat is not similar to the first or the second templates and no beats are similar to the second template.

45. The method of claim 32, wherein the control system replaces the first template using the beat if one or more beats are similar with the second template and no beats are similar with the first template and the first template was formed by retrieving a stored template.

46. A body implantable system for characterizing a patient'"'"'s supraventricular rhythm, comprising:

a lead system, the lead system comprising implantable electrodes;

an implantable detector system, coupled to the lead system and configured to detect rate channel signals and shock channel signals of a plurality of beats sensed by the lead system electrodes; and

an implantable control system coupled to the detector system and configured to determine if the beat signals meet predetermined criteria, the control system comprising;

means for providing a plurality of templates;

means for selectively updating the plurality of templates using the plurality of beats comprising;

means for selecting a template for updating based on a similarity between the template and a beat of the plurality of beats; and

means for updating the selected template using the beat; and

means for characterizing the patient'"'"'s supraventricular rhythm by identifying a particular template of the plurality of selectively updated templates as representative of the patient'"'"'s supraventricular rhythm.
