1. A method for invasively treating a medical condition associated with a spinal joint within a body of a patient, comprising:
- positioning a directional ultrasound transducer at a location within the body of the patient and externally adjacent to a structure associated with the spinal joint; and
using the directional ultrasound transducer at the location externally adjacent to the structure, directionally delivering a therapeutic level of ultrasound energy in a first direction into a region of tissue associated with the structure of the joint from the location and without delivering a substantial level of ultrasound energy in a second direction opposite the first direction.

Abstract
An ultrasound therapy system and method is provided that provides directional, focused ultrasound to localized regions of tissue within body joints, such as spinal joints. An ultrasound emitter or transducer is delivered to a location within the body associated with the joint and heats the target region of tissue associated with the joint from the location. Such locations for ultrasound transducer placement may include for example in or around the intervertebral discs, or the bony structures such as vertebral bodies or posterior vertebral elements such as facet joints. Various modes of operation provide for selective, controlled heating at different temperature ranges to provide different intended results in the target tissue, which ranges are significantly effected by pre-stressed tissues such as in-vivo intervertebral discs. In particular, treatments above 70 degrees C., and in particular 75 degrees C., are used for structural remodeling, whereas lower temperatures achieves other responses without appreciable remodeling.

2. A method for treating a medical condition associated with a spinal joint within a body of a patient, comprising:

positioning a directional ultrasound transducer at a location within the body of the patient and externally adjacent to a structure associated with the spinal joint; and using the directional ultrasound transducer at the location, directionally delivering a sufficient level of ultrasound energy in a first direction into a region of tissue associated with the structure of the spinal joint that is sufficient to necrose nociceptive nerve fibers or inflammatory cells in such tissue region without substantially affecting collagenous structures associated with the structure, and without delivering a substantial level of ultrasound energy in a second direction opposite the first direction.

3. A method for treating a region of tissue associated with an intervertebral disc in a body of a patient, comprising:
positioning a directional ultrasound transducer to a location within the body externally adjacent to the intervertebral disc such that a therapeutic level of ultrasound energy may be directionally coupled from the transducer in a first direction into the region tissue from outside of the intervertebral disc, and such that ultrasound energy will not be delivered in a second direction opposite the first direction.

4. A method for treating a patient, comprising:

positioning a directional ultrasound transducer at a location within the patient'"'"'s body; delivering directional ultrasound energy in a first direction from a directional ultrasound transducer at a location externally adjacent to a structure associated with a spinal joint and such that the directional ultrasound energy is directed from the location into a region of tissue associated with the structure and without delivering substantial energy in a second direction that is opposite the first direction; and controlling the directional ultrasound energy delivery such that the tissue receiving the directional ultrasound energy does not experience a rise in temperature of to more than about 55 degrees C.

5. A method for treating a patient, comprising:

directionally delivering energy from a directional ultrasound transducer positioned at a location externally adjacent to a structure associated with a spinal joint and at a location within the patient'"'"'s body, such that the energy is directed in a first direction into a region of tissue associated with the structure and without delivering a substantial level of energy in a second direction opposite the first direction; wherein such directional energy delivery is at a thermal dose within a range that is between about 10 and about 300 equivalent minutes at about 43 degrees C.; and wherein such directional energy delivered in a manner that does not substantially remodel collagen in the structure.

6. A method for invasively treating a medical condition associated with an intervertebral disc within a body of a patient, comprising:
directionally delivering a therapeutic level of ultrasound energy in a first direction into a region of tissue associated with an intervertebral disc from a directional ultrasound transducer positioned at a location within the body of the patient and externally adjacent to the intervertebral disc and without delivering a substantial level of ultrasound energy from the directional ultrasound transducer in a second direction that is opposite the first direction.
